Home or Maintenance meds are not resumed Adverse Events Examples:
◦ Antibiotics not restarted after therapeutic levels reviewed◦ Psychotropics not restarted ◦ Transplant meds
Problem We Are Trying to Solve
Pt admitted to ICU had home antipsychotics discontinued for evaluation. After extubation transferred to general care unit. Pt had active delirium and required transfer back to ICU to manage withdrawal and redosing of home antipsychotic.
HOLD indefinitely
Requires a current med order

Team decides to place med on hold by doses starting Nov 15th 1800 through Nov 16th 10:00amProvider selects hold to begin 11/15 1800
Provider selects resume Nov 16 1000
The 10:00am WILL BE A SCHEDULE TO GIVE
