Scene 2 Welcome to the Homeless Operations Management and Evaluation System (HOMES) course. Since 2011, HOMES has been the primary data collection system for the VA’s specialized homeless programs. This single system integrates data collection from all of the specialized programs to help staff have a more complete picture of services Veterans are receiving and therefore assists in the coordination of care.

episode; 3. navigate around HOMES to include viewing the Veteran History
Screen, saving, and submitting forms; 4. demonstrate the procedure for searching for a Veteran Record,
creating a Veteran Record, and updating Contact Information; 5. demonstrate completion of the Assessment Form; and 6. demonstrate completion of the Referral Form.

Scene 1 VA specialized homeless program staff have documented their services as well as information about the Veterans they serve using data collections designed specifically for the homeless programs. Over the years, these methods have evolved from “paper and pencil” data collections to VA intranet-‐based information systems. The information provided by homeless program staff is used in reports at the medical center, VISN and national levels.
Scene 2 Scene 2 Monitoring and evaluation has been part of the VA specialized homeless services programs since their inception in 1987. The Northeast Program Evaluation Center (NEPEC), stationed under the VA Connecticut Healthcare System, has played a major role in designing, implementing, and evaluating these programs.

Scene 3 Information from these data collections has been reported back to the field and to VA Central Office in a series of monthly, quarterly and annual reports. Examples include residential program monthly census summaries and the Healthcare for Homeless Veterans (HCHV) Quarterly report.

Scene 1 The current data collection system for the homeless programs is the Homeless Operations Management and Evaluation System, or HOMES. By integrating the data collection for all VA specialized homeless programs into a single system, HOMES helps staff to have a more complete awareness of services Veterans receive and therefore assists in the coordination of care.

VHA and VBA administrative databases provide data to the Homeless Registry. Diagnostic codes, workload information, and benefits information are integrated into each Veteran’s Registry record.

HOMES data is combined with other data sources to provide a rich summary of services and benefits for each Veteran in the Homeless Registry.

The Veteran Profile provides a single screen “snapshot” of each Veteran in the Registry. Among other items, the Veteran Profile displays each Veteran’s most recent services, diagnostic information and housing status.

Scene 6 Homeless Services Data Cube
The Homeless Services Data Cube is the main method for viewing Homeless Registry data. It contains a series of reports (briefing books) with program specific metrics.
Scene 7 Performance Measurement, Case and Management Reporting
Scene 7 Performance Measurement, Case and Management Reporting
Homeless Registry data are used to provide feedback about program processes and outcomes that are identified as performance measures by VA Central Office. Those measures identify program areas targeted for improvement. Management reports, also known as “dashboards” provide concise data summaries to program leaders to assist in policy decisions.
