How to get consumers and providers to use a HRB? • Provide value to consumers and providers, as determined by them.• Provide a secure and trusted environment and processes to

ensure the confidentiality of the data.• Make it easy to use for the consumer, and easy to incorporate into

the physicians workflow.• Marketing and education, so they know about it and know how to

access it.• Convenient enrollment and help.• Forum for making recommendations.• Data collection of what works, what isn’t working and for future

efforts. This legacy of knowledge would aid in creating value in the future resulting in greater acceptance.

How much/what data is needed to provide value to the consumer/user of a HRB?

• Minimally, medication lists and allergies. More is better.

What are the next steps indicated for moving towards provider participation?

• Identifying data that will be valuable to providers, and addressing provider concerns including patient identification, control of data being released from electronic medical record systems, and quality of data in HRBs.

How can timely data be obtained?• Interfaces from multiple data sources where the

consumer receives care or service, triggered by the request of the consumer. The second method is by the consumer entering information themselves.

What needs to be done to earn the trust of the public?• Trusted service organization• Clear policies and procedures that are developed with

consumer input.• Consumer control of the record and output.• Educational sessions to help understand HRBs and

medical information and to answer any questions they might have.

What needs to be done to earn provider trust and minimize provider barriers?

• Trusted service organization• Clear policies and procedures that are developed with

provider input.• Provider guidance in the development and delivery of a

pilot (Physician Advisory Committee). Processes to incorporate the HRB record into their workflow in a manner that provides more value than effort.

Other questions the Applicant recommends be considered for the evaluation plan.

• What information is the most important to consumers and providers to include? Who is willing to pay for the service, and how much is it worth to them?

Our SolutionCombines a newly developed INHS HRB as a data source with Google Health as the consumer-friendly front end.

Our RationaleRelatively quick method to test the HRB model using a variety of external data sources and a large consumer/physician population.

• Consumers – Target Population 300,000- 400,000

Pilot Project Design• Two Patient Populations

1. Associated with independent physician practices which have a referral relationship; high percentage of older adults and individuals with cardiac conditions.

2. Associated with a large multispecialty clinic system which has a system-wide EMR; representative of general population.

• Provider and Consumer Advisory Committees1. Providing guidance on policies and practices,

marketing, enrollment and education.

• Google Health – User Interface (View, Import, Annotate, Delete, Print)– Mechanism to import a variety of health data from external

sources – Retail Pharmacies, Reference Laboratories– Health information web searches

• INHS HRB– One of the imports available from Google Health– Consent management, authentication, privacy and security for

the INHS HRB– A new and separate database on the INHS system specifically

designed for the HRB– Receives data from physician clinics, which contain clinical

information across the continuum of care.

How Consumers Will be Engaged• Goal of 5,000 participants

– Target cardiac patients & general health consumers

• Enrollment Concierge (high touch)• Marketing Approaches

– Broad Community Campaign• Advertising, web, public relations, etc.

– Direct marketing through physician offices• Part of the office visit

– Enrollment “fairs”• In the community

• Feedback– Consumer focus groups to assist in developing and enhancing

messaging and engagement

Pilot Alignment with Goals of the Project• INHS HRB provides a “proof of concept” that

allows consumers to sign up for accounts and directly populate the record with copies of their health and medical records from a variety of health care sources.

• Consumers can access, utilize and annotate this information and add new information of their choosing

• Consumers can control when, how, and with whom they will share all or portions of their HRB account.
