PART “A”

(For Circulation among State and local Branches of IMA) Goal

To assist the efforts of the Revised National Tuberculosis Control Programme (RNTCP)

in achieving its goal to bring down the burden of TB in India until it ceases to be a public

health problem.

Objective

To improve access to the diagnostic and treatment services of DOTS and thereby improve

the quality of care for patients suffering from Tuberculosis in fifteen states and one union

territory of India.

Project States:

Andhra Pradesh, Bihar, Chhattisgarh, Gujarat, Himachal Pradesh,

Jharkhand,, Kerala, Maharashtra, Odisha, Punjab and Chandigarh,,

Rajasthan, Tamilnadu, Uttar Pradesh , Uttarakhand and West Bengal

Project Scope

The Project will provide impetus to the private health sector to join and participate in

RNTCP. It is important that the participation of private sector be fully in accordance with

the policies of RNTCP. The programme will run till 2015 and consist of the following

activities.

1. Branch level CMEs – to sensitize MBBS doctors, members of IMA and non IMA

members, with an aim to encourage them to diagnose and manage TB patients

and notify the diagnosed patient to RNTCP and ensure cure.

2. District Training Programme (DTP) – a 4-hour modular training that enables

private medical practitioners to identify, diagnose and categories TB patients and

to contribute to the RNTCP through the establishment of PHIs , Designated

Microscopy Centers (DMC) etc at their clinics/hospitals.

3. Annual State Workshops/Review Meetings (in each Unit) – for one (1) day with

district IMA and RNTCP representatives of the Unit as participants to review the

performance of the project.

4. Annual National Workshop/Review Workshop – to sensitize national and state

IMA leaders and RNTCP Programme Officers to RNTCP and PPM and to

review the performance of the project each year.

5. Facilitating the IMA member establishments as a Private Peripheral Health

Institutions(PHI) participating under RNTCP :

A PHI is a health facility which is manned by at least one medical officer. All

health facilities in the private and NGO sectors participating in RNTCP are also

considered as PHIs by the programme. Some of these PHIs also function as

DMCs (.Designated microscopic centre). Some of the schemes for PHIs are:

1. Designated Microscopy center and can run the sputum collection scheme if they

have the infrastructure for collection.

2. Scheme for treatment adherence scheme ( PHI as DOTS centre.)-

6. Facilitating notification of TB patients by private practitioners :The Government

has ordered that the district nodal officer should be notified in the prescribed

format of all patients diagnosed of TB. The project will facilitate the notification

by the private Practioners. (Government order & reporting format in the

annexure).

Note: It is important to note that these programmes ( 1-4) are not to be clubbed with any other

IMA function or program Implementing Organization and Responsible Bodies RESPONSIBILITIES OF THE IMA NATIONAL WORKING GROUP (TB)

At the IMA Headquarters (HQ), the National Working Group (NWG) will be the policy making body and will

constantly monitor and review the implementation of the project. It will submit periodical reports to the CWC and Central Council. It will also submit the necessary reports and accounts to the Government of India and GFATM through the Central TB Division (CTD). The NWG will interact regularly with key partners including CTD and WHO. The National President and Hon. Secretary General are ex-officio members of the

NWG. All the funds will be received and disbursed by IMA HQ through a separate account for the project being operated by the Secretary General and Finance Secretary as per IMA Rules. The NWG will be the monitoring and evaluating body for all project activities. The activities of the NWG would include:

1. To publish a dedicated newsletter which will be circulated to all members of IMA in the project States and to office bearers and central council members of other states. This newsletter will contain academic and administrative information on RNTCP and the project.

2. To insert in JIMA every three (3) months an article to sensitize and educate members of IMA on RNTCP and Tuberculosis

3. To reproduce Information, Education and Communication (IEC) material for IMA members of the project States. 4 Conduct field visits to Project states on a periodic basis for monitoring the project activities.

Responsibilities of the State

At the state level, the State Secretary and Treasurer only will receive and operate the funds as per the guidelines through a separate saving bank account for each Unit in a nationalized bank for this project which has electronic funds transfer facilities.

Responsibilities of the State/Unit Coordinator

Each Unit of a project state will have a Unit Coordinator for the project. The Unit Coordinator will be appointed by Hony. Secretary General in consultation with NWG

and State IMA. This will be reported to NWG through the State Secretary. (S)he will

coordinate all activities of the project within the Unit.

The specific responsibilities of the Unit Coordinators are as follows:

1. To maintain close liaison, and good rapport, with the State TB Officer of the respective State Government

2. To organise the annual Unit workshops (or review meetings) in the respective project Unit in consultation with the State President & State Secretary

3. To coordinate the organization of the DTP in each district of the Unit

4. To allot CME Programmes to the local branches in the Unit in consultation with

the State Secretary as per guidelines of the project

5. To work closely with the NWG and State Secretary to attain the goal and objective of the project

6. To collect all the reports and accounts of the CME/DTP through the concerned IMA Technical Consultant and forward them to the State Secretary for onward transmission to IMA HQ. for release of payment after due verification ,sign and stamp each bills and vouchers of the activity accomplished.

7. Ensure project activity report and expenditure of the activity accomplished submitted to State on time.

8. To maintain the records concerning to the Project and the Project Indicators e.g. MOU signed under RNTCP, Notifying TB patients, PHIs in each district of the Unit

9. To participate in state RNTCP review meetings at least on a quarterly basis

10. To ensure participation of the Unit in the World TB Day (March 24th) celebrations.

IMA Technical Consultant

Each project Unit will have a full-time medical officer contracted by the IMA as a Technical Consultant for project.(S) he will function in the respective Unit . The role and responsibilities of the position is mentioned separately in PART-B

DISTRICT CO-ORDINATOR:

The State Secretary and respective Unit Coordinator will nominate a District Coordinator (TB) in each district. The responsibilities of the District Coordinators are as follows:

1. They will coordinate with the local branches in organising the branch level CME programmes in that district

2. They will organize the DTP along with Unit Coordinator

3. They will maintain close liaison and good rapport with the District TB Officer (DTO) and establish close cooperation between the private health care facilities and

government machinery in the district

4. They will attend the annual Unit Workshop/Review Meeting along with enrolled PPs for RNTCP.

5. They will utilize the services of the IMA Technical Consultant and coordinate with

RNTCP programme officers

6. They will participate in district RNTCP review meetings at least on a quarterly basis .

7. They will ensure participation in World TB Day (24th March) RNTCP activities in the district every year and in other advocacy and awareness programmes organized under RNTCP

Branch Coordinator

The Branch Coordinator will be nominated by the local branch in each of local branches

in the project. The responsibilities of the Branch Coordinator are :

1. Coordinate with the branch secretary in organizing CMEs

2. Confirm the participation of IMA members in the activity.

3. Propose names of members for Dist. Training programme

4. Increase the participation Of Branch Members in RNTCP including Notification and formation of PHIs

Branch CMEs

The Unit Coordinator will allot CME programmes to the branches as per the guidelines

with an average attendance of 40 members for each session. The objective of the CME programme is to sensitize the IMA members to RNTCP and to the Standards of TB Care and enable them to diagnose and manage TB patients and notify the diagnosed patient to RNTCP . The members are encouraged to establish PHIs. The branch CMEs are to be planned and organized by the IMA Technical Consultants along with the local branches and with the cooperation and assistance of Branch /District Coordinator, DTO, Trained MO-TC and the WHO Technical Consultants. The State TB Cell may re-allocate the branch CMEs taking into consideration the strength of the branch membership. Structure of Branch CMEs

On an average, one CME on RNTCP and ISTC, STCI will be conducted in each IMA local

branch every year. However, as some branches are very large, they may host more than one CME each year. At the same time, branches with very few members will be clubbed with larger ones. In branches with few members but which cannot be clubbed with larger branches due to great geographical distance, a low CME attendance may be permitted in consultation with HQs. The structure of project CMEs at local branches is as follows:

1. The CME programme will be of 2 hour duration

2. CME should only be based on the resource material for the same and supplied by IMA HQs./ NWG

3. The IMA Technical Consultant and one of the following should be the resource person for the CME

a. DTO (the MO-DTC or MO-TC may also be deputed by the DTO)

b. RNTCP/WHO Consultant for the District (deputed by the DTO/STO)

c. Any other doctor who has undergone the 2-week RNTCP modular training

4. The budget for each branch CME is Rs. 20330/-, distributed as follows:

and should not exceed allocated budget .

5. IMA Hall can be used for programme and can be charged under Boarding and Venue rent.

6. There will be no payment for traveling expenditure to any of the participants

7. Modalities of activity amount release to the branches: For each CME an advance of Rs 8000/- cheque in name of the hosting branch will be handover to IMA Technical

Consultant by the State Secretary after confirmation of activity date. The Technical Consultant will hand over the cheque to the Branch Secretary for organizing the activity. The balance payable amount (if any) of the activity will be released to the Unit Account for further payment to the branch. All payments need to be done through a/c payee cheque only and to the branch account.

8. The CME report shall be submitted:

a. In the proforma enclosed along with a statement of account

b. With original bills and/or vouchers, photographs of the meeting with evidence of the location and date; details of the event should be written on the reverse

c. With the Attendance Sheet in original

The IMA Technical Consultants will be responsible for collecting the reports and accounts from the branches within 7 days from the date of activity completion and submission of the same to the Unit Coordinator for verification and for onward transmission to the State Secretary. The Unit Coordinator will verify and forward the reports to the State Secretary for onward transmission to IMA HQ for release of funds. The payment will be made by State Secretary to the Secretary of the Local Branch on receipt of the same from the IMA HQ. A record of the reports and accounts should be retained by the State Secretary/Unit Coordinator.

The Success of the sensitization programme in the branch will be measured by

partnership schemes with the local DTO. How many of these schemes have become operational.)

District Training Programme(DTP)

A district level RNTCP training programme (DTP) will be organized annually in each district for those IMA members and PPs who have been sensitized during the Branch CMEs. Training will be planned and organized by the IMA Technical Consultant along

with the District Coordinator, supported by the Unit Coordinator for the Unit

1. The number of participants shall average 20 involving only those engaged in active Private Practice, who have been sensitised in branch CMES, who are managing significant numbers of TB patients and who are willing to establish PHIs Those selected for training should be willing to work with RNTCP and should strongly advocate DOTS for their TB suspects and patients.

2. The training is for 4 hours duration. The faculty and training material are as per the existing guidelines for training medical practitioners under RNTCP

3. The DTO, RNTCP/WHO and IMA Technical Consultants shall form a part of the faculty for the training programme

4. Different participants shall be trained each year

A Participants Feedback form to be filled by all doctors undergoing the training. These have to be retained by the TCs as part of the M & E Process and will help in Quality Assurance. Post test and pre-test is mandatory in DTP.

The budget of District training session is Rs. 30,180/- distributed as follows:

activity date. The Technical Consultant will hand over the cheque to the Branch

Secretary for organizing the activity. The balance payable amount (if any) of the activity will be released to Unit Account for further payment to the branch after scrutiny at HQ . All payments need to be done through a/c payee cheque only.

9. The DTP report shall be submitted in the enclosed Performa along with the following

documents in original:

a. Bills and vouchers in original

b. Photographs of the meeting with evidence of the location and date;

c. Attendance Sheet in original

d. Personal Performa with TA receipt

The IMA Technical Consultant will be responsible to collect these documents and forward them to the Unit Coordinator who will verify and further forward them to the State Secretary for onward transmission to IMA HQs. for release of balance payment. The payment will be made by State Secretary to the Hon. Secretary of the IMA, Local Branch (host Branch).

The payment for the DTP can be made to the account of the local branch hosting the event. Alternatively, it could be organized directly by the state office through the Unit Coordinator. Payments made by the state office can be made only to the account of the

local branch or alternatively handled by the state office itself. Under no circumstances, shall payments be made to individuals in the branch or district.

IMA Technical Consultants will collect the MoUs of trained PPs who have signed RNTCP schemes and submit the same to the DTO under information to the Unit Coordinator. Copies of the MoUs submitted shall be photocopied and retained by the Technical Consultant.

The MOUs have to be filled with complete details of the PP along with name and signature. Before submitting the MOUs to the DTO, a list of MoUs has to be prepared and the DTO should be requested to acknowledge receiving the MOUs by signing the list, which is to be retained by the TC.

The success of the DTPs will be measured by:

The number of participants who were trained

The number of PPs trained who established DOTS centres , DMCs, PHIs or who have signed an RNTCP scheme as a result of the training

The number of trained PPs who have received accreditation from the DTO/RNTCP, through the issuance of certificates and/or sign boards stating that their clinics were DOTS Centres

PART “B” of INSTRUCTION MANUAL FOR IMA-GFATM-RNTCP-PPM-RCC PROJECT

(For Use of State /Unit under the Project)

The NWG member(s) will advise the State TB Cell on behalf of Headquarters. State/Unit TB Cell meeting

The convener will inform IMA HQ well in advance about the date, time and venue of the meetings to be held. The HQs. may depute an NWG member to attend the meeting. The travelling expenditure of the participants will be reimbursed from their respective travel budget. In addition an amount not exceeding Rs.5000/- per meeting will be reimbursed on submission of bills as part of M&E activity.

Finances of the State/Unit TB Cell

Funds will be received by the State Secretary and will be operated through a separate account for each Unit in a Nationalized Bank with a Multi City Cheque Facility. It will be operated by the State Secretary and State Finance Secretary as per IMA Rules. Unit coordinators & Technical Consultants are not authorized to operate the project accounts. The accounts along with original bills are to be submitted to IMA headquarters by 7th of the following month.

All Project accounts are to be maintained by the State, for each Unit separately by the State Secretary and Finance Secretary. Reports

The State TB Cell is required to submit:-

1. A quarterly report on the activities latest by the 15th day of the month following each quarter (15th July, 15th October, 15th January and 15th April of each year)

2. Audited Receipts and Payment( R &P) for every Financial year on the amount received and

payment done for the project and submit to IMA HQRS by 30th April of the following financial Year.

3. The State IMA is expected to maintain the following accounts books:-

a. Ledger, Cash/Bank book, Journal(Day book), Petty cash book, Advance register and Stock register for each Unit separately for CMEs/DTPs/Unit Workshops/Unit Staff/Unit Travel/Unit Office Expenditure

b. Ledger for State Office /Travel Expenditure/ State Staff

It is subject for Audit and inspection by IMA HQRs, Global Fund and CTD

The Annual Unit Work Shop/Review Meetings

1. A one (1) day Workshop/Review Meeting will be organized annually in each Unit of the project by the Unit Coordinator,, in consultation with the State President & State Secretary.

The convener of the Annual Unit Work Shop/Review Meetings will be the State Secretary.

2. The State TB Officer, DTOs and RNTCP/WHO consultants are to be involved while organizing the workshops

3. IMA participants will include all the District Coordinators & Branch coordinators ,Other IMA participants from the districts shall be proportionate to the strength of IMA members in that district ,and will only be private practitioners involved in the RNTCP, as having DMCs, PHIs,

DOTs centre & sputum collection centre.

4. Agenda for the workshop will be sent from the IMA HQRS

5. The State TB Cell members and NWG member(s) will participate in the workshops along with 4 resource persons

7. The State TB Officer, all DTOs and RNTCP/WHO Technical Consultants are to be invited. Their travel expenses are not covered from this project. However, only Government officers will be offered complimentary accommodation under this budget. WHO/RNTCP Technical Consultants shall bear their own accommodation charges.

8. The Budget for each workshop is Rs. 6,50,000/-, distributed as follows:

Modalities of release of fund to the Unit Account : For each Unit workshop( review meeting) an

advance amount of Rs.3,00,000 towards Boarding & Lodging ,Venue rent & Miscellaneous will be released. The balance amount including the TA for participants will be released immediately on the receipt of report and bills and TA forms in original. TA should be paid through cheque only.

As per the CTD procurement manual, a minimum of 3 quotations for the venue to be obtained. The substantially responsive quotation( Cost, Service, Delivery) with the lowest evaluated cost should be accepted.

The accommodation will be only provided to the outstation participants. (Format Attached)

* Participants will be eligible for actual train fare not exceeding 2 A/C train fare or road travel costs (Taxi/Own Car . Originals of train tickets/bus tickets should be submitted along with TA forms. Out-

station participants may travelby taxi or own car provided the total distance does not exceed 300 kilometers both ways and at @ rate not exceeding Rs 10 per kilometers. Outstation participant traveling by taxi should submit the original taxi bill . Participants travelling by their own car should the certify the same in the TA form along with the car registration number. ** Miscellaneous expenses should be supported by original bills duly verified by Unit Coordinator and attested by the State Secretary and should not exceed allocated budget in any case.

All payments (barring misc. expenditure), including travel, should be made by a/c payee cheques only.

Outcome of the Workshop

1. Motivate, educate and give direction to IMA branch leaders about RNTCP & subsequent project updates.

2. Develop district wise action plans with timelines & emphasis will be on TB Notification implementation and establishing the PHI in the State/Units & Districts . Develop rapport and good working relationships between government functionaries and district IMA leaders

5 Review the progress of the project, outline barriers, gaps & challenges and identify their solutions/ corrections- in achieving the targets of the performance indicators /outcome indicator.

6 6 Provide mid-course correction or to make some new intervention.

7 .Lessons learnt from central internal evaluation/Internal Monitoring, Evaluation and Action taken report on the recommendations.

The Unit Coordinators will submit a report on each Unit Workshop/Review Meeting within a week after the end of the workshop to IMA Headquarters. This should include:

1. TA bills of participants with original tickets and personal information proformas

2. Original proper bills( VAT/Service Tax/ Tax identification number(TIN) wherever applicable) of venue, accommodation, refreshments, audio visual equipment, etc.

3. Photographs of the workshop, some having evidence of the date and venue with details on reverse

4. Attendance sheet of the participants bearing essentially their individual names, mobile nos., email addresses and signatures.

The disbursement of funds will be made by the State Secretary and could be through the host local branch or directly by the State Office, but not through any individual.

The State office bearers and Unit Coordinators are expected to work together for the success of the programme and organise all the events in the state. However, in extreme circumstances, where for various reasons (financial, political or otherwise), the functioning of the State/District TB Cell is impaired, the Hony. Secretary General of IMA would intervene and organise the programme in the local, district or State levels directly through the NWG. The decision of the Hon. Secretary General in this matter is final and binding on all. This would be an extreme step taken by the Secretary General to facilitate the success of the project and in view of the accountability and answerability of IMA HQs to partners like the Government of India, GFATM, WHO etc. and also to ensure the credibility of IMA with National & International bodies.

Unit TB Cell

The Unit TB Cell will be provided with the following technical and administrative machinery.

A. An IMA Technical Consultant for each Unit who is a medical graduate( MBBS) and will be recruited under NWG guidelines with a renewable contract.

1. They will be provided with a computer and printer each. This will be the property of IMA and is to be used solely for the purpose of the project and returned at the end of the individual’s contract or, of the project, whichever is earlier. The IMA Technical Consultants are expected to travel extensively to help in planning and organizing the State Workshops/Review Meetings, branch CME’s and DTPs. They should also expend considerable effort and time to meet with individual PPs to:

i. Encourage PPs to participate in CMEs

ii. Ensure mandatory notification of TB patients in the unit

iii. Encourage PP to establish PHIs and submit reports to RNTCP as per the guidelines

iv. Identify from among those who have been sensitized, PPs who are suitable for training

v. Facilitate the recognition, as DOTS Centres, of PPs who have been trained

vi. Mentor PPs who have been trained, trouble shooting issues with district RNTCP officials and ensuring continued compliance with RNTCP/ISTC

2. The consultants should submit their Monthly Activity Plan(MAP)/activity tour plan( ATP) to the Unit Coordinator and to Project Director by the 30/31st of each month. They should also maintain a log book of their work and travel .Monthly Activity Report( MAP) will be sent to the Unit Coordinator, State Secretary, STO, National Coordinator and to the Project Director by the 3rd of each (following) month. Tech. Consultants should always take the cooperation of the Dist. TB officers/WHO consultants. All correspondence with these officials should be marked to the National Coordinator, Project Director and to the STO.

3. The consultants should send their monthly plan of CMEs/DTPs to the State Secretary and State president by the 30th of each month to keep them informed about the activities of the state in advance.

4. The primary responsibility of IMA Technical Consultants is to enroll doctors into the existing schemes of RNTCP, Collect Notifications, Efforts for the establishing PHIs and keep records. They shall also assist the Unit Coordinators in preparing reports and maintaining project accounts.

5. Consultants will be eligible for reimbursement of expenses on the basis of actual, subject to an overall monthly limit (or prorated annual limit) on submission of original bills. Their eligibility details are as follows:

i. Per Diem of Rs.600/- per day for outstation overnight stay

ii. IMA Guest House or hotel stay (where necessary) not exceeding Rs.1500/ + taxes per day

iii. Monthly Telephone expenses on submission of original bills

iv. Internet usage @Rs 1000/- per month on submission of original bill

v. Travel and conveyance expenses as per actual, subject to submission of all relevant documents in original. For own car or hired car, per kilometer limit must be maintained. In case of using hired taxi, original bill is required.

vi. Personal accident Insurance policy Rs 4000/- at the beginning of each contract period

vii. These expenses for each consultant, when compiled, will not exceed the allotted maximum limit as mentioned in their letters of appointment. The consultant will submit the bills to the respective Unit Coordinator, who after scrutiny verify each bills and vouchers and will forward them to IMA HQ for reimbursement on a monthly basis. The monthly claims should reach to IMA, Hqs maximum by 10th of the each (following) month duly verified by the Unit Co-ordinator.

B. Each State will be provided an amount of Rs 4000/- per month for each Unit that state for Secretarial Assistant cum Accountant for use of the Project only on reimbursement basis. The monthly remuneration voucher ( Attached)

C. Each Unit will be provided an amount of Rs 4000/- per month for Secretarial Assistant cum Accountant for use of the Project only on reimbursement basis. The monthly remuneration voucher ( Attached)

D. Each Unit TB Cell will also be allowed expenditure of Rs. 4,830/- per month for telephone( Only one mobile phone number and one landline number will be permitted), stationary(Item-wise

details of stationary purchased, postage( Details of the recipient/Courier(Shippers) and

miscellaneous expenses( Photocopy, etc) on production of actual bills. All the bills must be duly verified by State Secretary before forwarding to IMA , HQRS for reimbursement.

E. The Unit TB Cell members and District Coordinators traveling within the Unit for this project are eligible for reimbursement of travel expenditure as per above rules (state workshop guidelines). The total expenditure for communication and travel of the Unit TB Cell members should not exceed Rs5,910/- per month for each Unit. The travel of Unit TB Cell members and District Coordinators will be reimbursed to the concerned office bearer on submission of the TA bill with all attached documents.( Format Attached). All the bills must be duly verified by State Secretary before forwarding to IMA , HQRS for reimbursement.

F. The State Branch, IMA is eligible for reimbursement up to an amount of Rs.5,910/- per month for Travel, Communication(,( Only one mobile phone number and 1 landline number will be permitted) local conveyance, Yearly Audit etc. of the State TB cell for the project work. For all official visit, ( Reporting format attached) along with the claims a report is to be submitted.

G. PROCEDURE OF FUND DISRBURSEMENT TO THE STATE FROM IMA HQ FOR TRAINING ACTIVITIES: IMA HQ will send a consolidated advance for the planned training activities. After receiving the report and account for the activity the payable amount will be deducted from the consolidated advance, which will be replenished from time to time. The HQ will send a statement of account to the state and units at the end of every month.

H. Annual Targets for each Unit

These targets feed into the overall project indicators which are submitted to CTD by IMA HQ on a quarterly basis:
