Proof of minority status & citizenship Birth certificate US passport Drivers license with voter registration card
Resumes of all principals, key employees and stockholders
Prior Year’s federal tax return including schedules
Current financial statement Balance and income statements
Assumed name certificate in the county of residence
Certification Documentation
Partnership agreement Must show 51% of business is owned and controlled by the
minority applicant including buy-out rights and profit sharing agreements
Certificate of incorporation or organization Articles of incorporation and bylaws with amendments
Minutes of organizational meeting and last meeting where officers were elected
Both sides of stock certificates including the next consecutive number that is not issued
Stock transfer ledger and stock options outstanding
All agreements that restrict ownership and control
