From an inpatient hospitalization: Referral packets, including psychiatric evaluation, current medication regimen, demographics, and laboratory results can be faxed to 215-348-8957. Contact the department’s director at 267-893-5030 for an initial appointment. Transitional Outpatient ProgramLenape Valley Foundation’s Transitional Outpatient Program, also referred to as TOP, provides a more intensive group therapy model and a higher level of services than traditional outpatient services. The Transitional Outpatient Program is available five days per week, Monday through Friday, from 9:00 am to 12:00 pm. Individuals may be scheduled two to five days a week. This program offers a 90-minute process therapy group and a 60-minute support/educational group covering a variety of topics. Medication appointments and individual therapy are scheduled independently and are available through the outpatient department as needed and desired.

Who is eligible for this program?• Individuals requiring additional

support with a psychiatric illness.

• Individuals who wish to return to daytime jobs and other daytime commitments.

• Individuals who are ready for discharge from an inpatient setting, but are assessed to be in need of continued support and ongoing therapeutic intervention.

• Individuals with co-occurring disorders will be served by LVF staff or referred to ancillary services as appropriate.

What specific services are provided?• Each new admission will receive a

comprehensive psychosocial assessment/intake. New clients, along with the program clinician, will develop an individual treatment plan that is tailored to meet each person’s specific goals and needs.

• Transportation is available, at no charge, to any client who resides in Bucks County and attends the TOP group.

• The TOP program includes a 90-minute group therapy session as well as a 60-minute support/educational group.
