Near miss 14/06/12 Hit against a safety board and
received a knock but no injury while
on a normal routine job of chemical
loading
Safety board was wrongly positioned in
work area. Mechanical has been
informed to reposition board
2 MMD- Chemical
warehouse zone 3A
Near miss 13/06/12 During BSC audit inspection Mr. AKS
right leg slipped into drain through
the open spaces between gratings.
There was no injury.
Scheduled inspection of all gratings to
be included in housekeeping inspection
checklist
3 PPOU (Chemical
loading unit 22)
Near miss 13/06/12 Nearly stepped on a brown snake Abandoned pota-cabin harboring
reptiles including snakes. Plans have
been concluded to evacuate pota cabin
and clear the area.
4 MMD warehouse Near miss 05/06/12 A tyre pressure cylinder machine
almost fell of the forks of forklift
during lifting operation.
It was due to wrong lifting practice.
Forks were wrongly positioned on
machine. Refresher material handling to
be handled again during Tool Box Talk
5 Olefins Plant Dox
unit (Hot section)
Near miss 05/06/12 Permit was issued to connect hose
from BFW line to 1-CA-103 for back
washing. On attempting to carry out
back flush, it was discovered that
the hose was connected to a DS
drain line.
Non supervision of mechanical
maintenance personnel by process area
operator. Area operator must always be
notified before carrying out jobs in their
unit.
6 Olefins-Hot section Near miss 05/06/12 The metallic scrap carrying truck
entered ISBL area without vehicle
entering permit and went very near
to K-1
Safety Ambassador was not immediately
available to sign vehicle entering permit.
Security at the entrance gate has been
caution to prevent unauthorized
entering of vehicles

7 BaggingForklift
charging area
Near miss 30/05/12
(Reported in
June)
High electrical spark at the forklift
charging area while unplugging a
charger of the forklift battery. There
was no fire incident
Battery was covered during charging and
charging cable was found defective.
Strict compliance of procedure for
charging battery was emphasized to
operators. Note: Only competent
electrician should be allowed to charge
battery
8 Olefins 1-E-51A Near miss 10/05/12
(Reported in
June)
Work was started on 1-E-51A
process I/L flange opening when an
explosive noise was heard. It was
discovered that while work was
going on, Nitrogen was fully lined
up.
Permit was issued without confirming
the site preparation. Permit issuer must
confirm equipment ready for
maintenance before issuing permit for
jobs
9 PPUONitrogen
plant-ASU II
Compressor filter
Near miss 18/06/12 In an attempt to climb the fixed
ladder attached to ASU II
compressor suction fitter, my head
hit the ladder cage. I was wearing
helmet. No injury
Standard requirement were not
followed in installing the ladder. It will
be reviewed in line with OSHA standard
for installation of fixed ladders

plant-ASU II

Near miss 20/06/12 In an attempt to climb the fixed
ladder attached to ASU II
compressor suction fitter, my waist
hit the edge of the ladder cage. Near
miss incident had earlier been
reported on same ladder
Standard requirement were not
followed in installing the ladder. It will
be reviewed in line with OSHA standard
for installation of fixed ladders
11 Old Admin-New flats
Reported by MarbleHead contractor
Near miss 11/06/12 During block laying in block A & B, a
worker in an attempt to rest foundhimself in an opening (duct). He was
held back when his coverall got
entangled to a nail. He quickly
struggled out.
Scheduled inspection and close out of
observations should be strictly followed.All opening that cannot be immediately
covered should be barricaded and
warning signs placed.
12 Old Admin-New flats
Reported by Marble
Head contractor
Near miss 6/6/12 A worker carrying a concrete on a
concrete pan to the slab. He fell off
when he attempted to hold on to
the ladder rail. Unknown to him that
Supervisor & HSE officer to ensure
regular work place inspection

has pulled off.
13 PPCB staircase-Power
Plant
Near miss 19/06/12 The staircase lighting fixture cover
was observed not well secured after
maintenance job and hung loosely
with the potential to fall and hit
somebody
Close supervision of all maintenance
jobs should ensure by all Supervisors
and Engineers
14 Canteen Injury
Incident
20/06/2012 Pressure cooker got bust and lid hit
oil pan, hot oil fell on the leg of the
cooking helper causing burn injury.
