    PREFACE

    Due to changes that took place in our insurance coverage locally as well as globally it has become

    necessary to incorporate the same in our Insurance Hand Book written in 2003-2004. Therefore, an

    updated version of the Insurance Hand Book has been prepared to make Siemens employees aware of

    changes on locally as well as Global Property Policies and Public & Product Liability Policy due to wider

    risk coverage, wherein increase in premium rate and deductibles have been enhanced by SFS/VVK.

    Previously only fire risk was covered for Co.s Fixed Asset & stocks etc, now upon shifting it to

    mandatory Global Property Coverage Program of Siemens Germany VVK in the year 2004, a wider

    coverage has been offered for our Fixed Assets, Stocks (own & customer) held in trust or for repair with

    additional coverage of business interruption for which our Fixed Assets and furniture fitting have been

    revalued by professional valuator. Our factory has been surveyed twice by Global Risk Consultant to

    evaluate risk exposure to our Factory and Corporate Offices. Premium rates are fixed upon their risk rating

    Moreover, keeping in view several queries received by us regarding project coverage a write-up has also

    been added for EAR i.e. Erection All Risk to make them understand what an EAR cover is, what it covers

    and not. What can be added in standard coverage when it starts & finishes at what stage a project has

    maximum risk at what stage its minimum and what is its issuance requirements.

    Recently, Travel Health Insurance Coverage has been made a compulsory requirement for obtaining

    German Visa at Islamabad and Visa for many other countries. This now caters for medical assistance

    1. INTRODUCTION

    The prime objective of this Insurance Handbook is to provide readily available knowledge to all for

    maximum and proper utilization of insurance facility and to indicate the Division ofResponsibilities (DOR) of concerned Department/Business Unit regarding insurance claim

    handling.

    We have tried to cover maximum information in this handbook. However, if there is any query now

    or at a later stage regarding insurance/claim handling, please contact Shared Services, Karachi.

    2. CORPORATE FUNCTION FOR RISK ANALYSIS AND RESPONSIBILITIES

    2.1 It is one of the functions of Shared Services to provide protection to all company business andassets against expected and unforeseen risk.

    2.2 In order to meet the above requirement, the Shared Services Deptt. arranges insurance coverage at

    corporate level to safeguard companys interest and limit unforeseen losses in conformity with the

    International Insurance Guidelines of VVK Deptt. Siemens AG, Germany.

    2.3 Shared Services Deptt also negotiates with insurance companies to make sure that proper and most

    cost effective insurance coverage is in its place.

    2.4 For this purpose annual policies are obtained at corporate level and all concerned department/

    business units are informed accordingly.

    2.5 We have provided the details of insurance policies regularly being taken by Shared Services

    Department at Corporate level on yearly basis for various risks with brief description of its purpose

    and to make business units aware of coverage available at corporate level and to make its

    maximum utilization to safeguard our business energies i.e. ROCE.

    3. TYPES OF COVERAGES AVAILABLE

    3.1 Corporate Policies

    3.1.1

    3.1.1.1

    Fire

    All Risk Property Damage

    And Business Interruption

    Policy:

    Insurance program implemented through Siemens in house brokerSFS/VVK. It covers all assets of Siemens Pakistan including ThirdParty goods held in trust/repair, business interruption resulting dueto damage to property w.e.f. 01.10.2004.

    All our fixed assets as per the FC records in the year 2004 wereassessed by professional evaluator M/s. Oceanic Surveyor (Pvt.) Ltd.Record basis of its sum insured is replacement cost. Interior decor

    have been covered on the basis of covered area and stocks, rawmaterial, finished goods & semi finished goods, stocks in process.Gross Profit is based on report of FC and estimated third party goodsto be held in trust/repair provided by respective BUs and leaserentals & rental income by Shared Services.

    These are submitted to Siemens Germany SFS Deptt. in April everyyear for Global arrangement of this coverage. This policy is issuedunder Siemens Global Property through leading insurer Zurich

    Versicherung AG and their Local Fronting Partner M/s. AdamjeeInsurance Co. Ltd., with harmonized rates and with deductibles of10% of the loss amount subject to minimum Euro 50,000 andmaximum Euro 100,000 per occurrence for property damages and10% of the loss amount subject to minimum of Euro 250,000 andmaximum of Euro 2 million per occurrence for Business Interruptionclaim in excess are covered under Global Insurance Program.

    3.1.1.2 Householder Policy: It covers Household effects and building of MD, CFO and ExecutiveDirectors residence as per list provided by them with Electrical

    Clause B and also Household Effects and Building of Siemensowned/leased Beach Huts

    3.1.2 Marine:

    3.1.3.2.1 Stocks It covers all types of theft and burglary risk to our stocks, kept in ourpremises at any place in Pakistan to limited extend.

    3.1.2.2.2 Office Machines and Allied

    Equipment

    It covers office machines and allied equipments includingcanteen/party domestic utility machines kept at different locationsuch as calculators, telephones sets and other portable machines etc.On estimated values being obtained by user deptt. yearly.

    3.2 Policies At Department / Business Unit

    3.2.1 Cash:

    3.2.1.1 Cash-in-safe It covers cash-in-safe kept at different locations of Siemens Officesduly approved by Financial Controller. If cash-in-safe is increased asstated in the policy at any time or otherwise, extra coverage will

    have to be obtained for which immediate written intimation is to begiven to Shared Services Deptt within office time prior to 4 PM.

    3.2.1.2 Cash-in-Transit It covers cash-in-transit from banks to HO Branch Office ProjectSites residence and back to office by Siemens Employees. Cashcarried shall not be exceeded in any case i.e. the single loss limit Rs.5000. Amount more than Rs.500 and more shall be carried by atleast two persons in brief case and in companys hired or leasedvehicles.

    3.2.1.3 Fidelity

    Its blanket policy which covers cash fraud or dishonesty of ourpermanent and contract employees up to Rs.5000,000/- whichcontains cash limit fraud upto Rs. 2500,000&stocks limit up to Rs.2500,000/-

    3.2.2 Contractor, Plant &

    Machinery:

    3.2.2.1 Mobile Crane Policy It covers loss or damages to our Engineering & Construction MobileCrane at Factory and Project sites and to third parties due to itsoperation whilst in transit any where in Pakistan.

    3.2.2.2 Machinery Policy It covers our 135 KVA standby Generator installed at Ilaco HouseOffice compound for power failure and risk arose due to itsinstallation to third party property.

    3.2.2.3 Machinery & Tools Policy This policy covers all risk to our Engineering and construction

    3.2.4 All Risk Policies:

    3.2.4.1 Mobile Phones It cover (all risk) & losses to Siemens owned Mobile Phones,while transit, anywhere in Pakistan such as theft accidentaldamages etc.

    3.2.4.2 Customer Engineering Work

    Station & DiagnosticsEquipment & Notebooks

    It covers all risks to our declared equipment being used by our

    services & sales Engineers for diagnostic faults at customerpremises and project sites while performing maintenance & salespromotion jobs etc.

    3.2.5 Workmen Compensation

    Policy:

    It covers accidental death, disability and compensation to workersnot been able to work on job due to accident or sickness. Itincludes our trainees, apprentices, contract employees working atvarious installation sites and workers employed by our sub-contractors as per existing Workmen Compensation Act.Maximum coverage under this policy is Rs. 200 for death anddisability.

    3.2.6 Group Life Insurance Policy: It covers Siemens Pakistan Employees benefit due to Accidental,Death, Accidental Disability, Natural Disability and TerminalIllness as per employees category(Underwriter M/s. EFU General Insurance Ltd. of Pakistan)

    3.2.7 Travel Health Policy: It provides coverage for emergency accidental medical treatments,urgent admission to hospital and eventual cost related to youremergency evacuation, death repatriation. A part from this direct

    settlement of all hospitalization claims through out the world with(certain deductibles of US# 100 for accident & sickness medicalexpenses in Hospital & OPD plus it also provide coverage to lossof baggage, Passport baggage delay, personal liability, accidentaldeath(24) hours common carrier and permanent total disability.

    It is now mandatory requirement for Germany & for many othercountries visas. Maximum coverage age is 64 years. For multitrippolicy maximum duration of coverage is 60 days per trip.

    4. INSURANCE CLAIMS

    4.1 Procedure For Insurance Claims

    4.1.1 Property Damage And Business Interruption

    4.1.1.1 If any damage what so ever nature or fire breaks out at our any premises then immediatelythe concerned person should inform Shared Services, who will inform the underwriter about

    the incident and lodged a provisional claim.

    4.1.1.2 Lodge report with Police Station of jurisdiction in consultation with Shared Services.

    4.1.1.3 Its survey will be managed at Karachi by Shared Services and for survey at Branch Offices;

    the person incharge will arrange in consultation with the Regional Offices and will obtain asurvey report from he surveyor.

    4.1.1.4 Insurance claim form shall be forwarded to the concerned person by Shared ServicesDepartment.

    4.1.1.5 The person concerned will give all relevant information to Shared Services and will submitclaim form duly filled-in for onward submission to our under writers.

    Shared Services will follow it up with insurer till claim is settled.

    4.1.2 Following Documents Are Required In Case Of Claim:

    4.1.2.1 Claim form duly filled-in , signed and stamped.

    4.1.2.2 Fire Brigade Report in case of Fire.

    4.1.2.3 Police Report (FIR).

    4.1.3 Marine

    4.1.3.1 All claims against Marine policies are to be lodged by the respective BusinessUnits/Department and also to be settled by them at their end.

    4.1.3.2 As soon as the consignment arrives, if its packing apparently seems damaged immediatelysurvey is to be arranged and preferably damaged packages shall be opened in the presence

    of surveyor.

    4.1.3.3 The survey should only be arranged with the authorized surveyor of Government and whichhave been appointed by our under writers a list of which is enclosed herewith theiraddresses.

    4.1.3.4 In the event of losses or damages immediate notice shall be given to our underwriter and

    survey report shall be obtained from the surveyor.

    4.1.4 Procedure In Case Of Damages/Shortages Detected At Port.

    4.1.4.1 Immediate intimation shall be given to shipping company request them to appoint surveyor

    and also to insurance company for arrangement of joint survey/verification of effected itemsat port. This should be done within period of Three Days from date of arrival of ship/Aero

    plane.

    4.1.4.2 In short landing cargo case a short landing certificate shall be obtained from KPT (sea)Airfreight Unit (AIR) Shipper/Airline.

    4.1.4.3 Claim shall be lodged with carrier, under intimation to insurer.

    4.1.4.4 For assessment of loss, inform as and when the consignment reaches the Factory or finaldestination.

    4.1.5.7 Carrier/ship surveyors report in original.

    4.1.5.8 Copy of all correspondence exchanged with carrier/airline.

    4.1.5.9 Our claim bill in triplicate.

    4.1.5.10 K.P.T final short landing certificate / Air freight unit Airline confirmation of short landing

    in original.

    4.1.6 Documents Required In Marine In Land Transit Claim i.e. (Within Cities And

    Pakistan).

    4.1.6.1 Immediate Intimation to Insurance Co. about the occurrence of loss and the place where theeffected/damaged consignment is lying.

    4.1.6.2 Name of person to be contacted with telephone No. and extension thereof.

    4.1.6.3 In case of theft obtain Police FIR from concerned (local jurisdiction) Authority.

    4.1.6.4 Claim to be lodged on carrier/transporter.

    4.1.6.5 Along with surveyor report following documents should be submitted.

    4.1.6.5.1 Original Marine Policy/Insurance Certificate.

    4.1.6.5.2 Copy of Invoice/dispatch note.

    4.1.6.5.3 Our claim bill in triplicate.

    4.1.7 Accident

    4.1.7.1 Vehicles Claims:

    4.1.7.1.1 The driver of the vehicle/person incharge must immediately inform about the incident to thelocal (jurisdiction) Police Station and obtain First Information Report (FIR) which is must,in case of Road accident, theft & snatching cases.

    4.1.7.1.2 Thereafter immediately the information must be given to Shared Services Department.

    4.1.7.1.3 Shared Services upon receiving the accident information will lodge provisional claim to ourunder writer i.e. M/s. EFU.

    4.1.7.1.4 The user of the vehicle/incharge obtain repair estimate from renowned workshop and shall

    forward it to Shared Services.

    4.1.7.1.5 Upon receiving the estimate cost of vehicle repair Shared Services shall evaluate that

    whether repair of vehicle is beneficial to the company or not through comparative costcalculation as per specimen enclosed as per Annexure A & B. If repair cost is less then

    No Claim Bonus value, Shared Services will advise the concerned Deptt to get the vehicle

    repaired in Co.s Account i.e. without lodgment of insurance claim through Shared Services(Transport Deptt) in case where repair cost is more then NCB, immediately survey of the

    car will be arranged by Shared Services Department

    4.1.7.1.6 After survey has been carried approval for getting the repair of the vehicle will be given byShared Services thereafter car will be sent for repair to workshop.

    4.1.7.1.7 It is the responsibility of the vehicle user department/driver to obtain and send all repairersbills & satisfaction note duly signed to Shared Services Department.

    4.1.7.1.8 Shared Services will complete the prescribed vehicle claim form and submit it along with

    4.1.10 Claim Procedure For Burglary And Theft Insurance Policy: Immediately after the

    knowledge of any above incident is received by the concerned person, the matter should bereported to the local police station and FIR to be obtained. The moment information is

    received by the person concerned about the incident the same is to be conveyed to SharedServices Deptt . Shared Services Deptt. will immediately inform the underwriters and lodgeprovisional claim. Thereafter detailed statement of loss or damage with one estimate of the

    market value of each article cost and the amount of damages sustained is submitted to ourinsurer along with claim form. Shared Services Deptt. follows it with the insurance

    company till the payment is realized.

    5. EAR means Erection All Risk insurance, it is Branch of Engineering Insurance, it coverscomprehensive protection against all risks involved in the erection of Machinery, Plant

    and Steel of any kind as well as Third Party claim regarding Property damage or Bodilyinjury arising in relation with execution of an erection project.

    The insured under EAR Policy is normally:

    The manufacturer or supplier of Machinery or plant if he carries out erection work or

    responsible for it.

    The Firm commissioned with erection.

    The purchaser of the Machinery or Plant to be erected.

    5.1 EAR Coverage: An EAR insurance provides wider cover almost against any sudden andunforeseen loss or damage occurring to the property insured on the erection side during the

    period of insurance will be compensated. The insurance company will compensate loss ordamage arising due to following causes:

    5.1.1 Fire lighting, explosion .

    5.1.2 Flood, Inundation.

    5.1.3 Wind Storm of any kind.

    5.1.4 Earthquake.

    5.1.5 Theft, Burglary.

    5.1.6 Faults in Erection.

    5.1.7 Negligence, lack of skill, lack of experience, malicious act.

    5.1.10.1 Loss or damage due to faulty design defective material or casting, and/or BadWorkmanship.

    5.1.10.2 In cases where the manufacturer of the machinery or plant to be erected is at the same time

    commissioned with erection work and named in the policy as insured. It is possible includeabove resultant physical damage in respect of the insured items due to above mentionedcauses (with additional premium) here in any case it does not cover expenses for removing

    (rectifying) the original fault if such fault had been discovered before the loss occurred arestill excluded from cover.

    5.1.10.3 Loss or damage due to will full acts or will full negligence of the insured or itsrepresentatives.

    5.1.10.4 Loss or damage due to War or war like operations, civil commotions of any kind as well as

    acts on the part of strikes and lock out person. Consequential loss of any kind such asclaims from penalties, losses due to delay, lack of performance loss of contract.

    5.1.10.5 Loss or damages due to Nuclear reaction, radiation, radioactive contamination.

    EAR coverage starts directly after the un loading of the items to be insured at Site and

    continues until the erection work and testing operation has been completed. It thereforeincludes loss or damages during storage during the period of erection, test operation or test

    loading. The maximum risk under EAR policy is at the time of its testing andcommissioning of the project and all risks are transferred to owner after obtaining of FAC

    (Final Acceptance Certificate).

    In addition it is possible to include a maintenance period.

    Please note that Theft, Fire, Earthquake risk etc. are not covered during maintenance periodof EAR Policies. Only faults developed during erection period but detected during

    maintenance period are covered provided extended maintenance cover has been taken.

    Premium is normally tailor-made and in order to arrive at a premium rate proportionatewith the risk from an engineering point of view. The insured must supply following for

    obtaining an EAR cover which forms the basis of cover. It is essential for the insurer toobtain full particulars such as:

    5.4.1 Technical specifications of the relevant project, the contract price, the value of the items to

    be erected.

    5.4.2 Information on the experience of the erection firm with similar projects.

    5.4.3 A flow chart of the production process, a layout plan (with a view to third-party liability).

    5.4.4 The geological, hydrological and meteorological data on the erection site.

    5.4.5 The supply and erection time schedule, details on pre-storage facilities.

    5.4.6 Guard services, fire-fighting facilities.

    5.5 Erection All Risk Policies: The claim arising against Erection All Risk / Engineeringpolicies are to be registered and simultaneously settled by concerned Business Unit /

    Engineering Construction Department.

    of the company same function is to be performed by Human Resources deptt under

    intimation to Shared Services Deptt.
