THE VITALITY™ PROGRAM -
The Vitality program is a comprehensive and customized wellness program the helps people lead their healthiest life. When Vitality members work out at a Partner Health Club they earn additional perks.
VITALITY™ PARTNER HEALTH CLUB PROGRAM -
Who Is Eligible –
Active Vitality members and eligible spouses can participate in the Partner Health Club Program
Eligible members can earn two types of monthly rewards: POINTS and/or SUBSIDIES (Subsidies range from $10-$35 per month)
How It Works –
Vitality members enroll online only! Vitality members are required to enroll on the Vitality website.
No physical enrollment form needed at the club level!
Vitality members are required to present their member ID card at the NIHCA Participating fitness center, in order to activate their workout tracking submission to Vitality.
Confirmation is sent to the NIHCA participating fitness center notifying them of member enrollment.
NIHCA participating fitness center submits Vitality member’s workout dates, membership dues, etc., to Vitality’s service processor monthly.
Vitality awards their members with points based on workout dates and will issue subsidy payments, as a direct deposit only, to the members.
Members are also required to workout at their fitness center at least 12 times in a calendar quarter to maintain their subsidies.
Vitality promotes the advantages of Partner Health Club participation to its members. Program details are available in their entirety on the Vitality website for members.

As of January 1, 2011 there will be some changes to UCARE. Ucare will no longer require 8 visits per month.
Starting January 1, 2011, Ucare members will not be required to meet a minimum number of visits per month.
The Eligible Ucare members will need to have an active a membership at your facility. (This change was
requested by Centers for Medicare and Medicaid Services (CMS), Ucare’s federal regulatory agency.)
The amount of the reduction will change.
The amount will change from $20.00 to $15.00 per month per member. A family or couple membership may
have up to 2 adults participating with a maximum reimbursement of up to $30 per month.
New UCARE Members If you sign up new Ucare members, they will be required to fill out a NEW Ucare Enrollment Form, while providing their health club with a copy of their UCare member ID. (Please note: The NEW Enrollment form will not be done or distributed until January 2011. Please use the old or current enrollment forms at this time.) Being Ucare will not require a minimum number of visits for their Ucare members participating in the dues reduction program, they still want you to submit each month for the number of times each Ucare member worked out. If the Ucare member didn’t workout at all, then please report “0”. You will still be required to submit a monthly Ucare file, in order for the Ucare members to be reimbursed. Frequently Asked Questions: If the member is paying $0 to the club what will they get back? Members can never make money on the program. So if the member is not paying anything for their membership, they will not receive anything from UCARE. UCARE will match the membership fees up to $15 per person For example: If the member is paying $1 for their membership, they will receive a credit of $1 from UCARE. If the member is going to more then two clubs? (Example: snow-birds) If both clubs process for the same month – the member will receive the reimbursement from one club; it all depends on which club’s file gets processed first. So which ever club processes their UCARE file first, is the club that will reimburse the member. Should clubs report attendance? YES, Ucare wants these stats. If the Ucare member has more eligibility questions, please have them call UCare Customer Services at 612-676-3600 or 1-877-523-1515 (toll free). Please call NIHCA with additional question on this 2011 change. As soon as we have some additional information on the new enrollment forms we will send them electronically to you.

ST. ALEXIUS HUMAN PERFORMANCE CENTER
St. Alexius’ Human Performance Center is a distinctive facility designed to meet the physical and fitness needs of our customers. Our specially trained and certified staff is dedicated to the improvement of your physical mobility and athletic ability through a variety of training techniques. Whatever your goals, the professionals at the Human Performance Center work with you to help you achieve them.
Conveniently located on the campus of St. Alexius Medical Center in Bismarck, ND, we have access to some of the region’s most highly respected healthcare professionals.
Services include:
Therapy ~ The Human Performance Center offers a variety of therapy services including physical, occupational and hand therapy. There is a wide range of outpatient services that the professional and certified staff at HPC are happy to provide to patients whether they are physician referred or self-pay patients.
The Acceleration Program ~ The Acceleration Sports Training is part of the Athletic Republic; one of the premier sports training programs. The scientifically designed program features progressive conditioning levels which develop an athlete's quickness, agility, dynamic sprint speed, strength and overall confidence. The program can be used as a pre-season conditioning program, an ideal post-season conditioning program or part of a year-round training program designed by the professionals at the Human Performance Center.
Fitness ~ The Human Performance Center is a unique fitness clinic, providing complete fitness services for every person who has a desire to become healthier, no matter what age or level of fitness.

As of 9/1/10, Fargo Public Schools added eligibility for a spouse onto the Well U Health Club Credit Program. Previously, eligibility in the program had been limited to solely the employee of Fargo Public Schools - as of September 1, 2010 both the employee and employee’s spouse will be eligible to collect an up to $20 reimbursement for a total of up to $40. Please remember that no member can make money on this program, therefore with the addition of a spouse, dues are going to have to be
an eligible employee, the spouse must add an “S” to the end of the employee badge number. This will become the unique identifier for the spouse, working as an insurance policy number would.

By choosing to pay monthly, there are
additional fees that do apply which are
based upon payment option choice. In
addition, by choosing to pay monthly,
our fitness centers are agreeing to the
annual contract laid out when your
fitness center joined NIHCA and are
responsible for payments throughout
that year.
When choosing this option, please
make sure that if anything changes at
NIHCA is making several changes
with our billing system. Last year,
NIHCA changed the look of our
invoices. This year, NIHCA has been
proud to announce new payment
options for our fitness centers.
Previously, our fitness centers have
only been allowed to pay in one
annual payment. As of 2010, NIHCA is
allowing our fitness centers to pay
your NIHCA renewal monthly. The
monthly payment amount will be listed
on your invoice and will be calculated
appropriately. The calculations are
made by our billing department and
your facility that might affect your
payment you in fo rm NIHCA
immediately so we can make the best
decision for your fitness center.
Examples of these situations include
closing your bank account or credit
card, selling your facility, closing your
facility, etc.
