How to Create a Portal AccountFamily Physicians of Evans
Navigate to www.fpevans.followmyhealth.com in your web browser.
Click on Create an Account. Enter your email address, last name, first name, date of birth, and zip code. Scroll down to read the agreement and click I Accept.
Click on the green FMH logo. Create a Username. It must be unique and contain no spaces or special characters. Create a Password. It must be at least 8 characters long.It must contain a number and a symbol. It does not need to have a capital letter. Confirm your password. Type your email address in the bottom two boxes. Click Continue.
Search for Family Physicians of Evans if we do not automatically show up.
Click Connect.
Click Next.
Review the Release of Information Agreement and click I Accept.
Click OK.
Congratulations!Your account is now created. Your connection request will be reviewed and approved by our staff (usually within 1 business day). You will receive an email to notify you. Log in and review your information.
