Liver Radioembolization with 90Y Microspheres

This book provides an in-depth account of all aspects of radioembolization, a relatively novel technique based on the efficacy of radiotherapy for the treatment of liver tumors. Radioembolization combines embolization (intravascular deployment of particles – microspheres loaded with yttrium-90) and brachytherapy (local administration of radiotherapy), thereby allowing delivery of high doses of beta-radiation specifically to the tumoral area in an attempt to overcome the problem of permanent radiation-induced damage to nontu-moral liver parenchyma. Performance of the proce-dure demands close familiarity with the hepatic vascular anatomy in order to ensure avoidance of vessels going to other territories. Furthermore, a detailed knowledge of liver function, tumor volume, and liver vascular shunting is needed in order to calculate the appropriate therapeutic dose in each particular case. All of these topics are addressed in depth in this book, and initial prom-ising clinical results are presented.

Functional Anatomy of the Shoulder

The anatomy of the shoulder is based on complex joint biomechanics, which guarantee the coex-istence of both maximum mobility and stability within the same joint. In recent years, diagnostic techniques such as magnetic resonance and arthroscopy have made it possible to study and better interpret those fine anatomical structures which were formerly very difficult to appreciate through “open surgery” dissection techniques that would compromise their integrity. Difficulties of technical nature, which today have been overcome thanks to technology, delayed the use of endoscopy in shoulder treatment thus filling the gap previously existing if compared with other joints surgery (i.e., knee). Shoulder arthroscopy, exploiting anatomical integrity, has contributed with excellent results to the identification of those structures that have been given little descriptive importance in classical texts.

Improving Oral Health for the ElderlyAn Interdisciplinary Approach

Two important oral health policy concerns in the United States are disparities in the oral disease burden and the inability of certain segments of the population to access oral health care. Both of these challenges are largely due to socioeconomic stratification in US society. As a consequence of the release of the surgeon general’s report on oral health, there has been a call to action to improve the oral health of underprivileged groups who have difficulty accessing dental services. But in contrast to the extensive national attention focused on children’s oral health in recent years, America’s growing elderly population has received relatively little attention and almost no public health or public policy interventions. Population projections for the United States indicate that the elderly will constitute an increasing percentage of the popula-tion. In 2001, the population of the United States was almost 278 million, and 12.6% of the popula-tion was 65 years of age or older.

The Aetiology of Deep Venous ThrombosisA Critical, Historical and Epistemological Survey

What we now call ‘deep venous thrombosis’ (DVT) has been elucidated by a diversity of investigative approaches during the past four centuries. The authors of this book survey the history of the field and ask: why has one of these perspectives – the haematological/biochemical – come to dominate research into the causation of DVT during the past 50 years and to exclude alternatives? In answering this question, they show that the current consensus model is conceptually flawed. Building on the work of William Harvey, John Hunter, Rudolf Virchow, Ludwig Aschoff and a number of pathologists in the mid-20th century, they offer a revised account of the aetiology of this condition. In the process they retrace and review the 160-year-old philo-sophical and methodological schism in biomedical research and, using DVT as an example, propose how this schism might be bridged to the benefit of both research and clinical practice.

In the network of cardiologists within the Euro-pean Society of Cardiology Working Group on Cardiac Rehabilitation and Exercise Physiology there is a strong view that the time indeed is right to publish definitively on the methods involved in cardiovascular prevention and rehabilitation. We are facing a transition from conventional cardiac rehabilitation to a phase of combined preventive and rehabilitative efforts, as witnessed in the recent Joint Task Force Guidelines on Preventive Cardi-ology. For this revision of our clinical routines we lack a practical textbook, based upon the condi-tions and resources of European health care. This textbook is designed to fill that gap.

An earlier book by Rodrick Wallace entitled Consciousness: A Mathematical Treatment of the Global Neuronal Workspace Model, introduced a formal information-theoretic approach to indi-vidual consciousness implementing approaches developed previously by the cognitive scientist Bernard Baars and the philosopher Fred Dretske.This book takes a more formal ‚groupoid‘ perspec-tive and generalizes the results of that book to processes of ‚distributed cognition‘ characteristic of large institutions that can entertain several, sometimes many, simultaneous ‚global workspaces‘ which must compete for resources while commu-nicating and cooperating.Equivalence classes of ‚states‘ produce a network of language-analogs characterizing interacting cogni-tive modules which entertain multiple workspaces. Equivalence classes of these language-analogs produce dynamical manifolds describing temporal processes carried out by multiple-workspace institutions.
