Healthcare exchanges – what they are and how they work

Marketplace fast facts:• Designed for comparing and purchasing

health insurance

• Each state must have a state exchange or use the Health Insurance Marketplace

• Carriers have the option of whether or not to sell plans on a state exchange or the Health Insurance Marketplace

• Carriers and their agents can still sell directly to consumers

• Consumers eligible for financial assistance must apply for the subsidies and advance premium tax credits on the Marketplace

• When enrolling on Humana.com, you are seamlessly redirected to the Marketplace for the financial assistance calculator and application portion of the process, and then automatically redirected back to Humana.com

How are the plans categorized?

Platinum

Individual medical plans are organized in five categories known as “metallic tiers.”

Explaining the Metallic Tiers:The highest tier is Platinum, followed by Gold, Silver, and Bronze, and basic Catastrophic coverage is the lowest.

Basically, the more benefits a plan offers, the higher the premiums will be. Premiums, often paid monthly, are the payments a member makes for their healthcare coverage.

Bronze plans are frequently the least expensive with the lowest monthly premiums, but they may have the highest out-of-pocket costs when insurance is used. Out-of-pocket costs include deductibles, coinsurance and copays. On the other hand, Platinum plans generally have the highest premium payments, with lower out-of-pocket costs when healthcare services are received.

So for example, if a member wants more benefits, and wants to pay less for services when the benefits are used, they may want to consider a Platinum plan.

Catastrophic plans are lower-cost with limited benefits for people under 30 or others above 30 with financial hardship. Financial hardship would apply to those above

the 400% federal poverty level (FPL), who would pay a premium exceeding 8% of their income for the lowest-priced Bronze plan.

Actuarial value – fast facts Silver plans have an actuarial value of 68%-72%. That means, for a standard population:

• The plan will pay around 70% of their healthcare expenses.

• The enrollees themselves will pay around 30% through a combination of deductibles, copays, and coinsurance.

How does it relate to financial assistance?For Americans with incomes below 250% of the federal poverty level, who qualify for financial assistance in the form of cost-sharing reduction, their out-of-pocket portions will be less on Silver plans.

Financial Assistance

Any individual who makes $46,680 or less should check to see if they qualify for a subsidy

Fast facts about financial assistance:• Lower- and middle-income families can qualify

• 138% of the federal poverty level (FPL) is the new cutoff to qualify for Medicaid (may vary by state)

• Rather than referring to the federal poverty level, simply say, “A lot of people don’t even know they may qualify for financial assistance based on their income.”

Advance premium tax credits • For clients who are 100% to 400% FPL

(many states start at up to 138%)

• They ensure that your clients don’t pay more than a certain percentage of their income for health insurance premiums

• To use an advance tax credit or cost-sharing subsidy, they must enroll on state Marketplace or exchange

• There’s a tool on the Health Insurance Marketplace where you and your client can check if they qualify

• When enrolling online directly with Humana, the site seamlessly redirects you and your client to the state Marketplace to qualify and apply for financial assistance, then automatically sends you back to Humana to complete plan enrollment

• Tax credits can be: – Taken in advance to lower monthly payments – Or taken later to lower taxes owed or increase refund at the end of the year

• An individual may receive health insurance coverage subsidized by the premium tax credit only for the months they are enrolled in a qualified health plan through a Marketplace

• Changes after enrollment in income or family size may change the tax credit amount

• People eligible for public coverage, like Medicaid, aren’t eligible for premium assistance, and people who have employer-based health insurance coverage that meets a minimum value of 60% and is affordable are not eligible for a premium tax credit either

Cost-share reductions• For clients who are 100% to 250% federal poverty level

(many states start at 138%)

• Protects from high out-of-pocket costs at point of service

• Cost-sharing reductions are available only on silver plans

• So, depending on your client’s income, instead of 68%-72%, they could receive somewhere in the range of 73%-94% actuarial value by purchasing a silver plan

• This means they pay less out-of-pocket for services without any added premium costs for the eligible client

• Shopper sees only the plan levels for their income

In 2014, enrollees in a federally facilitated exchange plan received a lot of financial assistance through the Affordable Care Act (ACA):

• Nearly 90% received help paying for insurance*

• Tax credits lowered premiums an average of 76%*

• Nearly 50% paid $50 or less per month for their plan*

The total household incomes and the related approximate hourly wage associated with each would be for the combined earnings for wage earners in the family in the plan year.

• A family of four making up to $95,400 per year would qualify for some assistance.

• A couple would qualify at up to $62,920 together

• An individual making up to $46,680, which is about $22 per hour, might qualify.

Those in certain income ranges may qualify for Medicaid coverage instead of financial assistance. The qualifying income levels for Medicaid vary by state – for example, in some states individuals with incomes under $16,000 would qualify for Medicaid instead of a subsidized plan through the exchange.

Here is a chart of the new poverty level guidelines and the thresholds for qualifying for Medicaid coverage or premium assistance for private insurance in 2015:

2015 Uninsured Tax Penalties

Those who do not have health insurance may find that they will incur a penalty as they file their taxes at the end of the year. Penalties in 2015 are $325 per adult and $162.50 per child. A family maximum of $975 is in place for 2015, or 2% of their household income, whichever is greater.

In 2016, the penalties will go up to $695 per adult and $347.50 per child up to a family maximum of $2085, or 2.5% of household income, whichever is greater.

How does enrollment work in markets where Humana plans are available in the state Marketplace?Here’s what the enrollment process looks like in markets where Humana plans are available in the state Marketplace:

To enroll clients through Humana’s website:1. Sign in

2. You can still run a quote and interact with our online enrollment process

3. Once you initiate an enrollment for a plan listed on a state exchange, you’ll be seamlessly directed to the marketplace website to determine your client’s subsidy eligibility

4. After subsidy eligibility is determined, you’ll be sent back to Humana.com to complete plan selection and enrollment

5. You’ll just sign in to Humana.com, and we’ll automatically direct you from there to take you through the process

6. Tools available via our Agent Workbench, like our AOA and AIA processes, will be modified to comply with ACA

Enrolling via Marketplace sites:1. Consumer signs in directly to Marketplace

2. Agent assists consumer with eligibility application

3. Agent assists consumer in comparing plans and completing the application

4. Consumer enters agent’s identification information so agent receives credit for sale

5. Marketplace sends information to Humana

Selling Strategies

The following tips will help you be successful selling in the individual product market.

1. Begin early – Even though the Open Enrollment Period doesn’t begin until November 15th, you can work to establish yourself as a knowledgeable resource by holding educational seminars or utilizing the Health Insurance education email templates prepared for you (see Lead Generation Tools section of this guide). Many of the currently uninsured looked at plans last year but could not decide in time.

2. Be consistently visible – Consumers in this market will largely be unaware of their opportunity to buy or switch plans during this Open Enrollment Period, so it is important to keep your message in the market frequently and in various formats. In addition, some consumers are procrastinators so communicating in the last couple weeks and days of the period will help you to connect to those who have put off the decision until the end of the period.

3. Be an educator – Because consumers are largely unaware of the financial assistance that is available or the penalties they could face if they don’t purchase insurance, you can be a hero by bringing these things to their attention. See the email series on page 19 and seminar PowerPoint presentation on page 13 for talking points.

4. Take care of your current book of business – Contact your current customers early to ensure that they are happy and that they have used their benefits. The insurance education emails provide messages. Know what, if anything, is changing in 2015 for them and discuss it with them openly. Reassure them that they do not have to do anything to continue their existing coverage – their plan continues as long as they continue to pay their premiums.

5. Get referrals – When you contact your current customers, always ask if they know of anyone who may benefit from having the type of health insurance they do.

6. Multi-touch approach – Cultivating a prospect may take several points of contact. That’s why we’re providing several touch points in the materials on page 16.

7. The pre-approved materials in this catalog are created for your convenience in accordance with insurance marketing best practices. They may save the expense and time of creating your own materials, since any use of Humana name, logo, or mention of Humana’s plans, premiums, benefits or services must be approved by Humana.

Using the Campaign Materials We have created a variety of direct response advertising tools for you to use during open enrollment. There is a campaign developed to help you promote seminars and another intended to encourage personal consultations. You will notice that some of the campaigns are for the general audience while another is intended specifically for families with children or the young invincible audience.

We recommend you try a mix of materials and audience targets to increase your exposure and opportunities for lead generation.

Purchasing Print MediaWhen placing a print ad, be sure to gather information about the publication where you are placing an ad. These tips should be helpful when purchasing print media:

1. Make sure the publication reaches your target audience. Publications should be able to supply demographics on their audience, which can aid in your decision-making.

2. Advertise in community sections and/or family-centered publications. Many communities have local publications, which can serve as great outlets to deliver your message to the consumer in your local area.

3. Plan your marketing campaign in advance. If you know you will run your ad in a publication multiple times, ask the rep for information on their frequency discounts.

The majority of markets will have Humana HMOs and PPOs, and/or EPOs in Texas. Here’s how they differ:

HMO - Health Maintenance Organization*• Local network of physicians, hospitals, and specialists

offering services at a reduced fee

• Members must choose a primary care physician (PCP) from the network who helps coordinate care

• Members must get a referral to see an in-network specialist, go to an urgent care facility and for hospitalization, with few exceptions

• No coverage for providers outside of the HMO network, i.e. patient is charged the full fee with the exception of medical emergencies

EPO – Exclusive Provider Organization • EPOs, sometimes referred to as open-access HMOs, use

a local network of physicians, hospitals and specialists

• Somewhat of a hybrid between an HMO and a PPO

• Only cover in-network doctors, specialists or hospitals (except emergencies like HMOs)

• Referrals and PCP selection are not required (like PPOs)

PPO – Preferred Provider Organization* • Referrals are not required to see a specialist

• Offers the freedom to receive care from any provider of choice, but members save money by choosing those in the network

• Monthly premiums are typically higher than HMO due to the increased flexibility

Bottom line ... HMOs are the most affordable option. EPOs allow direct access to any in-network providers. PPOs cost a little more but allow members more choices for when and where they can access healthcare. There are different plans in different metallic tiers and levels of cost and benefits to meet the different needs of your customers.

with certain hardship situations, have high deductibles. The plan includes copays for doctor visits; otherwise the member has 100% coverage after they meet their deductible. The advance tax credit is not available with this tier, but the premium will be lower.

• Bronze plan will be the lowest-priced plan where the advance premium tax credit can be used. Appeals to those looking to just “comply with the insurance mandate.”

• Silver is the only plan tier where those who qualify can apply a cost share reduction subsidy and the advance premium tax credit.

• Gold and Platinum tiers have higher premiums, but much lower deductibles, copays and maximum out of pocket.

Humana’s new 2015 product names have been streamlined to incorporate the metallic tier, the plan’s individual medical deductible, the network name and product type.

Plan names describe important details of the plans: Humana <metallic tier> <$deductible> <Network area/type>Our plan names include some of the details you will want to consider in making a selection. These include:

The metallic tier – Basic, Bronze, Silver, Gold or Platinum. (Bronze might have the lowest premiums and the highest out-of-pocket costs. Platinum plans generally have the highest premium payments, which buys you lower out-of-pocket costs when healthcare services are received.)

The individual medical deductible amount (again, that is the cost you would be responsible for)

The network name as it is listed in the provider directory and the member’s ID card. Some networks serve a specific area and others are national in scope.

The type of plan, HMO, PPO or EPO

Audiences: The lead generation marketing tools we have developed target the following three audience segments:

• Boomers (age 50-64) – An important audience segment that values insurance because they seek healthcare services more frequently. They also are the most likely audience to value/prefer human contact to help them understand plans. This audience also offers opportunities for transitioning to Medicare as they age.
