The first step in determining a suitable investment solution for you is to understand certain information about you (and if this is a joint account, also the other account holders), including your personal circumstances, investment objectives, time horizon, age, and risk tolerance. To do this, we have prepared this Investor Profile Questionnaire that asks a series of questions. The answers will help us determine your investor type for a specific account and will result in you being identified as one of five investor types, which we call Investor Profiles - Conservative, Moderate Conservative, Balanced, Growth, Aggressive Growth. Once you know your Investor Profile for a specific account, you can work with your Mutual Fund Advisor to select investments that are suitable and will help you achieve your goals for your account.

Unless otherwise indicated, in each of the questions and answers, the terms “you,” “your,” “I” and “my” mean the account holder, annuitant or subscriber (as applicable) named above or if this is a joint account, all of the account holders named above together.
