Introduction1 Eachyearthereareaccidentsanddangerousoccurrencescausingseriousandsometimesfatalinjuriestooperatorsofhightemperaturedyeingmachines,aswellasdamagetoplant.
2 Forthepurposesofthisguidancenoteahightemperaturedyeingmachineisonethatoperatesattemperaturesabove100°Candpressuresaboveatmospheric.
3 Dyevesselscontainingsteamorhotwaterabove0.5bargpressure(ieover110°C)willbesubjecttothePressureSystemsandTransportableGasContainersRegulations1989(PSR).1-5TheRegulationsaredesignedtoreducetheriskofseriousinjurycausedbytheunintentionalreleaseofstoredenergywithinapressurevesselandtoreducetheriskofscaldingfromsteam.
4 ThisguidancenoteisissuedbytheHealthandSafetyExecutive(HSE)andaimedatsuppliers,operators,andthoseconcernedwithmaintenanceandthoroughexaminationofhightemperaturedyeingmachines.Itdrawsattentiontovarioushazardsanddescribeswaysinwhichthosehazardscanbereducedoreliminated.
Dangers5 Themajorityofaccidentsatdyeingmachinesarecausedbyexplosionsorscalding.Forexample:
(a) Pressurevesselsorcomponentssuchasopeningcovershavefailedwhile underpressure,duetomechanicalfailureorfaultyclosing.(b) Vesselshavefailedduringbleachingoperationsusinghydrogenperoxide(H2O2). DecompositionofH2O2hasledtotherapidevolutionofoxygenwhichhasbeen beyondthecapacityofthereliefdevices,causingthevesseltofailcatastrophically.(c) Operatorsmaybeengulfedwithsteam,boilingliquid,orhotwater,when: (i) thetemperatureofdyeingliquorisaboveorabout100°Candalidor samplechamberisopened,allowingliquortoflashviolentlyintosteamand eruptfromthevessel; (ii) doorsareopenedandhotliquorallowedtospillout,egfromhorizontal vesselsnotcompletelydrainedorverticalvesselsfilledabovecover-flange level; (iii) oneofabankofmachinesisinadvertentlypressurisedfromanadjacent machine,viaacommonblow-downordrainline.
Design safeguardsIntegrity of the vessel
6 Designers,manufacturers,importersandsuppliersofhightemperaturedyeingmachineshaveadutyunderthePSRtopreventdangerbyensuringthateverypressurevesselisproperlydesignedandconstructedtoprovideanadequatemarginofsafetyforthedutyithastoperform.TheuseofappropriatestandardssuchasBritishStandardBS5500:1994Specification for unfired fusion welded pressure vessels6isrecommended.Thevesselshould:

(a) beproperlyconstructedfromsuitablematerial;(b) permitaccesstoconductexaminationsforpreventingdanger(egnon- destructivetestingofwelds);(c) havemarkedonitthesafeworkingpressureandcorrespondingtemperature;(d) haveacorrectpressuregauge;(e) haveasuitablesafetydevice(safetyvalveandorburstingdisc)havingsuch capacityandbeingsoadjustedastopreventthevesselfrombeingover- pressurised.(Asafetydevicefittedtoavesseldesignedfordyeingmaynot havesufficientcapacityforbleachingwithhydrogenperoxide-seeparagraphs 36to38.);(f) haveitssupplypipefittedwithasuitablereducingvalve,orothersuitable automaticdevice,topreventthesafeworkingpressurebeingexceededif pressureissuppliedfromanoutsidesource,egsteamorairatpressuregreater thanthesafeworkingpressureofthevessel.
Multi-bolted doors
7 Anoperatorisunlikelytoreleaseallboltsonamulti-bolteddoorunderfullpressurewithoutbeingwarnedofdangerbyagradualleakageofsteamorliquidduringtheslackeningofthebolts.However,whenthevesselisunderslightorresidualpressure,thereisadangerofdoorsstickingafteralltheboltshavebeenremovedandthedoorthenbeingviolentlyblownopenwhenfreed.Tofacilitatethesafereleaseofanyresidualpressure,thedoorshouldbefittedwithadevicetoeitherbreakthesealortorestrainthedoorfromopeningmorethan3mm.Typically,aswingboltmaybefittedoppositethehingeinwhichitstrappednutisrestrainedbythedoorhousinguntilthesealisbrokenandthedooriscrackedopenedby3mm.

Quick-opening doors (other than multi-bolted doors)
8 Pressurevesseldoorsofthequick-openingtypeshouldbefittedwiththefollowingdevices.
(a) Aninterlockbetweenthedoor-lockingmechanismandthepressurisingsystem, eg (i) Ifpressureisappliedbysteam,liquid,airorothergases,theinletvalve shouldbeinterlockedwiththedoor-lockingmechanisminsuchamanner astopreventthevalvebeingopenedtopressurisethevesseluntilthedoor isfullyclosedandlocked. (ii) Ifthepressureisappliedbyboilingtheliquoruntilsteamisformed,then thereshouldbeaninterlockwhichpreventsheatbeingapplieduntilthe doorisfullyclosedandlocked.Theinterlockshouldalsoensurethatthe sourceofheatisturnedoffbeforethedoorcanbeunlocked. (iii) Ifthevesselispressurisedbyanexternalpumpthenthereshouldbean interlockwhichpreventsthepumpfromapplyingpressureuntilthedooris fullyclosedandlocked.Theinterlockshouldalsoensurethatthepumpis switchedoffbeforethedoorcanbeunlocked.
(b) Aninterlockbetweenthedoor-lockingmechanismandthemainvent,arranged sothattheventmustbeopenbeforethedoorcanbeunlocked(butitcanbe closedoncethedoorisclearoftheseal).Note:Itissometimesnecessaryto carryoutnon-pressurisedorlowtemperaturedyeingwiththedoorofthe vesselopenbutitshouldnotbepossibletocommenceahightemperature dyeingcyclewiththedoorpartiallyclosed.(c) Atest-cock,unlessthemainventoutletisclearlyvisibletotheoperativeatthe vesseldoor.Thetest-cockshouldbesituatedatalevelbelowthevesseldoor andsointerlockedthatithastobeopenedbeforethedoorcanbeunlocked. Thetest-cockshouldgiveaudibleorvisualindicationastowhetherthevessel isstillpressurisedorhashotliquidabovethedoorlevel.Theboreofthe test-cockshouldbelargeenoughtoavoidblockage,typicallyatleast12mm.

(d) Arestraint,toholdthedoorafterithasbeenunlockedandtopreventit openingviolentlybyanyresidualpressureremaininginthevesselnotindicated eitherbythetest-cockorpressuregauge,egacatchwhichhastobeopened independentlyafterthedoorhasbeenunlockedandwhichmaynotbe releaseduntilthedoorhasbeenunlockedandcrackedopen.Alternatively,a hydraulicorpneumaticdamperorram,designedtoactasaneffectiverestraint tothedoormovement,maybeused.(e) Atemperaturesensitiveinterlock,topreventthedoorbeingopenedwhilethe temperatureoftheliquorinsidethevesselexceeds80°C.Thetemperature sensingprobeofsuchaninterlockshouldbelocatedinapositionsothatit normallyregistersthehighesttemperatureinthesystem.Thedeviceshould fail-safe,ieiftheinterlockisnotinplace,thenthetemperaturemaynotexceed 80°C.Wherethenormalflowofliquorisdisruptedordistorted,egbyfailureof pumpormotor,orbyjammingofvalvesorentanglementoffabric,then extremecareisnecessarytoensurethatthewholeofthemachineandits contentsarecooledtoasafetemperaturebeforethevesselisopened.
Interlocks
9 Whileitispreferableforinterlockingsystemsondyeingmachinestobesimpleandmechanicalinoperation,itissometimesimpracticabletoachievethis.Aninterlockingkeysystemisoftenappropriate,egonsamplingdevices.
10 Allinterlockingsystems,whethermechanically,electrically,pneumaticallyorhydraulicallyoperated,shouldfail-safeintheirmodeofoperation.

11 Whensafetydependsuponelectricalsystems,andnootherback-upsafetydevicesareprovided,integritycanbeimprovedbyfittingtwolimitswitchesdesignedtooperateinoppositemodes.Whereelectricalinterlocksareusedforsafetypurposesthecircuitshouldbecompletelyindependentofanyothersystem.AdviceontheapplicationofelectricallimitswitchesisgiveninBSEN1088:19967andBS5304:1988.8Limitswitchesmountedonahightemperaturedyeingmachineoperateinanadverseenvironmentofheatandmoisture.Topreventmoisturegettingin,itisrecommendedthatswitchesfittedhaveenclosuresthatachieveaminimumratingofIP55asdefinedinBSEN60529:1992Specification for degrees of protection provided by enclosures.9
Sampling device
12 Numerousaccidentshaveoccurredwhileoperatorshavebeentakingsamplesfrompressuriseddyeingmachines.Thereareseveraldifferentmethodsofsamplingbutanysamplingdeviceshouldbeinterlockedtoensurethatthesamplingpotordeviceisdepressurised(opentodrain)beforeitcanbeopenedtoremovethesample.Itisimportantthatdesignerscarefullyconsiderthesamplingmechanismtopreventforeseeablemistakenactionsbyoperatorstakingsamples.
13 Overrecentyearsprogrammablecontrollershavebeenfittedtohightemperaturedyeingmachinestoreducecostsandensureconsistentquality.Withmanydye-housesaimingtoachieveaonestopdyeprocess,newmachinesfittedwithprogrammablecontrollersareoftensuppliedwithoutasamplingdeviceandthemachinemustbestoppedinmid-cycleandthemainlidopened.Insuchsituationsthedoorinterlocks(seeparagraphs7-11)shouldensurethattheliquoriscooledandanypressurewithinthevesselisrelievedpriortosampling.
Installation and modification
14 Installersshouldensurethatthemethodofinstallationdoesnotgiverisetodanger.Hotwork,suchascuttingorwelding,whichmayaffectmechanicalintegrityshouldnotbecarriedoutwithouttheagreementofacompetentperson.Itisgoodpracticetoensurethatpipeworkandfittingsdonotobscurepressuregaugesandmanufacturer’smarkingsorobstructpartsofthedyeingmachinewhereaccessisrequiredforexamination.
15 Whenmachinesareinterconnectedviaacommondrainorblow-downline,itisrecommendedthatanon-returnvalveandanisolatingvalvearefittedineachline.Thisistopreventthepossibilityofsteamandhotwaterbeingtransferredfromonedyeingmachinetoanotherinwhichpersonnelmaybeworking,forexampleduringmaintenance.UsefuladviceisgiveninHSEGuidanceNotePM60Steam boiler blow-down systems.10
16 Acompetentpersonshouldbeconsultedbeforeworkbeginsonanymodificationsorrepairsthatmayaffectthevesselintegrityortheoperationofanyprotectivedevices.
Information provided by the supplier
17 Suppliershaveadutytoprovidesufficientwritteninformationtoenabletheusertooperatetheequipmentsafely.Typically,thisinformationwouldincludethemaximumpermissibleworkingpressureandtemperature.
18 Suppliersalsohaveadutytoprovidesufficientinformationconcerningthedesign,construction,examination,operation,andmaintenanceoftheplantto

assisttheusertoprepareawrittenschemeofexaminationunderPSR.Itishelpfulforthemanufacturertoprovideasamplewrittenschemeofexaminationastheywillknowhowtheequipmentisdesignedtooperate,itsanticipatedlife,andtheinitialfrequencyofexamination.
Operations19 Inadditiontostructuralfailures,itisimportanttorememberthathumanfailurescanleadtoaccidents.Sooftenandfrequentlyafterlongassociationwithaprocess,eventhebesttrainedoperatorscanbecomecomplacentandpronetoerror.Itisthereforeessentialthatalloperatorsareadequatelytrainedandsupervised.ThePSRrequireuserstoestablishthesafeoperatinglimits,andtoensurethattheequipmentisoperatedwithinthem.Themanufacturer’sinstructionsoradviceonoperationshouldbemadereadilyavailabletooperatorsandshouldbefollowedstrictly.
20 Operatingpersonnelshouldbesurethatvesselsarecompletelyventedbeforeattemptsaremadetoopenlidsordoors,andtheyshouldbemadeawareofthedangersofbypassingsafetydevicesandofusingextensionstohandlesofdoor-openingmechanisms.Theyshouldalsobefamiliarwiththefunctionsandoperationsofvariouscontrolsandsafetydevicesusedwiththeunitandshouldreportatonceanymalfunctionordamagedcomponents.Managementareadvisedtokeeprecordsofreportedmalfunctionssothattheycanidentifytrendsandtakeappropriateremedialaction.
21 Atjetdyeingmachinesinparticular,attemptsatunblockingtangledfabricmayreleasepocketsofliquorwhichcanbeatahighertemperaturethanismeasuredbythetemperaturesensingprobeandthismayflashintosteamwhenreleased.Itisthereforevitalthatcorrectproceduresarefollowedandtheseshouldbesetoutinasafesystemofwork,whichmayincludetheuseofsuitableprotectiveclothingandfaceshields.

MaintenanceLids and doors
22 Itisrecommendedthatapreventativemaintenanceschemebeemployedtomakesuretherearenofaultsormissingpartsonthedoormechanismsandthatthevariouscontrolsorsafetydevicesarefunctioningcorrectly.Wherelimitswitchesareusedinthedooropeninginterlockcircuitsitisimportantthatthemechanicaloperationoftheswitchesisregularlytestedinadditiontoanyelectricaltestcarriedoutduringroutinemaintenance.Accidentshaveoccurredduetoswitchescominglooseontheirmountings,throughwearontherollersandcams,andincorrectpositioningsothattheswitchcontactsopenafterthedoorhasbeencrackedopen.Anyfaultsordefectsshouldbereportedtoaresponsiblepersonandremediedbeforethevesselisre-used.
23 Itisrecommendedthatthebearingsurfacesofthedoorlockingdevicesberegularlychecked.Ifcracksorotherdefectsaresuspectedinthevesselordooroperatingmechanism,thevesselshouldbetakenoutofuseandacompetentpersonconsultedimmediatelytodeterminewhethertheintegrityofthevesselhasbeencompromised.Repairsshouldbecarriedoutinaccordancewithparagraph16.
24 Whenadoor-lockingmechanismisdesignedtopermitreleaseintwostages,itismostimportantforthegapbetweenthecoverandthesealtobesufficientlywidetoensurepressureequilibrium(generallyatleast3mm).Thisgapshouldnotbereducedbytheuseofnewseals.Thecorrectjointsizeshould,therefore,beestablishedandonlyreplacementsealsrecommendedbythemanufacturerused.Twostagedoorsshouldneverbemodifiedtogivesinglestageopening.
Operating procedure
25 Beforeeverydyecycle,itisgoodpracticetocheckthedoor-lockingmechanisminboththeopenandclosedpositionstoensurethatengagementofthelockingdeviceisevenandcompleteovertheentirecircumference,andthatthedoorlocatescorrectlyeachtimeitisclosed.
26 Afterloadingthevesselandbeforeitisclosed,alldirtandspillageshouldbecleanedfromthelockingpartsofthedoorandthejointexaminedtoconfirmitisproperlyseatedinthegroove.
27 Thedoorsafetydevicesshouldneverbeforcedintoposition.Anyresistancetoclosingorlockingshouldbeinvestigatedimmediately,andrectifiedbeforethemachineisputintouse.Careshouldbetakenthatthelockingringsofdoorsdonotbindduringrotation,otherwisethepointoffrictioncanbecomeafulcrumleadingtohighstresseswhichcancausefailure.
Vents and pressure relief devices
28 Itisimportantthatventdevicesaredesignedsothatproofofclearwaycanbeestablishedbeforecoversareopened.Itispreferablethatdischargeoutletsarevisibletooperatingpersonnel,andareledtoasafeplace.
29 Aneffectivemaintenanceregimewillensurethatthesafetyvalvesareingoodorderandthattheyoperateatthesetpressure.Whereloosestockisdyed,fibresmayaccumulateinthereliefvalveoritsassociatedpipeworkwhichwouldimpede

efficientventing.Usersareadvisedtocheck,andwherenecessaryclean,thesafetyvalveatregularintervals.Theexaminationandsettingofthesafetyvalveshouldbeincludedinthewrittenschemeofexamination.Itisimportanttotestpressuregaugesregularly.
Common drain and blow-down lines
30 Ifablow-downpitormufflersystemisinstalleditisimportantthatitisinspectedfrequentlytoverifythatthedrainsystemisnotblockedduetodeteriorationand/orsludge.
31 Maintenancestaffhavebeeninjuredwhenadjacentmachineshaveventedintoacommonpit.Ventingcanoccurduringthenormaldyecycleorifadrainvalveisunintentionallyopened.Itisrecommendedasafesystemofworkisemployedthatincludestheisolationofthemachinebeingworkedon.
Thorough examination32 ThePSRrequiretheuserofahightemperaturedyeingmachinetoensurethatawrittenschemeofexaminationisdrawnup,certifiedassuitablebyacompetentperson,andthatthevesselisexaminedinaccordancewiththewrittenscheme.Thewrittenschemeofexaminationshouldspecifythenatureandfrequencyoftheexaminationsnecessarytopreventdangerandshouldtakeaccountofknownfailuremechanismssuchascorrosion,erosion,andfatigue.Itshouldbereviewedovertimetoreflectchangesinoperatingconditionsandanydeteriorationdetectedonpreviousexaminations.Adviceonthepreparationofawrittenschemeofexaminationcanbesoughtfromthemanufacturerorsupplier(seeparagraphs17and18).
33 UsershaveadutyunderthePSRtokeeprecordsofpreviousreportsofexaminationandanydocumentsthatwillassistthecompetentpersontoassesswhetherthevesselisfitforfurtheroperation.Itisrecommendedthatrecordsbekeptofallmaintenanceandremedialworkcarriedout.Recordsforeachindividualdyeingvesselshouldbekept,togetherwithentriescountersignedbyaresponsibleperson.
34 Regulation12(protectionagainstspecifiedhazards)oftheProvisionandUseofWorkEquipmentRegulations199211(PUWER)concernstherequirementtopreventtheunintendedorprematuredischargeofanygas,liquidorvapourstoredinworkequipmentsuchasahightemperaturedyeingmachine.Asthecorrectoperationofthedoorinterlocksiscrucialtopreventingdanger,theintegrityofdoorinterlocksshouldbeexaminedperiodically.Whereusersdonotpossessthenecessaryin-houseexpertise,competentspecialistsshouldbeconsulted.
35 AhightemperaturedyeingmachinewillnotbesubjecttothePSRifitdoesnotemploysteamandcannotbeoperatedabove110°C.However,tocomplywiththeirdutiesunderPUWER,employersareadvisedtoensurethatsuchvesselsareexaminedatregularintervalsbyacompetentperson.Theperiodbetweenexaminationsshouldreflecttheriskofinjuryfromstoredenergyandscalding.Anyfaultsrevealedduringtheexaminationshouldbereportedtoaresponsiblepersonandremediedbeforethepressurevesselisre-used.

Use of hydrogen peroxide (H2O2)36 Hydrogenperoxideissometimesusedasableachingagentintextilefinishingprocesses.IfthechemicalreactionduringperoxidebleachingisnotstrictlycontrolledtherecanbearapidevolutionofoxygencausedbythedecompositionoftheH2O2.Therapidityofthereactionandthevolumeofoxygenproducedmaybebeyondthecapacityofthereliefdevicesandthevesselmayfailcatastrophically.Thevesselcouldalsofailduetostresscorrosioncrackingifitwasnotdesignedorconstructedforperoxidebleachingusinganappropriategradeofausteniticstainlesssteelorothersuitablematerialresistanttoattack.FurtheradviceontheexplosionrisksatpressurevesselsusedforbleachingwithhydrogenperoxideisgiveninHSEInformationDocumentHSE764/3.12
Design
37 Toreducetheriskofexplosion,itisrecommendedthattheconcentrationofthebleachingliquor,normallyexpressedinmillilitres/litre(ml/l)orgrammes/litre(g/l)doesnotexceed14mlof35%H2O2perlitreofliquor(16g/l).Atthatconcentrationthepressurevesselheatingsystemshouldlimittherateoftemperaturerisetoamaximumof6°C/min.Ifahigherconcentrationisrequiredaseparateassessmentshouldbecarriedoutastothestabilityoftheperoxideandthecapacityofthepressurereliefsystem.Itisstronglyrecommendedthattheuserseekadvicefromtheperoxidesupplier.
38 Thereliefdevicesshouldbeproperlysizedforadischargecapacitybasedonthemaximumforeseeableconcentrationofperoxide,andtheyshouldensurethatthedesignpressurecannotbeexceededbymorethan10%.Thereliefdevicesshouldalsobesizedfortwophaseflowconditions,andventtoasafeplace.
Operations
39 Usersareadvisedtoimplementprocedurestodefinethevariousrecipesandconditionsunderwhichbleachingtakesplacetoensurethatthemaximumconcentrationofthedosingsolutionisnotexceeded.
40 Peroxidebleachingsolutionmustalwaysbedilutedbeforeitistransferredtothevessel.Atemperatureinterlockshouldbeprovidedsothatthebleachingsolutioncannotbetransferredtothepressurevesselifthetemperatureofthesolutionexceeds45°C.

41 Asafemethodofcontrollingtheconcentrationofhydrogenperoxideistoinstallavolumetrictankbeforetheadditionstankofthedyeingmachine.Thevolumetrictankshouldbesizedatthemaximumsafevolumeofhydrogenperoxidethatthevesselcanacceptandbefittedwithanoverflowpipe.Beforethelineconnectingtheadditionstanktothevesselcanbeopened,thefeedtotheadditionstankhastobeisolatedtopreventfurtherperoxideflow.
42 Operatorsshouldbeawareofthehazardsofperoxidebleachingandbeproperlytrainedtocarryouttheprocessinsafety.
Maintenance
