Introduction

This log-book is designed to be used throughout the 5 years of paediatric training, including the DCH year. You should record details your training, procedures done and cases seen.

For most procedures there is space to record 4 examples of each, which at some stage in your training you are likely to be taught and carry out. On at least one of these occasions each procedure should be supervised by a paediatrician or other specialist (surgeon, anaesthetist) who will assess and certify your competence. It is very important that you learn procedures correctly, so you will practice safely. Many adverse events occur because of poorly performed procedures, or the failure to do basic procedures when they are necessary or life-saving. It is your responsibility to be competent in these procedures by the end of your training.

Your log-book will also contain case records. It is not necessary to record all cases you see or manage, but the list of cases is indicative the range of conditions that are seen in PNG and the Pacific. The cases are listed alphabetically in systems or groups, so the order does not represent priorities or the frequency of these conditions. The list is designed to be comprehensive and representative, covering all systems but not all conditions you treat will be included, and their omission does not mean they are low priority. At some stage in your 5 years of paediatric training you are likely to manage at least one of each of these cases. Some of the cases are seen more commonly in outpatients or community clinics than a tertiary paediatric ward, but the list represents the range of acute and chronic paediatric conditions. The record is designed so you can record not only acute treatment, but follow up information on longer term outcomes. Sometimes you will follow patients with chronic conditions for months and years in outpatients, and the outcomes and important milestones can be recorded also.

Most paediatric conditions have implications for nutrition and development, so you should record the effect of the disease on these areas. Many children have more than one condition, and it is essential to recognise these comorbidities, and treat or manage them.

Your log-book should be reviewed regularly by your supervisors, and you should take time to reflect on your progress in learning procedures, the cases you have seen, and the knowledge you are acquiring.

At the end of your DCH year you are expected to have made a start in recording cases you have managed, and examples of basic procedures you have done. The DCH year is designed to be strongly clinically focused, with successful trainees acquiring a good knowledge of common paediatric conditions and how to manage them at a provincial hospital level. You are not expected to have completed many sections of this log-book in your DCH year, but you are expected to have made a good start. As part of your DCH assessment you will be assessed on the start you have made in recording cases and procedures. In the MMed Part II you will be further assessed on the completion of this log-book. Even after 5 years of paediatric training there may be procedures you have not fully completed, or cases you have not seen, but you should make every effort to complete as much as possible, and regularly discuss progress with your supervisors.
