As your employer, we recognize the value of invesng in a comprehensive Wellness Program which promotes healthy lifestyles and builds a culture of wellness through educaonal opportunies, wellness acvies, and support for self-improvement through healthy lifestyle choices. The goals of our wellness program are to decrease the risk of disease and to enhance the quality of life of our employees. For this reason, insured employees can parcipate in the LCIC Wellness Points Bank. You can earn points by parcipang in a variety of health and wellness acvies. When you earn enough points in your bank, you will be eligible for an incenve of $150. The LCIC Wellness Points Bank program will start on September 1st, 2013 and run through August 1 st , 2014. Each acvity you parcipate in will earn you points. You must earn 450 points in order to receive your $150 incenve. Parcipaon in this program is completely voluntary; however employees must follow the steps outlined in order to be eligible for the 2014 incenve. All acvies must be completed by August 1 st , 2014. Checks will be cut quarterly. If you meet all criteria then you may receive your check before the school year 2014-15. How to Earn Points for your Wellness Bank 1. Health Risk Assessment (HRA) You must complete an online HRA through Blue Health

solutions. HRA’s must be completed by Jan 15th, 2014. Point value-100

2. Biometric Health Screening Must be completed by January 31 You must participate in the

free health screening offered through the workplace which includes height, weight, Blood

Pressure, BMI, and a Cholestech finger stick which includes a full cholesterol panel and glucose

results. (All tests must be performed in order for you to get points.) Point value=100

3. Health Coaching-Calls must be completed by August 1, 2014. Coaching for a medical condition,

disease management or lifestyle changes. Must complete a minimum of 4 calls through Blue

Health Solutions. Calls must be 30 days apart. Point value=100

4. Proof of annual physical exam through a physician’s office (Insurance will pay for only one

physical yearly). Must fill out annual physician physical exam form and turn it into Carmen Terry,

Health and Wellness Coordinator or you can turn in the Blue Cross NEPA physical form to BCNEPA.

(If you received an annual physical in August of 2013, it will be accepted as long as form is turned

in.) Point Value = 100

5. Non-Tobacco User Pledge-If you are enrolled through Blue Health solutions, you may request

the tobacco affidavit form to be filled out and mailed back. Forms will be mailed to you in January

from BCNEPA. Forms must be turned in and received no later than May 1, 2014. You may also

send a copy to Carmen Terry. Point value=25

6. Weight Management program-Weight Watchers at Work or Community meeting, must show

proof of active member. Minimum attendance-14 meetings or if participating in the Blue Health

Solutions weight management you must complete 6 calls with a registered dietician which must

be 30 days apart. Other certified weight management programs must be able to show proof of

consistent attendance or participation. Verification must be turned into Carmen Terry, Health and

Wellness Coordinator by August 1, 2014 for point value to be awarded. Point value=100

7. Enrollment on the Spirit of Women Program through SHS. Point Value=25

for enrollment only. Participating in a monthly event or Signature Event= 10 point each

(Maximum number of 3 events for 30 points) Total points allowed in this category is 55.

8. Participating in a 5k Walk/Run through the YMCA or other programs. Point Value= 20

(Maximum number of points in this category is 80)

9. Annual Flu shot-Verification form must be turned into Carmen Terry, Health and Wellness

solutions program must have 6 verified calls which must be 30 days apart. Point Value=75

must fill out the dental verification form and turn it into Carmen Terry. Point value= 25

for each preventative cleaning (maximum of 50 points)

15. Annual Preventative Vision Exam-Verification form must be turned into Carmen Terry, Health and Wellness

Coordinator by August 1, 2014. (Only one visit allowed for points per incentive year) Point Value= 25

16. Educational session- Sponsored by LCIC Health and Wellness. Dates and times TBD during the year. Point

value is per session. Must be signed in to attend session. Point Value=10 .

17. f you meet 3 out of 4 benchmarks outlined below please provide us with a verification statement from your

physician or you may use your verification through your health screening, provided that you have signed the

waiver form. Verification cards will be available on the health and wellness website. Point Value =75

*Please keep track of all points in the online tracking system at shapeuptracker.org.

You must register in the system if you did not participate in the Employee Wellness

Challenge last spring. Please use your email address from school and your school

password when registering. Then you must enroll in the 2013-14 Wellness Points

Bank Program. All points values are already loaded in the system.

Body Mass Index 26.0 or less

Blood Pressure 130/85 or less

Cholesterol Level 200 or below

How to Earn $150 from your Bank

Once all the criteria are met for your 450 points, step

2 is listed below. Please understand that you will

receive only one check for each year of the program,

so if you would like the full amount of $150, please

wait until you have receipts for the total amount.

You may turn in receipts at any time, but if it is less than the $150 you will

only receive a check for the amount you turn in. Please turn in your receipts

and receipt form to Carmen Terry, Health and Wellness Coordinator at PO Box

3609, Williamsport, PA 17701. Please mark confidential on the envelope.

*You may not use receipts for copays if you have used your 125 or Flexible

spending plan to pay for any of these items listed below.

Eligible expenses for health and wellness are:

1. Gym membership, personal training sessions, or fitness classes

2. Weight watchers or other certified weight loss program (may

not be used for in-store weight loss products)

3. Tobacco cessation medication

4. Copays*-health visits and prescription medication Physical

therapy

5. Massage therapy

6. Acupuncture

7. Exercise equipment (such as kettle bells, physioball,

dumbbells, cardio equipment, adult bike, running shoes)

Dining with Diabetes Dining with Diabetes is a program

offered by Penn State Cooperative Extension. It will help you to understand some of the most important things you need to know about how to manage your diabetes.

Classes are offered weekly for five

weeks, with a three-month follow-up

class. Each class offers food

demonstrations and tastings, physical

activity, ideas to take home, and

discussions regarding important

information to help you manage your

diabetes. Lab tests will be offered to

measure A1c.

In addition to class handouts and test

results, participants also receive a

variety of resources, including a

complete collection of Dining with

Diabetes recipes. If you have been

told that you have Type 2 diabetes,

you will want to attend this class.

Dining with Diabetes is an approved

research project conducted by Penn

State University. Participation is

voluntary.

Growing Stronger

A Strength Training Program for Women and Men

Make a commitment to grow stronger and trimmer and feel better about

yourself! Growing Stronger is a safe, simple, and highly effective strength

training program.

Who should attend?

Women and Men who have been active or relatively sedentary for the past

few years and are interested in improving their health, vitality and

well-being.

Strength training offers physical,

mental, and emotional benefits:

increased muscle mass and strength

improved bone density

reduced risk for osteoporosis and related

fractures

reduced risk of diabetes, heart disease,

arthritis, depression, and obesity

improved self-confidence, sleep, and

vitality

improved balance
