    Presumptive Surveillance

    Laboratory Surveillance

    Fever o Fever < 7 Days

    1. Only Fever 2. With Rash 3. With Severe Joint

    Pains & Swelling 4. With Bleeding 5. With Daze / Semi or

    Unconsciousness o Fever > 7 Days

    Cough with or without fever 1. < 2 Weeks 2. >2 Weeks

    Loose watery stool of less than 2 weeks duration 1. With Some / severe

    dehydration 2. With no dehydration 3. With blood in stool

    Jaundice case of less than 4 weeks duration

    o Cases of Jaundice Acute Flaccid Paralysis in less

    than 15 years of age Unusual symptoms leading to

    death or hospitalization that do not fit into the above

    Fever o Fever < 7 Days

    Only Fever- Malaria PV & PF 1. With Rash, Blister-

    Measles, Chickenpox 2. With Severe Joint Pains & Swelling-

    Chikungunya 3. With Bleeding- Dengue 4. With Daze / Semi or

    Unconsciousness-AES, Meningitis o Fever > 7 Days-Typhoid

    Cough with or without fever < 2 Weeks 1. Sudden onset of fever >38c and cough or sore

    throat- ARI / Influenza Like Illness (ILI) 2. Fever cough with difficult breathing + Chest X

    ray -Pneumonia Loose watery stool of less than 2 weeks duration

    1. Acute Diarrheal Disease- ADD 2. Acute diarrhea with visible blood-Bacillary

    Dysentery 3. Acute watery diarrhea with severe dehydration-

    Cholera Fever with headache, myalgia, conjunctival,

    suffusion, haemorrhages and jaundice- Leptospirosis.

    Fever >101F continuous or intermittent for two weeks and unknown cause (even after lab testing)- Pyrexia of unknown origin (PUO)

    Jaundice case of less than 4 weeks duration Acute jaundice, dark urine, anorexia, urobilinogen & 2.5 times alanine aminotransferase etc.-AVH (HAV&HEV)

    Acute Flaccid Paralysis in less than 15 years of age-AFP

    Dog Bite Snake Bite Unusual symptoms leading to death or

    hospitalization that do not fit into the above

    PHC,UHC-CHC 1.Malaria (Microscopy) P. Falciparum, P. Vivex

    1. Bacteriological test for water (H2S test & MPN test)

    District Hospital, Sub District Hospital 1. Malaria (Microscopy) P. Falciparum, P.

    Vivex 2. Typhoid (Widal tube, Antibody detection

    test, Typhidot & Blood culture) 3. Bacteriological test for water ( H2S test

    & MPN test) Medical College 1. Malaria (Microscopy)

    P. Falciparum, P. Vivex 2. Diphtheria (Culture) 3. Cholera (Culture) 4. Typhoid (Widal test,, Typhi dot, Blood

    culture) 5. Hepatitis A&E (Igm ELISA) 6. Measles (Igm ELISA) 7. Dengue (Igm ELISA , Test kit, NIV, NS1) 8. Polio 9. Chikungunya (Igm ELISA) 10. JE(Igm ELISA) 11. Meningococcal Meningitis (Latex

    agglutination test) 12. Hepatitis B (Anti HBc) 13. Leptospirosis (Rapid test, MAT& PCR,

    Igm ELISA) 14. Bacteriological test for water (H2s test &

    MPN test) Compiled S, P & L Form should be sent to DSO from PHC/UHC through THO/MOH on Monday & Last date of Data entry on portal

    every Wednesday by DSO
