THE RANGE
Morphology of Premium Straight,Premium SP and Premium TG implants The different morphologies that characterise the entire family of Premium implants (Straight, Switching Platform “SP” and Transgingival “TG”) and Kohno implants (see following page) mean that the correct implant design can always be selected to suit the site in which they are to be fitted.
Only the thread of the implants with diameter 3.30 mm has a pitch of 0.60 mm and a triangular profile characterised by a 50° angle and a depth of 0.30 mm.
Implants with diameter 3.30 mm are available only with a cylindrical-shaped coronal emergence with a smooth collar 0.80 mm high.
All Premium Straight implants with a cylindrical-shaped coronal emergence have a smooth collar 0.80 mm high.
The standard thread of Premium implants has a pitch of 1 mm and a depth of 0.40 mm.

ShortyShorty implants are available in heights 7.00 and 8.50 mm, while the TG version also comes in height 6.00 mm.

The neck of Premium SP implants widens gradually in the cornal direction, then returns to the implant diameter at connection level. The maximum widening is 0.60 mm on the diameter.
The height of the transgingival part of Premium TG implants is 2.2 mm irrespective of the endosseous length of the implant. Switching

Premium TG implants have a rounded apical profile.
All Premium implants (in all the different morphologies) have three apical cutting notches that increase the penetrating capacity, ensure a good self-threading capability, improve primary stability, ensure the non-rotational aspect and, at the same time, offer three decompression and release areas for coagulation.

THE RANGE
Morphology of Kohno Straight,Kohno SP and Kohno TG implants
The bevel offers a spacer plane between the crest bone and the post closure and allows the Switching Platform technique to be performed.
The different morphologies that characterise the entire family of Kohno implants (Straight, Switching Platform “SP” and Transgingival “TG”) present the same length of the endosseous portion and differ only in the coronal portion (connection and transgingival).

The Straight coronal emergence is characterised by a smooth neck 0.80 mm high.
The thread geometry is the same for all the specifications of the Kohno implant family (Straight, SP and TG).
Kohno implants (in all the different morphologies) have a rounded apical profile.

The 2.2 mm height of the transgingival neck is ideal for ensuring compatibility with the biological width and at the same time allows excellent management of the soft tissues.
The rounded apex of Kohno implants (Straight, SP and TG) is optimum in maxillary sinus lift procedures.
The asymmetrical profile is convex and has a flat lower surface that is ideal for supporting compression during chewing.

IMPLANT SYSTEM
Surfaces It has been widely demonstrated that the closer the roughness is to the size of the fibroblasts, the more influence it has on cell behaviour, causing the platelet activity to increase compared to a smooth surface, thus accelerating the repair and osseointegration processes: the roughness can guide the layout of the cells, alter their metabolism and proliferation, differentiate osteoblasts and modulate production of extra-cellular matrix. For clinical findings concerning Sweden & Martina surfaces refer to the paragraph in the bibliography (see page 164) with the list of numerous in-vitro and in-vivo studies.
ZirTi surfaceAll three versions of Premium implants (Straight, SP, TG) are available with the ZirTi surface, characterised by a smooth neck and a body with ZirTi treatment.
The implant body is treated with appropriate subtraction techniques that give the surface the characteristic ZirTi morphology, able to significantly increase the bone-implant contact surface and ensure excellent primary stability. The ZirTi surface has shown to have a sub-layer that promotes cell regrowth, such as to adequately boost its differentiation and proliferation.
The smooth collar allows the perfect control of the connection diameter and prevents the accumulation of plaque in the area where it joins the post.

All Premium and Kohno implants (Straight, SP and TG) are available in the DES version characterised by surfaces with progressive increase of roughness and with a smooth neck that passes through a ZirTi treated zone to an apex coated in HRPS.
The apical portion is coated with HRPS (High Roughness Plasma Spray) and has the maximum level of roughness that can be obtained, thus guaranteeing excellent primary stability even in the presence of slightly mineralised bone and significantly increasing the bone-implant contact surface.

- Ø 3.30 mm implants: The use of the Premium ø 3.30 mm implants is only for replacing the central and lateral incisors and upper and lower canine teeth. They can also be used in the pre-molar and molar zone, but only to support the prosthetic structures which are also supported by implants with a bigger diameter.
- Shorty implants are characterised by a reduced vertical dimension, it is recommended to use them with implants with traditional dimensions with joined prostheses.

Surgical cover screws
Premium SP and Kohno SP implants: The length includes the portion relating to the bevel (the counter-conical upper portion of the collar that brings the crown diameter back to the diameter of the connection platform). It is equal to the length of the holes produced by the relative drills. If you want to leave the juxta-osseous bevel, simply screw the implants up to the desired level.

The better the processes of passivation, cleaning and decontamination of an implant surface, the greater the presence of pure titanium on its surface, which proportionally increases the possibilities of osseointegration.At the end of the surface treatments, the implants are subjected to a careful cleaning and decontamination process by means of cold plasma triggered in Argon after first being cleaned of the main processing residue with numerous washing cycles in specific solvents.What is decontamination? It is the total removal of dirt and particle residue from the surface of the implants.During the Argon treatment, the gas atoms are partially ionised, they acquire energy and “bombard” the surface of the fixture violently. This kind of “atomic sand-blasting” removes organic contaminants without leaving any traces or additional residuals.
As known, Argon is an inert gas that does not react with the titanium surfaces. The condition of surface decontamination is controlled regularly with randomised analyses of Bioburden residuals and a SEM visual examination on all the batches produced. This process activates the ionisation of the atoms on the surface of the titanium oxide which in turn increases the wettability of the fixture.

Implant packaging The implants are packaged in PMMA vials in which they are held/housed in special titanium baskets that protect the surface of the fixture against possible recontaminations due to contact. All the materials comprising the packaging have been suitably tested to verify their suitability to sterilisation, preservation and medical use.
The blister packs containing the implants are packed in cardboard boxes which also contain the labels for the patient records reporting the details for traceability of the product (code and batch number).
Upon opening of the vial, the implants appear with the visible connection ready to be engaged by the drivers.
A label that changes colour is applied on the bottom of the vial. This label has the property of changing colour from orange to red during sterilisation. When an implant pack is opened, a red label is therefore proof that it has been sterilised. The vials are contained in a special PETG blister pack sealed by a Tyvek cover, which guarantees sterility of the product for 5 years.
The surgical cover screw, supplied with every single implant, is housed in a special compartment in the top of a blue cap made of LDPE which closes the vial. In turn, a small transparent lid in PMMA closes the blue cap.
• Sterilisation is the total elimination of the residual microbial load present on the implant after the decontamination and packing process, it is carried out with the use of beta rays. The sterilisation procedures are carried out in accordance with the UNI EN ISO 13485 and UNI EN ISO 9001 quality standards. A beta ray sterilisation process was chosen because it has a variety of different advantages:
• the process occurs in a completely automatic way with computerised control of all the phases; • the process is quick, reliable and extremely easy to repeat with safety and precision; • the process is extremely eco-friendly, does not require the presence of radioactive sources and does not lead to the formation of toxic or radioactive
products; • beta rays are minimally invasive with regards to packaging due to the speed of the treatment. This guarantees preservation of the product’s sterility over
time (certified duration of 5 years).

Premium cylindrical implants and Kohno conical implants, in both the version with Straight cylindrical emergence and the version with enlarged emergence SP (Switching Platform), while offering different surgical options thanks to their different morphology, have a common connection platform for the prosthodontist which makes the dental surgeon’s job easier.
COLLARImplants with a diameter greater than 3.30 mm have a collar on top of the neck that gives great maximum stability to the connection and aids the correct distribution of the masticatory forces, which are unloaded along the entire implant perimeter. It also creates a stabilising connection for the post, capable of reducing mid-distal and lingual-vestibule movements.The internal part of the collar allows safe and easy engaging of the insertion driver.
INTERNAL HEXAGON FOR REPOSITIONING OF THE PROSTHESISLocated in a coronal position, it is characterised by excellent visibility. The 60° repositioning and the total connection depth of almost 2 mm ensure non-rotation and precision.
BEVELThe widening generated by the bevel in SP implants allows the crestal bone to be distanced from the implant connection by 0.30 mm in a horizontal direction and 0.50 mm in a vertical direction, thus offering a Switching Platform embedded in the morphology of the implant.

SWITCHING CONNECTION PLATFORM
Switching PlatformThe Switching Platform is a prosthetic rehabilitation technique that aims to distance the prosthetic connection platform from the cervical bone.In fact, the abutment-implant junction is today indicated as one of the factors responsible for peri-implant bone reabsorption because it can trigger inflammatory reactions.Clinical findings relating to the use of Premium and Kohno Switching Platform implants confirm the reliability of the Switching Platform technique with Premium and Kohno implants.
Diameters being equal, implants with Straight emergence and Switching Platform emergence use the same prosthetic components.The Switching Platform technique used in this case is defined as “implant” since it is incorporated in the implant morphology.

If the morphology of the receiving bone does not allow the use of implants with an enlarged coronal diameter and it is therefore necessary to use an implant with a straight coronal emergence, it is still possible to employ a prosthetic Switching Platform on implants with diameter 3.80 mm, using a post with a smaller diameter (3.30 mm).

With this configuration the Switching Platform that can be obtained on either side is 0.325 mm horizontally and 0.35 mm vertically giving a total of 0.725 mm on each side.
If implants with SP emergence can be used and you want to maximise mismatching between the level of the bone crest and the join between implant and prosthesis, the combined use of an SP implant and a post with a smaller diameter, as in the drawing below, allows you to obtain a highly accentuated Switching Platform.
With this configuration, with a ø 3.80 mm SP implant and a ø 3.30 mm post the total Switching Platform that can be obtained on either side is 1.25 mm.

WARNING: Considering the reduced diameter of Ø 3.30 mm prosthetic components, it is recommended to use them to obtain Switching Platform with 3.80 mm implants exclusively for single crown in front sectors (excluding premolars), while in distal sectors they must be used exclusively as a support for multiple prostheses.

TG (TRANSGINGIVAL) CONNECTION PLATFORM
Premium TG and Kohno TG The implants of the Premium TG and Kohno TG system have two connection platforms:• the “M” (Medium) connection, which has a platform with external diameter 4.80 mm, common to both Premium TG and Kohno TG implants with
diameters of 3.30, 3.80 and 4.25 mm • the “W” (Wide) connection, which has an external diameter of 5.80 mm and is only present in implants with a diameter of 5.00 mm, in this case too both
Premium TG and Kohno TG The M or W version is also present in the product codes of the prosthetic components, making it easy to identify the pieces to be used for reconstruction.
20° CONICAL SEAL This part of the connection makes use of the conical friction concept in direct screw-retained solutions. The friction guarantees high resistance against unscrewing, and also acts as an excellent anti-bacterial seal.
COUPLING PLATE The W emergence is very wide, which has led to the development of prosthetic components with a wider diameter. This makes them more stable and, thanks to the coupling plate, more resistant to chewing loads coming from the molar and premolar sectors.
SUPPORTING SHOULDER The supporting shoulder of the implant-prosthetic connection is formed by a wide external bevel with a 30° taper which contains the connection well.
INTERNAL COLLAR As for submerged Premium and Kohno implants, it is the anchoring point for the insert drivers.
INTERNAL HEXAGON FOR REPOSITIONING OF THE PROSTHESIS The hexagons of Premium TG and Kohno TG implants have the same dimensions as the submerged Premium and Kohno implants. This allows the optimisation of surgical instruments, increasing the flexibility of the entire family implants, because the same drivers and accessories are used.

REPOSITIONABLE POSTS AND ABUTMENTS WITH PASSING SCREWThe hexagon positioned at the base of the connection cone allows using repositionable solutions, which are fixed to the implants with the aid of a passing screw.
The connecting screw of the posts descends into the inside of the connection, thus minimising the lever fulcrum during the application of the loads and preventing the loosening of the connecting screws.
This patented connection design feature allows modelling of the posts with perfect angulations and profiles because the presence of the screw head does not interfere with the personalisation and insertion of the over-structure.
SOLID POSTS AND ABUTMENTS The presence of a coupling cone allows the use of pre-made prosthetic components with a single body: these solutions are screwed to the prosthetic platforms without the need for separate screws. Of course, they are not repositionable, and cannot be removed once screwed into the site.
Protection caps and practical closed-tray caps are available for abutments and posts which allow the impression to be taken in the mouth on the post screwed onto the implant.
Thanks to the conical friction, once the solid components have been tightened they give an extremely high tightening capacity because the unscrewing torque needed to remove them would be very high.

SURGICAL PROCEDURE
The surgical procedure of insertion is extremely simple. The implant does not require a mounter for inserting into the implant site because it is engaged directly inside the connection by practical Easy Insert drivers designed to guarantee a safe grip, to prevent deformations to the corners of the connections and at the same time to allow easy removal from the implant wells.
The Easy Insert drivers aid visibility of the operating field, do not occupy much space, and allow adjusting the connection hexagon properly because their hexagonal visual index is the same as a prosthetic index.
Engagement is extremely safe and reliable with the use of a special titanium o-ring that engages inside the connection.
The presence of a wide hexagon at the base of the prosthetic cone allows easy and safe engagement of the Easy Insert drivers for easy and safe insertion of the implants into the respective sites.
The colour code on the instrument shank and on the retaining metal o-rings facilitates the identification and choice of the item needed.

The image on the left shows how a traditional instrument (in green) edges inside the connection (in grey). This geometry inevitably determines the grip and deformation of the actual session.
The special design of the Easy Insert drivers (in green in the section on the right) enables the driver and implant to interact on a portion of the surface in the centre of the connection hexagon.
Easy Insert drivers are available with two different hexagons, characterised by the colours blue and magenta, to be used according to the implant connection.
Blue drivers are used for implants with diameter Ø 3.30 mm and Ø 3.80 mm while magenta drivers are used for larger diameters (Ø 4.25, Ø 5.00 and Ø 6.00 mm), except the TC version Ø 4.25 mm which uses the blue driver.

All the surgical kits include a practical torque-control ratchet that acts as a surgical key for inserting the implants and as a dynamometric key for checking the tightening torque of the prosthetic screws. The torque-control ratchet has a very small head, making it easy to use even in distal sectors. This torque-control ratchet is also included in the Screw Kit.
All the kits consist of practical boxes in Radel with a surgical tray inside that is set-up to hold the instruments according to a guided procedure. The sequences of use of the instruments and their arrangement are indicated and facilitated by coloured marks.

SURGICAL INSTRUMENTS
The surgical kits for the Premium/Kohno implant system have been designed for maximum simplicity and ergonomics and are available in three versions:
The surgical kits
The instrument codes are printed on the trays to allow the assistants to easily reposition them after cleansing and cleaning.
The standard surgical kits contain the stops for safely using the drills (they do not include countersinks and bone profilers). These stops are extremely practical because they allow manually inserting and removing drills in tip -> shank direction. The instruments contained in the kits are all made of stainless steel specifically for surgical use.
Premium Kohno surgical kit: contains all the surgical and prosthetic instruments* needed for all the implants in the Premium and Kohno families (in all morphologies: Straight, Switching Platform and Transgingival).
Premium surgical kit: contains all the surgical and prosthetic instruments* needed only by users of the implants in the Premium family (in all morphologies: Straight, Switching Platform and Transgingival).
Kohno surgical kit: contains all the surgical and prosthetic instruments* needed only by users of the implants in the Kohno family (in all morphologies: Straight, Switching Platform and Transgingival); it is particularly simple, immediate and complete.
The surgical kits also contain templates for the graphic representation of the implant measurements to allow choosing the most suitable implant diameters and lengths by means of x-ray or tomographic methods. Two test implants for practical exercises are also supplied with the kits (not to be used on patients because they are anodised and not sterile), as well as the surgical manual for Premium and Kohno implants. A maintenance set for the dynamometric ratchet is also supplied, including the driver for quick torque adjustment and a tube of lubricating gel for torque-control ratchet maintenance.

Initial, intermediate and countersink drillsAll Sweden & Martina drills are made of surgical stainless steel with high resistance to corrosion and wear. The extreme accuracy of design and production allows use completely free from vibrations and oscillations.
Precision drill: very sharp and precise, it facilitates the opening of the implant site especially in the case of very hard cortical bone.
Intermediate drills: the shape with progressive diameter of the 2 intermediate drills allows gradual expansion of the bone when using a drill with a diameter much larger than the previous one, so as to reduce bone stress to a minimum.
Pilot drill: the different heights of the laser markings allow better visibility in the surgical field and greater ease of recognition of the different depths during use.
Countersink drill: ideal for preparing the coronal part of the site in the case of implants with a prosthetic platform wider than the diameter of the spires.

Final drills and stopsAlso made of surgical stainless steel with high resistance to corrosion and wear, Premium Kohno final drills present a number of cutting edges proportional to the hole diameter, so as to allow a continuous and homogeneous cutting movement and greater instrument stability during operation. All this enables high-precision implant preparations to be obtained, with consequent ease in inserting the implant.
The surgical drills have a spiral designed to allow the operator better control of the forward movement and better centring during the drilling phase.
The coloured rings make it easy to recognise the dedicated instruments for each implant diameter.
The stops are inserted from the tips of the drills, with great ease and speed.

Always made of surgical stainless steel with high resistance to corrosion and wear, the final conical drills for Kohno implants (Straight, SP and TG) are characterised by four straight cutting edges.
Final conical drills and their stops
Laser marking: the 0.8 mm thickness of the laser marking corresponds to the height of the polished neck of the implants, thus allowing precise control of the depth of preparation of the surgical site.
Straight edge cut: the straight edge cut gives a great cutting capacity and allows much of the bone removed to be collected.

Drills for distal sectorsAs an option, shorter drills are available that are very practical in distal sectors with limited oral opening. They come in a wide range of diameters and are also useful for preparations in extremely compact bone where, in the most coronal portion, you want to widen the preparation diameter by 0.10 mm with respect to the size of the standard drills to facilitate the insertion of the implants. On the other hand, in low-density bone they can be used to under-prepare the implant site so as to obtain optimum primary stability.
The drills have etched depth lines from 7 to 15 mm.

OsteotomesA set of steel osteotomes is available, useful for maxillary sinus floor expansion and lift protocols via the crestal bone. The particular design of the tips, which follow the profile of each different implant in the range, allows the creation of particularly precise sites even after the compacting of very spongy bone matrix.
Osteotomes: the laser-etched codes on the handles show the diameter and height of the corresponding implant, so as to facilitate recognition of the correct surgical sequence.

The implant site of the specific Shorty implants with lengths of 7 mm and 8.5 mm can be prepared with the drills contained in the standard surgical kits. However, don’t forget that the drills contained in these kits, like all the standard drills, require an over-preparation connected to the measurement of the drill tip (see page 39). The drill tip has a guiding, centring and penetrating function and normally characterises the instrument’s ability to move forward, determining its efficiency. For this reason, even though it limits the choice of the preparation length, the drill tip is a normal aspect of traditional surgical protocols.On the other hand, the choice of a short implant is generally connected to the lack of available bone height in the implant site, so it would be desirable not to have to engage a working thickness with the tip of the drill, but instead to lodge a longer implant. For this reason, a drilling kit was created for preparing the Shorty implant sites of the Premium Kohno system (Straight, SP and TG) , in heights of 6, 7, and 8.5 mm with very short tip, where a portion of over-preparation is not deemed necessary (see page 39). For example, to insert an implant of 6 mm using the Shorty drills, an actual hole of 6 mm will be made, and not 6 mm plus the portion of over-preparation.The Shorty drills allow dedicating the entire bone available to lodging the implant, without any waste. Moreover, the Shorty drills have the advantage, compared to traditional drills, of being shorter than the standard drills (24.85 mm instead of 35 mm). This important feature makes it possible to use these instruments even in case of difficult to reach distal sectors or small oral openings.

The transgingival healing caps, in grade 5 titanium, can be identified by a laser marking that reports the diameter, emergence profile and height. In case of transgingival healing caps with straight emergence profile, the marking only reports the platform diameter and height.The transgingival healing caps must be tightened at a torque of 10 Ncm, using the HSM series of drivers, the full details and codes of which can be found on page 54.
