Join us in Sun Valley for the 121st edition of the Idaho State Dental Association’s Annual Session “Healthy Patients, Healthy Team, Healthy Practice” June 21-23, 2017. Dr. John Hisel, our executive committee and staff have worked to put together an outstanding lineup of speakers and events in response

to the needs and requests of our membership. This year’s meeting will be held at the iconic Sun Valley Resort, home to world-class venues and accommodations.

Golfers, don’t miss the Fifth Annual Component Cup on Wednesday at the scenic and unique layout of the Sun Valley Trail Creek course. Come join us and see if your team can take home the coveted cup!

This year’s exhibit hall will be limited to only our select vendors. These vendors are prepared for a successful time showcasing their products and services, and I ask that you come ready to spend some quality time and support

them. A great way to make that happen is to sign up for Lunch in the Hall on Thursday, and join us for the social Thursday after class.

New Dentists – be sure to register for the New Dentist lunch on Thursday. And to our Women Dentists, a special invitation to a reception on Thursday evening where we’ll be launching a new group just for you. All members are encouraged to attend the ISDA Member luncheon on Friday.

The President’s Dinner will be held on Friday night. We will be “Rocking in the Sawtooths” with great food, music and socializing. This evening always proves to be a fun time that you won’t want to miss! Come on out and let your hair down with all of our friends from around the state.

Brooke Fukuoka, DMD:Dr. Brooke MO Fukuoka completed her undergraduate studies at Idaho State University, and her DMD at the University Of Louisville School Of Dentistry, followed by a hospital based General Practice Residency at the University of Louisville hospital. She believes strongly in organized dentistry and is a member of: the American Dental Association,

the Idaho State Dental Association’s New Dentist Committee, the Special Care Dental Association, the Academy of General Dentistry, and a current participant in the American Dental Association’s Institute for Diversity in Leadership.

Currently Dr. Brooke practices full time with Family Health Services in Twin Falls, ID where she provides conservative preventative focused care to a diverse array of patients. She also has started her own practice, Your Special Smiles, PLLC providing access to care for adults with special needs in South Central Idaho.

Richard E. Ferguson, DDS, MS:For the past thirty-four years, Dr. Richard E. Ferguson has provided compassionate, state of the art periodontal care. He practices at Treasure Valley Periodontics in Boise, along with Dr. Kathleen Beaudry and their dedicated staff. Treasure Valley Periodontics is a leader in dental implants, cosmetic dental surgery and periodontal therapy.

Dr. Ferguson earned both his DDS and MS for postgraduate training in periodontics from the University of Michigan.

Throughout his career, Dr. Ferguson has served and led his profession. He was President of the Idaho State Dental Association in 2009-2010, and currently serves as the Treasurer of the Idaho Dental Foundation. Dr. Ferguson is a Fellow in both the International College of Dentists, as well as the Pierre Fauchard Academy.

Hilary Dunstan, DDS, MS:Dr. Hilary Dunstan was born and raised in Boise, ID and has practiced endodontics in Boise since 2006. She attended Boise High School and then earned her Bachelor’s Degree with honors from Gonzaga University in Spokane, Washington in 1998. She completed her Doctorate of Dental Surgery at Creighton University in Omaha, Nebraska in 2003. Dr.

Kevin S. Douglas, DMD, MSDr. Douglas is a board certified orthodontist practicing in Meridian, Mississippi. He earned his Doctor of Dental Medicine at the University of Alabama at Birmingham. He completed his orthodontic residency at Nova Southeastern University in Florida and completed a hospital residency at the University of Mississippi Medical Center in Jackson in 2005.

Before starting in private practice, Dr. Douglas served as the Co-Director for the Advanced Education in General Dentistry at Nova Southern University. Dr. Douglas is very active in his community, sponsoring local high schools and funding multiple youth projects in Meridian’s community service centers. He is an avid snowboarder, surfer and motorcyclist who enjoys traveling the country and abroad. When not in the office, he can usually be found playing with his three children. Sponsored by: ISOS

Dunstan graduated from Creighton with honors and also received numerous awards upon graduation. She then attended the University of Southern California and completed a one year program in Advanced Education in General Dentistry.

Dr. Dunstan attended the University of Louisville for specialty endodontic training. There she received training in the latest techniques and technology including use of the operating microscope and digital radiography. Upon completion of the program in 2006 Dr. Dunstan was awarded both a Certificate in Endodontics as well as a Masters in Oral Biology.

M. Lynn Gurney, DDS, MS:Dr. Gurney is an Idaho native and graduate of Emmett High School, BYU-Idaho, and Creighton University Dental School. After dental school, he completed a three year Advanced Prosthodontics Residency Program at Ohio State University where he obtained a certificate in Advanced Prosthodontics and a Master’s of

Science for research with clinical implant dentistry. “During my training I found that I really enjoy learning and sharing that experience and information with others to improve outcomes. I believe working together makes our profession more enjoyable and more successful. My real passion in life is my wonderful family. I’ve been married to my wife Erin for 13 years and we have 4 wonderful children; Porter, Peyton, Clara, and Preston.”

GOLF REGISTRATIONPlay for the Component Cup again in 2017. Get together your Component team and win back (or keep) bragging rights from SWIDS, last year’s winners. Sun Valley’s signature course, Trail Creek, promises to provide a challenging round of golf for every level of player. Coupled with fantastic views of the surrounding mountains and ever-present crystal skies, this will be a great way to kick off the 2017 Annual Session.

Game format will be a shamble. $190 Registration fee includes greens fee, cart, and box lunch. Caddy tip and club rentals are not included.

Late and On-site RegistrationAll Late Registrations (after May 8) will be charged an additional $25 per person late fee. All On-site registrations will be charged an additional $50 per person registration fee.

Member RegistrationFree registration to the ISDA Annual Session is a member benefit for ISDA Member Dentists, ISDA Member Hygienists and ISDA Allied Member Staff. All of these memberships are individual memberships.

To register as an ISDA Member Hygienist or an ISDA Member Allied you must have become a member with the ISDA for 2017 before April 1st. Contact the ISDA for more membership information at 208-343-7543 or complete a member application at www.TheISDA.org/become-a-member/.

Registration MaterialsConfirmation of registration will be emailed to the primary registrant after processing. Registration packets will be available for pick up at the ISDA Registration Desk in the Sun Valley Inn.

Class & Events RegistrationAll classes and events are ticketed, and some have limited space. Each attendee is required to sign up for all classes and events they wish to attend. Event tickets will be included with each attendee’s registration materials.

Course HandoutsCourse Handouts will be made available online prior to the conference. Handouts may also be viewed through the ISDA Mobile App.

Course CreditThe number of continuing education credits for each course is equal to the duration of the course in hours. CE completion certificates will be made available online at www.TheISDA.org after the session. It is the participant’s responsibility to verify the requirements of his or her state licensing board and to self-report any CE credits earned.

LodgingLodging in Sun Valley is available at discounted rates for participants until May 19th at the Sun Valley Resort by calling 1-800-786-8259 or emailing [email protected]. Be sure to let them know you are with the Idaho State Dental Association to receive our special discounted room rates. The deadline to receive the ISDA room rates is May 19th so be sure to book early!
