Overview
• CHIP is an intensive lifestyle program with proven results based on years of scientific research.
• Participants commit to make lifestyle changes for a number of weeks (determined by each program).
• Change happens in the way you eat, move, and think- it will challenge you for the better!
• After the program, participants decide if they want to continue or revert to their old lifestyle.
What is CHIP?
• CHIP focuses on a whole of health approach to living.• CHIP emphasizes lifestyle habits like stress
management and forgiveness, as well as eating more plants and whole foods, not eating less.
• CHIP combines evidenced based lifestyle medicine research with behaviour change principles to evoke positive change in participants.
• CHIP graduates often experience weight loss, lower blood pressure, lower cholesterol, more energy, and more enthusiasm for life.

CHIP is a program about making good choices. It seeks to help people move from the left side
of the spectrum to the right side.
NOTE: The science indicates that for disease reversal, plant based eating gives the best outcomes.

Virtual CHIP
What is Virtual CHIP?•Virtual CHIP is a way to bring an evidence-based intensive lifestyle intervention to more people. It involves the same program components of a traditional CHIP class such as a trained facilitator, CHIP kit, and blood draws, but is done via conference calls with participants completing some of the course work on their own time.
•Virtual CHIP is designed to provide group support, a key component of traditional CHIP classes, through two weekly “community engagement” group conference calls.
•Accountability is built into Virtual CHIP through retention-check quizzes.
• Available to employees working in multiple locations.
• Ability to share program content with family or other support networks by watching videos at home.
• Less workday time commitment, which makes CHIP more attractive to busy professionals who would like to improve their health.

Program Components Overview
Getting Started with Virtual CHIP • Find a CHIP facilitator (either internal or external).• Arrange blood draws and Health Risk Assessment. • Promote Virtual CHIP as a wellness program to your
employees.• Set up a virtual information session and enrollment
process.• Create a detailed syllabus for participants.• Order CHIP kits.• Start the class.
Use your own facilitator:• Use a facilitator within your
company that has been trained in Virtual CHIP.
Use our facilitator:• Select an experienced
facilitator from LMI or LHA to work with your organization.
Facilitating Virtual CHIP: Two Options
• Each company will need to arrange biometrics and blood draws for all participants.
• 2-3 blood draws to include:• Lipid profile• Fasting glucose(Prior to week one, at week six, and/or at week twelve)• Participants will also report BMI and
blood pressure.

Health Risk Assessments: Options
Eligibility, Incentives, and Promotion
• Determine who is eligible for CHIP in your organization.
• Define incentives and cost sharing if any.
• Send out information explaining the program and determine who has interest.
• Explains CHIP in more detail to potential participants and the commitment they must make if they decide to participate.
• Can be done by conference call, webinar, and/or in person.
• We recommend using our PowerPoint, co-branded with your organization.

• Application may include: • Why participant wants to attend/what they
want to get out of the program • Moral contract • Cost sharing information if applicable• Attendance agreement • Clear instructions on “next steps” for

Community Engagement Ideas
•Stress to the group the importance of building a community to enhance peer support.
•Collect photographs and short bio of all participants and share with group.
•Use a social network tool to allow the group to share ideas throughout the class.
•Call on people by name during the calls to share their experiences.
•Share CHIP-friendly recipes, and encourage people to try them and report to the group.
•Develop a walking competition within the class.

Sample Program Materials
Sample Eligibility Requirements• Full-time, part-time employee or PRN.• Completed application for participation in the pilot.• Access to an Internet-enabled computer for virtual learning and a telephone for virtual
class/support calls. • Targeted Participants would have:
• BMI of greater than 29 OR• BMI greater than 25 plus 2 chronic health conditions such as:
• Hypertension• Diabetes• Pre-diabetes• High Cholesterol• Coronary Artery Disease• Arthritis• COPD• Asthma
• Others may be considered after interview with program facilitator• Complete biometric screening (tests performed no longer than 90 days before the
class start date can be accepted) • Testing will be offered for those who do not have recent laboratory data
Participants must attend (telephonically or in person) weekly classes to remain eligible and cannot miss more than one class per month.

Sample Compliance Requirements
•Watch assigned video; complete quiz and workbook assignment prior to class.•Attend at least 14 of the 18 community engagement calls.•Cannot miss two classes in a row.•Complete all biometric screenings and Health Risk Assessments in the appropriate timeframe.
Examples of Accountability
• Have participants sign a moral contract outlining expectations and commitment.
• If the company pays for participation and employee quits the program, they will have to pay back 50% of the fees through a payroll deduction.

I further understand that I will join in on 90% of the conference calls and follow the CHIP lifestyle principals as best as I can for its 9 week duration. I will watch the videos, complete the quizzes, workbook, Health Risk Assessment, and blood draws as requested.
I agree that if I do not meet the requirements outlined above, I will be responsible for reimbursing the organization the cost of the program which is $______.
After the program is completed I will then decide how much of this new lifestyle I want to continue.
