Note ID: 7C1110293200000 Beg Abut -- Abutment: Seats and Pedestals -- Rated 4, Was 4 Referenced Photos: "2" The seats are developing a build-up of debris which is contributing to the corrosion of the structure's lower components. The fascia areas emit hollow soundings, however, they appear to well support the bearings above. No structural cracking is evident. Previous repair work is apparent. No significant change from the previous inspection. The rating remains "4".

Note ID: 7C1110293200002 Stream Channel: Erosion and Scour -- Rated 4, Was 4 Referenced Photos: "4", "5" All of the stream banks exhibit erosion. The fine, silty sand material is being eroded leaving banks with steep drop-offs to 4 ft. high along the shorelines. Currently, no impact on the structure is occurring. No change in condition since last inspection. The rating remains "4".

No accumulation of sediment or debris is evident in the channel. The rating remains "5".

Note ID: 7C1110293200006 Span 001 -- Deck Elements: Wearing Surface -- Rated 4, Was 4 Referenced Photos: "8" The entire wearing surface has moderate longitudinal cracking, primarily in the wheeltracks, with alligator and transverse cracking eminating.The asphalt remains tight with no raveling or delaminating occurring. The ride quality is not impacted. The approach interfaces exhibit distress having cracking with minor raveling and delamination across the full width. A very mild impact on the ride quality is occurring. A large concern is the leaking through to the structural deck. The cracking promotes leaking which will continue to accelerate the deterioration of the already significantly distressed concrete deck. The condition is unchanged since last inspection. The rating remains "4".

Note ID: 7C1110293200007 Span 001 -- Superstructure: Structural Deck -- Rated 3, Was 3 Referenced Photos: "9", "10", "11", "14", "15" Mapcracking w/ moderate efflorescence is scattered over the entire underside of the deck. The Beg. half of Bays #1 and #3 (Fascia Bays) and at midspan of Bay #2 are the more severe locations. Soundings revealed approx. 25% of mapcracked areas were slightly delaminated. No significant reflection of distress, depressions or deformation, on the top of wearing surface or along the bottom side is exhibited. The active cracking of the Wearing Surface above continues to promote leaking. No Deck Sketch as the extra Photos and Notes sufficiently describe the current condition. No change in the condition. The rating remains "3".

Note ID: 7C1110293200008 Span 001 -- Superstructure: Paint -- Rated 4, Was 4 Referenced Photos: "10", "12" Several of girders have upto 25% paint loss with surface rust and staining occurring. No section loss is evident. The condition remains unchanged. The rating remains "4".

Note ID: 7C1110293200004 End Abut -- Abutment: Bearings, Bolts, Pads -- Rated 4, Was 8 Referenced Photos: "1" The outer anchor bolt on the Lt fascia girder has corroded and broken off. The remaining anchor bolts are connected, however, are also corroding. The rating is lowered to "4"
