Coverage for your medicinesHERE ARE ANSWERS TO THREE COMMON QUESTIONS

1. Is my prescription covered?You can see if your prescriptions are covered by searching the 2018 Medicare Formulary. This is the list of medicines covered by your plan. Go to healthpartners.com/medicarerx and search by:

• The name of your medicine (ex. simvastatin)• The type of medicine (drug category) you're taking (ex. cholesterol)• The first letter of the medicine you're taking (ex. ‘S’)

2. How much do I have to pay for my medicines?You pay $10 for preferred generic and generic, $30 for preferred brand, $50 for non-preferred brand and $50 for specialty drugs.You can find more benefit details by looking at your Summary of Benefits.

3. What if my medicine isn't on the list?If your medicine isn't on the list, it's not covered by your plan. In that case, we may suggest alternative medicines listed on the formulary.If you want to switch to a covered medicine, we can help you see your options. Go to healthpartners.com/medicarerx and choose "Search the Medicare drug list." Then, search for the type of medicine you're taking. A list of covered medicines will come up. Print it out and bring it to your doctor to see if one of those will work for you.

Save time and moneyGet your medicines your way with HealthPartners mail-order. Your prescriptions are delivered right to your door. There's no additional cost and shipping is free. Typically, you can expect to get your medicines within five to eight business days of when the mail order pharmacy receives your order.Log on to healthpartners.com/mymailrx to sign up. Your plan pays for some great thingsWith the HealthPartners® Dental Open Access plan you get:

• Preventive dental services covered 100 percent

• Extra exams and cleanings covered 100 percent if you have diabetes and are at risk for gum disease

• Help covering the cost of other dental care at the amounts shown in your Summary of Benefits (SOB)

It works like thisPreventive care is covered at no cost to you when you see a network dentist. Extra work, like getting a cavity filled, might cost a deductible (an amount you have to pay before your plan helps with the cost) or coinsurance (a percent of the bill). Check your SOB for your specific amounts.And remember that your dental plan has an annual maximum. It’s a bit different than your medical plan. Your dental plan max is the most your plan will pay for dental care each year. You’re in charge of the rest.

You pick your dentistWith the largest network of dentists and clinics, you’ve got lots of choices. Go to healthpartners.com/dentalopenaccess to find one.

SAVINGS TIP! You’ll pay less if you see a dentist in the Open Access network. Out-of-network dentists cost more.

There’s a connectionPeople who are diabetic are at greater risk of gum disease. And gum disease can make it harder to manage your diabetes.

You have free helpIf you’re living with diabetes and are at risk of gum disease, your dental plan covers 100 percent of services to help control or prevent gum disease. That includes root planing and scaling. Think of those services as a deep cleaning for your teeth. All other dental services, like a cavity and root canal, are covered according to your Summary of Benefits (SOB).

Here’s how it worksIt’s easy to get the care you need to stay healthy:

• Visit a network dentist• Pay nothing, not even a deductible or

coinsurance for certain services• Relax, your plan will pay even if you’ve
