Medica Superspecialty Hospital in Kolkata has announced a levy of charges on medical representatives visiting its facility. The hospital has restricted the visit of a medical representative to only one day a week against a charge of Rs 350/will be levied. A hospital in Delhi is already doing this, charging Rs 5000 per month. The decision was aimed at discouraging MRs from visiting the hospital. The plea given is that the hospital wants to promote generic drugseMedinews does not agree with this. It’s like taking away the freedom of the doctors. MCI ethics regulation 6.8 allows a doctor to associate with pharma

work for pharmaceutical and allied healthcare industries in advisory capacities, as consultants, as

researchers, as treating doctors or in any other professional capacity. In doing so, a medical practitioner shall always:-

i. Ensure that his professional integrity and freedom are maintained. ii. Ensure that patients interest are not compromised in any way.

iii. Ensure that such affiliations are within the law. iv. Ensure that such affiliations/ employments are fully transparent and

disclosed.”

The MCI Ethics code does not restrict an ethical interaction between a doctor and a medical representative. The MCI Ethics Code also does not restrict academic gifts, CDs, scientific updates from pharma representatives as long as the ‘per time’ cost is less than 1000/-. Regulation 6.8.1 a has defined punishment for Gifts more than Rs. 1,000/- up to Rs. 5,000/- as ‘censure’.

allopathy IMA claims it is awaiting government decision on Monday on the NMC bill. Representatives from IMA have been meeting Union health department officials to oppose the inclusion of Unani doctors in the bridge course. Demanding that a bridge courses be allowed for Ayurvedic and homeopathic doctors under the National Medical Commission (NMC) Bill, doctors attached with All India Homeopathic Doctors Federation (AIHF) on Wednesday carried out a protest rally from JJ Hospital to Azad Maidan. The protest comes after Indian Medical Association’s (IMA) protest to remove the clause from NMC Bill that allows a bridge course for homeopaths, Ayurved and Unani doctors to prescribe allopathic medicines. IMA has threatened a nationwide protests if the parallel form of medicine is allowed to combine with allopathy. According to Dr Prakash Rane, attached with AIHDF, the protests have been ongoing since last three days. At least 500 doctors are part of the protest in Mumbai. IMA claims it is awaiting government decision on Monday on the NMC bill. Representatives from IMA have been meeting Union health department officials to oppose the inclusion of Unani doctors in the bridge course. Even as IMA has opposed the government’s intentions to allow Ayurvedic and homopathic doctors to prescribe allopathic medicines, under a project to upgrade sub-centres, public health department has already started posting homeopathic doctors to run health and wellness centres in Maharashtra.

Occupational exposure to loud noise increases risk

of heart disease Noise-induced hearing loss is the most obvious outcome of exposure to loud noise. Now a study says that high blood pressure (BP) and high cholesterol are also more common among workers exposed to loud noise at work. Researchers from CDC’s National Institute for Occupational Safety and Health (NIOSH) examined the prevalence of occupational noise exposure, hearing difficulty and heart conditions within industries and occupations in the US as well as the association between workplace noise exposure and heart disease.

1. 25% of current workers were found to have a history of work-related noise exposure.

2. 12% of current workers had hearing difficulty, 24% had high BP and 28% had high cholesterol levels.

3. 58% of hearing difficulty, 14% cases of high BP and 9% cases of high cholesterol in the study could be attributed to noise at work.

The study was published online March 14, 2018 in the American Journal of Industrial Medicine. Noise is an unwanted intrusive sound. A loud noise is 85 db or higher, or if a person has to raise his/her voice to speak with someone standing at a distance of 3 feet. Noise is a recognized environmental stressor, which has both physiological and psychological effects. It is associated with anxiety, high blood pressure, increased heart rate, insomnia, annoyance, stress. Progressive hearing loss may result from continuous and repeated exposure to loud noise. The safe limit for sounds at 85 db or less is 8 hours of exposure. Loud noise affects speech intelligibility and consequently work performance and increases chances of errors. Conversation has to be conducted at higher dbs for clear speech communication because of noise interference (normal conversation is between 60-70 db). The Central Pollution Control Board has recommended noise standards during day time (between 6 a.m. and 9 pm) and night time (between 9 pm and 6 am) as follows: for industrial areas (75 db day time and 70 db night time), commercial areas (65 db day time and 55 db night time), residential areas (55 db day time and 45 db night time) and silence zones (50 db day time and 40 db night time). Areas around hospitals, educational institutes and courts are silence zones. Hospitals are noisy work places. Control of noise levels is very important in hospitals for patient well-being and healing. Noise creates an unhealthy work environment for doctors. It affects concentration and increases the chances of mistakes, which can be costly for the doctors and hospitals. Inability to hear the warning patient monitoring alarm over the general background noise in an ICU may have potentially disastrous outcome. Moreover, doctors too are prone to develop high BP and other negative effects on health.

World Health Day: Universal health coverage: everyone, everywhere

The World Health Day is observed annually on April 7 to raise awareness about global health issues. The theme for the year 2018 is “Universal health coverage: everyone, everywhere”. Key World Health Day messages

1. Universal health coverage is about ensuring all people can get quality health services, where and when they need them, without suffering financial hardship.

2. No one should have to choose between good health and other life necessities.

3. UHC is key to peoples’ and nations’ health and well-being. 4. UHC is feasible. Some countries have made great progress. Their

challenge is to maintain coverage to meet people’s expectations.

5. All countries will approach UHC in different ways: there is no one size fits all. But every country can do something to advance UHC.

6. Making health services truly universal requires a shift from designing health systems around diseases and institutions towards health services designed around and for people.

7. Everyone can play a part in the path to UHC, by taking part in a UHC conversation. (WHO)
