The Healthy Rewards program is designed to encourage positive lifestyle choices for employees and their families. You are rewarded with Healthy Reward incentives, paid as payroll credits, for taking healthy steps.

To qualify for Healthy Reward payroll credits, you must be enrolled in one of the company’s medical plans.

How to Earn Healthy Rewards The first step to earning Healthy Rewards is to complete a brief, confidential Personal Health Assessment. It is only 20 questions and your results populate a personal scorecard, which rates different areas of your health and provides individual guidelines for improvement. The Personal Health Assessment is easy to access, and you choose how to take it — by computer, smartphone or tablet.

You can receive up to $500 in Healthy Reward payroll credits when you complete the Personal Health Assessment and qualifying activities between October 1, 2016 and September 30, 2017.

When your covered spouse or domestic partner also takes the Personal Health Assessment and participates in qualifying activities, you can earn up to an additional $500. Other covered dependents, in addition to your spouse or domestic partner, can also earn rewards up to $500. They do not need to complete a Personal Health Assessment; however, they will not earn Healthy Rewards unless you have completed your Personal Health Assessment.

The maximum Healthy Rewards you and your covered dependents can earn in total is $1,000.

The chart that follows shows the activities in which you can participate. To complete and track your activities, go to Incentives > Summary.

Your covered spouse/domestic partner or covered dependents, age 18 and over, can visit the Healthy Rewards Web site by selecting Resources for U.S. Spouses/Domestic Partners on the MyTycoHR.com login page. Upon the initial visit, he or she will need to register as a first-time user. You will not see activities completed by your spouse/domestic partner in your own Healthy Rewards Incentive box.

When You Will Receive Payroll Credits Healthy Rewards payroll credits are paid at the beginning of each quarter (January, April, July and October). You can earn up to half of the Healthy Rewards payroll credits by December 31, 2016 and the remainder from January 1 to September 30, 2017. Or, you can earn all of your credits after December 31, 2016.

To earn Healthy Rewards, you (and your spouse/domestic partner if he or she chooses to participate) must meet the eligibility criteria for the program, including completion of the Personal Health Assessment between October 1, 2016 and September 30, 2017. If you believe you are unable to complete some of the activities associated with Healthy Rewards due to a health condition, we will assist you in finding alternative qualifying options. Please contact Employee Access at 1-800-924-8518 for additional information.

Although the company offers the Healthy Rewards program to help employees manage their health and wellness, the company will not receive any individually identifiable personal health information. We will only review results on an aggregate level to evaluate program results. Healthyroads, our wellness provider, may use and/or provide participation information to the company or its contracted entities that administer your plan for incentive fulfillment purposes. Healthyroads may also use this information to provide you with other services on behalf of the company. Your participation serves as your consent for Healthyroads to use and/or provide this information.

Benefits eligibility and plan provisions for employees covered under a collective bargaining agreement are specified in the union contract.

Go to MyTycoHR.com > Health & Group Benefits > Enrollment Center

Once you review your medical options on this tool, select the Summary page. In the “More info on medical coverage” section toward the bottom, you will see a code to enter in the Healthy Rewards Incentive Plan page.
