PHARMACY

Haileleule Getahun, Chicago – pharmacist license (051­40817) placed in refuse to renew status after defaulting on an Illinois educational loan.

Panayiota T. Megremis, Oak Lawn – pharmacist license (051­038686) placed in refuse to renew status after defaulting on an Illinois educational loan.

Kennis Hal Wallace, Jr., Galatia – pharmacist license (051­028035) suspended for two months and immediately followed by probation for three years and fined $10,000 after allowing a pharmacy technician to perform dispensing duties without a pharmacist present, failure to properly secure Duragesic patches, a Schedule II drug, failure to routinely sign or initial Schedule III, Schedule IV, and Schedule V prescriptions, and failure to obtain a “pharmacist not on duty” sign.

Luis Alfonso DeJesus, Chicago – pharmacy technician license (049­158171) placed in refuse to renew status as a result of diverting controlled substances.

PODIATRY

Stephen Alan Weinberg, Des Plaines – podiatrist license (016­002671) and controlled substance license (316­000441) placed on probation for one year and fined $2,500 for unprofessional conduct concerning periodic self­medicating with controlled substance for headache condition; failure to maintain controlled substance inventory logs.
