The goal of the Illinois Community Supportive Services andReferrals Directory is to provide each person discharged from

Illinois correctional facilities with information that will assist them inmaking a successful transition on the “outside.” This guide includesinformation about health care facilities; case management; housing;counseling; food assistance; and other social services throughout theState of Illinois. This guide was developed by Men and Women in

Prison Ministries with generous funding from the IllinoisDepartment of Public Health.

The mission of Men and Women in Prison Ministries UniversalHouse of Refuge Center (M&WIPM/UHORC) is to provide services

that will promote spiritual and cultural awareness, health education,substance abuse counseling, family development, male/femaledevelopment, youth development, support groups, and prison

ministry through outreach and self-help programs on a community-based level.

M&WIPM/UHORC provides prison ministry and outreach workshopsdesigned to walk community and faith-based organizations throughthe process of creating and maintaining effective prison ministries.

In addition, we provide workshops and trainings for the incarcerated,formerly incarcerated, families of the incarcerated and the

community at large.
