IDT Meeting Procedure (AT&R) Procedure Number WCDHB-PH-019 Version Nos: 8 19 Controlled Document – WCDHB Hannan Ward Procedure Manual This Procedure outlines the process for Interdisciplinary Team meetings for the West Coast District Health Board (WCDHB) Grey Hospital AT&R Unit (Hannan Ward). This Procedure is to be followed by all clinical staff throughout Hannan Ward. There are no definitions associated with this Procedure. For the purposes of this Procedure: All WCDHB Clinical Staff Members are required to abide by the requirements of this Procedure This Procedure requires no specific resources. 1.00 A weekly AT&R Interdisciplinary Team (IDT) meeting will be held Tuesday 1330 hours - 1500 hours in the Community Services Meeting Room, Grey Hospital and Friday 0900 hours in Hannan Ward Office. 1.01 The objectives of the IDT meeting are to identify: i) To discus present health status of each patient ii) And discuss patient social and support needs and set goals accordingly iii) And plan discharge needs iv) Identify functional status prior to admission from domiciliary services 1.02 The following team members should participate in the meetings: Geriatrician Clinical Nurse Manager AT&R Nurse Specialist Physiotherapist Occupational Therapist Speech Language Therapist Social Worker Community Mental Health Nurse Dietician Pharmacist District Nurses Nursing staff as appropriate from other wards (if patient in surgical or medical ward) 6.     Students

    1.03 At each meeting, each patient is to be discussed and goals for the following week set.

    1.04 Geriatrician or RMO will document in patients clinical record using IDT stamp. 1.05 AT&R Nurse Specialist or delegated person is responsible for minute taking. Minutes

    will identify social, rehabilitation and support service needs. Minutes once typed will be circulated to all team members.

    The objectives of the IDT meeting are to identify and discuss patients social and support needs and set goals, plan discharge needs and identify functional status prior to admission from domiciliary services.

    At each meeting, each patient is to be discussed and goals for the following week set. Geriatrician or RMO are responsible for documenting a summary outcome at the meeting
