◦ Antibiotics not restarted after therapeutic levels reviewed◦ Psychotropics not restarted ◦ Transplant meds

Problem We Are Trying to Solve

Pt admitted to ICU had home antipsychotics discontinued for evaluation.  After extubation transferred to general care unit.   Pt had active delirium and required transfer back to ICU to manage withdrawal and redosing of home antipsychotic.
