The Housing and Urban Health (HUH) Section strives to improve the health and wellbeing of
homeless persons while reducing the costly over-utilization of emergency services. Central to
HUH’s approach is the philosophy that housing is healthcare.

• Move TWHC and HUHC primary care homeless services to Primary Care & Wellness Center at 230 Golden Gate
– Create a safe, healing, inviting space
– Enhance services and coordination of patient care
– Attain sustainable and affordable primary care homeless services
Scope of Services across Sites
• 230 Golden Gate – Primary Care – Behavioral Health – Office-Based Opioid Therapy (OBOT) – Wellness – Specialty population services (e.g. HIV, women,
Transgender, Clinica Latina) - TBD
• 50 Ivy – Urgent care for homeless population – Dental services
• Community Sites
Decisions to Date
• About 4,500 patients will be assigned to a primary care provider at 230 Golden Gate
• Team-based, multidisciplinary care
• Services will be multi-modal (e.g., one-on-one, group-oriented, panel management, nursing case management)
