The  Midlothian  Enhanced  Rapid  Response  Team  (MERRIT)  has  now  been  opera9onal  for  over  a  year  now,  since  star9ng  in  the  summer  of  2014.  This  poster  shows  some  of  the  data  collected  in  the  last  12  months,  star9ng  from  September  2014,  for  the  Hospital  at  Home  component  of  the  team.  Once  again,  we  thank  NHS  Lothian  and  Midlothian  Council  for  their  con9nued  

The  Hospital  at  Home  team  are  part  of  the  wider  MERRIT  service  and  aim  to  offer  an  alterna9ve  to  hospital  admission  for  frail  older  pa9ents.    Although  designed  as  a  service  for  the  over  75s,  many  of  our  pa9ents  are  younger  than  this  and  we  are  happy  to  discuss  any  poten9al  pa9ents  via  the  referral  number  0131  270  8890.  We  can  offer  daily  visits  to  pa9ents  to  undertake  assessment  and  observa9ons,  adjust  medica9on,  give  advice  and  support  to  them  and  their  families  and  to  make  recommenda9ons  on  ongoing  management.    We  can  give  intravenous  medica9on  when  necessary,  for  example  furosemide  or  once  daily  an9bio9cs  and  we  can  administer  subcutaneous  or  intravenous  fluids  when  appropriate.    Our  pa9ents  have  access  to  the  same  range  of  Radiology  and  Cardiac  Physiology  tes9ng  that  the  tradi9onal  hospital  inpa9ents  do.  In  fact,  many  pa9ents  do  not  require  invasive  management,  but  do  benefit  from  the  daily  input  from  the  highly  trained  nurse  prac99oners  on  the
