Community Nursing Team About the Community Nursing Team The Community Nursing team provide home or community based nursing care, treatment and support to people living in North East Lincolnshire who are housebound or have severe mobility problems which mean they are unable to aend the surgery for treatment. Our team work in partnership with local GP’s delivering care and support to people requiring on-going nursing care. They are based within several of the GP Pracces throughout North East Lincolnshire. The teams include Nurse Managers, Nurse Team Leaders, Complex Case Managers, Associate Praconer, Community Nurses and Health Care Assistants and provide a range of long term condion management from leg dressings, injecons and end of life care. Information for patients and carers

Our AimTo provide in-home nursing support to individuals who through ill-health or mobility problems are not able to access treatment in their GP practice. We provide a high quality of care to our patients and carers during what can be a very difficult time due to poor health.

What can I expect?Your Community Nurse will be in uniform and carry a care Plus Group Identity badge, please request to see this at any time. If you are not sure about the identity of the nurse and she/he cannot show you a Care Plus identity badge, please do not allow them into your home.

At the initial visit and discussions with you an assessment will take place and a care plan will be drawn up based on your nursing care needs. This plan will include information about treatments and frequency of visit. The Nurse will go through your care plan with you and you will be asked to consent to it.

You may also be asked about sharing your medical information with other health care professionals including your GP.

Some of our registered nurses have the ability to prescribe certain products to support your treatment.

It is a legal requirement that all this agreed information will be documented within your patient record using an electronic mobile device.

What services are not included?Our Community Nurses provide nursing treatment and care and therefore other social care services are not included, such as:

• Collecting and delivering prescriptions

• Visits without nursing need

• Personal hygiene

• Housework, washing and shopping

Our nurses will, however be able to signpost you to other services and providers who may be able to help you with these other needs.

How can you access support from the Community Nursing Team?If you are registered with a GP in North East Lincolnshire you will gain access through your GP. We work closely with the GP’s in identifying and supporting adults with physical health problems.

If out of hours you can contact Single Point of Access (01472 256 256) which is staffed 24 hours a day, 7 days a week and is subject to an assessment by a health or social care professional.

How you can help us to help youThere are a few simple things that you can do to help us in delivering the best possible nursing care -

• Please tell us if you are unavailable for any planned visits or if your condition has improved and you are now able to attend your GP surgery

• We kindly ask that you supply liquid soap and paper towels e.g. kitchen roll when we visit you at home to help prevent the spread of infection. Please keep pets away from the treatment area and under control during our visits

• In order to keep yourself and staff safe we may require you to alter your surroundings

• Please consider the needs of our staff and provide them with a smoke free environment
