Introduction to an exciting new role. (article submitted by Amanda Brookes –HC Administrator)

As Bradford on Avon & Melksham Health Partnership is a member of the Clinical Commissioning Group in Wiltshire, we have a vision that Health and Social Care services should support and sustain independent living and the future system will see:

Increased personal responsibility to maintain/enhance well-being Care provided as close to home as possible A reduced reliance on bed-based care settings. eg. Care Homes

The Health Centre has appointed Anne Shilston to a new post.

An Integrated Team Lead (Senior Nurse for Older People) Anne has a wealth of experience in nursing, as a former Clinical Care Co-ordinator in Wiltshire, a former Care Home Manager and significant years working in the community. Anne starts her new position on 6 th

October 2014.Anne will provide peer support, promote and maintain effective communication with the following Practice projects:

*Nursing Home Project , *Clinical Care Co-ordinator Scheme,*Practice In-Reach Initiative,*Avoiding Hospital Admissions Service’ *Dementia One-Stop Clinic, *The Leg Club

Her priorities include developing and implementing, in conjunction with Public Health, a NHS Health Checks Programme (2500 over 75s) for Older People, to include a falls assessment and memory check (to link into the existing Falls Working Group and the Better Care Agenda). This post will provide Anne with both a supervisory and active patient case load,as well as providing leadership/strategic/development support to the above projects and staff. To act as the main communication point for the emerging integrated community teams and to work with local stakeholders to agree the referral pathway and personal care planning. ****************************************************************************************************************FORWARD INFORMATION from Wiltshire Council A falls prevention class designed to help those who have had a fall or who are afraid of falling, this class is a mixture of chair based and standing exercises designed to improve strength, balance and increase confidence.

This class will safely and effectively improve physical function and postural stability to reduce the risk of a fall and to help class members with their day to day living.(Continued on Page 3)

Being active and taking regular exercise can help by:* Reducing the risk of falling* Improving mobility, coordination and balance* Reducing the risk of stroke, Heart disease, or high blood pressure.* Improving general health and well being* Maintain independence and coping with daily living activities.

The class will start in the New Year and we are currently looking for a suitable. VERY SAD NEWS

IT IS WITH GREAT SADNESS THAT WE HAVE TO CONVEY THE NEWS OF THE DEATH ON FRIDAY SEPTEMBER 26th OF OUR MEMBERSHIP SECRETARY TERRY BILES. Terry has been involved with the Forum from it’s start back in 2004 (He was the first member) Initially a member of the steering group he then became the first secretary. For several years Terry has been membership secretary. You will remember him best as the person at meetings signing you in and collecting your subscriptions.

He was a steadfast and loyal friend to many of us. We shall miss him immensely and we send our heartfelt condolences to his wife Sheila.

SF FUTURE EVENTS

Seasonal Drop In –Thursday December 11th 2014 at 10.30 to 12.00 in the Upper Room of the Bradford on Avon Library (Fully Booked) Information table - Wills and Inheritance Tax and Planning with an expert, Health Watch update, Pre Falls Prevention Class Information.Plus seasonal refreshments,a quiz and competitions. TO SHARE - BE THERE Further information on October 29th.

2015We will be arranging a visit to the Robotics Laboratory at the University of the West of England in Bristol, also a Drop-In to talk to Energy efficiency groups, plus a rep from the Wiltshire Wild Life Trust. Both during January 2015.

March 20th 2015 19.00 at St Margaret's Hall is the “Question Time ----Meeting our Parliamentary Candidates”.We have secured agreement from the main Party Candidates to participate in a Pre-Election “Question Time” with a similar format to the BBC Version.

To ensure a spread of issues we have selected four possible headings. The Challenges of an ageing society Environmental Issues Local/Community Challenges for a New Government Selected Topical Others

More details will follow in our New Year Newsletter but this is advance notice for you to be thinking about what you may wish to ask. Questions will be pre selected from submitted questions pre the date.

April 22nd 2015 is the AGM probably in the United Church at 14.00 with a Guest Speaker. More Information in the New Year Newsletter

Summer Meeting in late June or Early July we are planning a Meet the Doctors ( What is it like to be a Doctor in our current Society and what does the Future hold for the Health Services)

Next Autumn 2015 October Date to be arranged will be a repeat of our 2006 Forum “Staying Put or Moving On”. We envisage this could lead to follow up meetings/Drop-Ins which could include more individual enquiries and discussion. 

Update of the Integrated Care Service Better Care Plan being introduced by the Wiltshire Health and Well Being Board /CCC into Bradford on Avon during 2014/15 .(We are hoping this might also be possible in Jan/Feb 2015)
