Hello and welcome to the April CBS Connect newsleer. Since we last wrote there have been some key developments. Firstly CBS has re-established its Client Advisory Group. The first meeng was held on Wednesday 30 th March with the next meeng due to be held on Wednesday 31 st May. The group’s terms of reference focus on being able to gain feedback and insights into topics that are important to both clients and CBS. Parcipants do not aend in order to represent themselves as individuals, but rather the broader views of all people who use our services. You would be very welcome to join, please contact Tracy Ferguson on (03) 6208 6600 for more details. Most of you will now be aware of the changes to Home Care packages. On 27 th February 2017 these packages became officially portable, meaning they may be moved between service providers if the client wishes to do so. Our Aged Care services team can provide you with any informaon you require. We are very happy to talk with your current provider if you wish to move your package to Community Based Support. If you know anyone that is not happy with their current service provider, ask them to give Community Based Support a call to see what we can do to provide them a service to beer suit their needs. The Naonal Disability Insurance Scheme (NDIS) currently offers services to eligible people aged between 12 and 28 years. The service will expand to include 4 to 11 year olds on 1 st July 2017. Community Based Support offers support services to people in the NDIS. We work closely with parcipants to ensure they get the most out of their packages. Jackie and her Disability Support services team would be very happy to discuss your needs with you. We have worked with real clients, asking them why they choose CBS as their service provider. The campaign is being seen across a variety of media statewide, so keep an eye out. Whilst there, the teams ran a series of short 30 minute informaon sessions about how to access Aged Care and Disability support services. Most of you will now be aware of the changes to Home Care packages. On 27th February 2017 these packages became officially portable, meaning they may be moved between service providers if the client wishes to do so. Our Aged Care services team can provide you with any information you require. We are very happy to talk with your current provider if you wish to move your package to Community Based Support.

If you know anyone that is not happy with their current service provider, ask them to give Community Based Support a call to see what we can do to provide them a service to better suit their needs.

If you, or someone you know, is aged 64 or under, and is in need of some home modifications such as the installation of a level access shower or large access ramps to assist you to remain living independently at home, CBS may be able to help.

CBS offers a Major Home Modifications program for people with disability who are aged 64 years or under. This is a co-contribution program that aims to assist in resolving access issues in and around the home. The program is funded until 30th June 2017, so be quick to ensure you don’t miss out.

Our staff member, Tracey, who lives in the area, will be there every Wednesday from 9am to 4pm, to assist you with your aged care and disability support services.

CBS has a team of local, professionally trained staff, ready to support you to live the life you choose, in the place you love.The Aged Care and Disability Support services teams have been out

and about visiting communities in southern Tasmania since January this year and will be visiting the north and north west of the state later in the year.

Corporate SupportThe Corporate Support Team looks after all administration for CBS clients. The team consists of Vanetta, Judy, Chelsea, Julie, Bronwyn, Sharon, Scott and Monique.

When you call CBS, the first person you speak to will be from the Corporate Support team. They are able to assist you with many of your enquiries, however, when you need more detailed information they will connect you with the right team.

The Corporate Support team can assist you with –

• Billing enquiries including payment and checking balances

• Cancellation of single visits• Checking your days and times of support• Re-booking your annual spring clean and garden

maintenance (only for clients who have previously received this service)

Aged CareThe Aged Care Team looks after clients who are aged 65 and over (or 50 years and over for Aboriginal and Torres Strait Islander people) and are currently receiving ongoing services from Community Based Support. The Aged Care team consists of Ebony, Sue, Tanya WB., Mikayla, Kate, Tracey C and Shanelle.

Who’s WhoOf The CBS OfficeWe would like to make it easy for you to find the right person to speak to when you call us here at CBS. Below you will find a list of ‘Who’s who’ at the CBS office.

The CBS A Team have developed an email account for you to use at any time when you have a suggestion on how we can improve our current services. We also invite you to provide any suggestions for new service ideas that you may have as well. This email account will be monitored by the A Team on a regular basis and your suggestions will be reviewed at our fortnightly meeting. Please keep in mind that the purpose of this email isn’t to lodge complaints or queries but is a place for you to share your great suggestions.

The Disability Support team consists of Jackie, Anna and Meegan.

Please ask to be put through to the Disability Support Team if you would like to discuss any of the following –

• Ongoing changes to your support. For example: you’d like to cancel one of your days permanently or you might like to find out if it is possible to access additional hours.

• You have any questions about what duties our Support Workers can undertake during your scheduled visit.

• You have any issues or concerns about anything to do with your ongoing Disability Support services.

Carer SupportThe Carer Support team looks after all carers. A carer is a person who provides ongoing support to a person at home or in the community.

The Carer Support team has a number of functions including physical and emotional support options, in home respite and residential respite (a short term stay in an aged care facility). The Carer Support team consists of Corinne, Liz, Tim, Craig, Georgia, Taylah, Jodi and Kay.

Please ask to be put through to the Carer Support team if you would like to discuss any of the following –

• In home supports such as short term respite, including social support and domestic assistance

• Residential respite (a short term stay in an aged care facility for the care recipient)

• Free quarterly program of carer activities and events

• Emergency support

The programs that fall under the Carer Support team include the National Respite for Carers program (NRCP); Mental Health Carer Support program (MH:CS); Young Carer program (YC); and the Respite

Support for Carers of Young People with Severe and Profound Disabilities program (SP).

Social Groups The Social Groups team coordinate all ongoing social groups for CBS. The social groups team consists of Tanya S. and Leah.

• Day centre groups• Gentle movement and chat groups• Weekday and Sunday social groups and activities• Sundowner respite group• Water aerobics group

RosteringThe Rostering Team schedules all visits for all CBS clients. The rostering team consists of Jude, Hannah, Morgan and Melinda.

• Short-term changes to your roster• Confirming or re-scheduling your assigned garden

maintenance or spring cleaning service• Any concerns you may have about your current

support worker.

Garden Maintenance, Spring Cleaning, Minor Home Modifications and Home MaintenanceThe Garden Maintenance, Spring Cleaning, Minor Home Modifications (such as small ramps and hand rails) and Home Maintenance teams are all coordinated by Sandra. There are a number of staff who work in this team, but all queries are directed to Sandra.

Major Home ModificationsThe Major Home Modifications program is coordinated by Paul. If you have any queries relating to Major Home Modifications including how this program works, if you are eligible for it, or how a particular job is progressing, please ask to be transferred to Paul.

Marketing and CommunicationsThe Marketing and Communications role exists to inform Tasmanians about CBS and how we can assist people to remain living at home for as long as possible.

In 2017, the main goals of the Marketing and Communications role are to:

• Develop an advertising campaign based on real customer experiences.

• Communicate with clients, staff and other Tasmanians about the variety of support options available through CBS

• Assist CBS to continue to be a customer service oriented organisation.
