January 2009 PAARNG DRUG TESTING SOP CHAPTER 1 1-1. GENERAL: As per TAG Policy Letter dated 03 July 2008, the use of illegal drugs is intolerable and incompatible with military service in the Pennsylvania Army National Guard. CHAPTER 1

CHAPTER 2 RESPONSIBILITIES

2-1. JOINT SUBSTANCE ABUSE PREVENTION OFFICER(JSAPO):

a. Coordinates all activities in the area of substance abuse to include education, prevention, training, rehabilitation, identification, referral, follow-up, and program evaluation for his/her state to include drug urinalysis collection. b. Acts as liaison between NGB-CD-SA (Substance Abuse Section) and the Adjutant General’s Office. c. Is responsible for training of MACOM ADCOs and UPLs on all aspects of the ASAP (Army Substance Abuse Program) to include urinalysis collection. d. Allocates testing quotas and coordinates urinalysis testing requirements for MACOMs and their subordinate units. e. Ensures that all positive results will be processed and forwarded to the unit of the soldier testing positive as reported by the toxicology laboratory and copies furnished to the MACOM. f. Serve as a member of the Alcohol Drug Intervention Council (ADIC).

2-2. JOINT SUBSTANCE ABUSE PREVENTION COORDINATOR (JSAPC):

a. Will manage the Drug Urinalysis Testing Program for the PAARNG. b. Has the responsibility of updating the personnel data base and conduct the random selection of soldiers to submit specimens for drug urinalysis testing. c. Provides a master list of soldiers to be tested, DD Form 2624s, Unit Ledgers, and preprinted bottle labels to the units to be tested. d. Obtains and distributes supplies to the units for conducting the drug urinalysis collection. e. Conducts training of the UPLs to perform the urinalysis collection process at their units. f. Is the POC for communication to the toxicology laboratory and NGB regarding any matters relating to the Drug Urinalysis Testing Program. g. Initiates paperwork of all positive results and track all positive cases. h. Establishes and maintains files on testing conducted and the results.

(1) Positive Results will be kept on file for a minimum of three (3) years. (2) Negative Results will be kept on file for a minimum of one (1) year.

2-3. ADCO-DIVISION, BRIGADE, AND BATTALION.

a. Ensure that all aspects of the urine collection process are performed in compliance with current governing regulations and directives. Urine specimens will be collected using guidelines in this SOP. b. Establish testing sites and dates for subordinate elements. c. Serve as the commander’s primary adviser and point of contact for the State ADCO for all ASAP matters.

2-4. UNIT COMMANDER:

a. Has overall responsibility for the urinalysis testing program including the selection and training of personnel to conduct the urinalysis collection process. See ANNEX (D), Commander’s Checklist. b. Assigns an NCO (E-5) or higher grade as UPL on unit appointment orders. c. Will use the DA 1379 to verify that each soldier identified for testing provide a specimen. d. Ensures testing facilities are available to include an appropriate number of latrines, a work area for the UPL and a holding area for soldiers until the collection process is complete. e. Appoints an NCO (E-5) or higher rank to supervise the holding area for all soldiers until they complete the collection process. f. Orders selected soldiers to take test at unit briefing. See ANNEX (E), Commander’s Briefing. g. The higher headquarters will use computers to randomly select personnel, units, or parts of units to be tested. The higher headquarters will notify the unit to conduct urinalysis testing or will coordinate a team to conduct the testing. Commanders, of course, retain the right to conduct testing for probable cause, reasonable suspicion, and mandatory requirements.

2-5. UNIT PREVENTION LEADER (UPL):

a. Must be an NCO E-5 or higher rank. b. Must be trained by the JSAPC to administer drug urinalysis collection process. c. Serves as the Unit Commander’s primary adviser and point of contact for all ASAP matters to include drug urinalysis testing. d. Trains and briefs the observers. e. Establishes specimen collection point.

(1) The UPL must designate a collection area that can be controlled at all times. (2) The collection area should be quiet and away from the mainstream activities of the unit. (3) Ensure adequate number of tables and chairs for the collection team.

f. Ensures materials are on hand and organizes the collection test site. See ANNEX (B), Drug testing supplies.

g. Prepares materials in advance (e.g. pre-printed labels, etc.) h. Ensures all aspects of the urine collection process are in accordance with this SOP. i. Ensures the DD Form 2624 is complete, correct, and that all specimens are secured and

stored properly until submitted to the JSAPC. j. Packages and mails urine specimens to the JSAPC in accordance with this SOP. k. Advises the commander of any soldiers refusing to provide a specimen, attempting to

contaminate the specimen, or any other irregularities or discrepancies. l. May be an observer. m. Ensures the observers follow guidance in 2-7 below. n. Briefs unit personnel prior to conducting drug testing. See ANNEX (F).

2-6. ASSISTANT UNIT PREVENTION LEADER (AUPL): a. Must be an NCO E-5 or higher rank. b. May be appointed by the Unit Commander to assist if more than one station is needed for urinalysis collection. c. Transfers specimens to UPL or ADCO using appropriate procedures and correct documentation. d. Must be supervised by UPL.

2-7. OBSERVER:

a. Will be an NCO (E-5) or higher rank. b. May be an E-4 if approved by the Counterdrug Office in writing prior to testing. c. Receives a briefing from the UPL or ADCO prior to conducting drug urinalysis testing each day. See ANNEX (G). Responsibilities of Observer During Drug Testing. d. Directly observes soldier voiding from the body into the bottle. e. Ensures the chain of custody is not broken. f. Advises the UPL of any soldier attempting to contaminate a specimen or otherwise trying to avoid proper procedures. g. Assists UPL in verifying/documenting entries on the bottle label, the Unit Urinalysis Ledger, and the Specimen Custody Document-Drug Testing (DD Form 2624). h. Will only observe soldiers of the same gender. i. Will only observe one (1) soldier at a time. j. Will sign the Unit Ledger to verify the Solder complied with the collection process and directly observed the soldier provide the sample and maintained eye contact with the specimen until it was placed in the collection box.

CHAPTER 3 FORM PREPARATION

3-1. FORMS: The DD Form 2624’s, Unit Ledgers, and the bottle labels are preprinted by the

computer at State level. 3-2. CHECKLIST: Follow the checklist to complete the forms and labels. See ANNEX (I) 3-3. MANUALLY COMPLETED: If the forms must be completed manually, follow instructions outlined in ANNEX (O). 3-4. HOW TO MAKE CORRECTIONS:

a. Only the person making the error can make the correction on the DD form 2624 or bottle label. Corrections will be made as follows:

(1) Line (draw a single line) through the faulty information. (2) Do not ever write over any number or letter. (3) Write the correct information directly above the faulty information. (4) Place your initials and today’s date close to the line through.

3-5. CERTIFICATE OF CORRECTION: If correction cannot neatly be made on the DD Form 2624 or the bottle specimen label by the individual who made the error, a certificate of correction, see ANNEX (K), should be used to correct and verify the process.

a. The certificate of correction will be filled out noting the faulty information as it now reads and the correct information as it should read. b. The certificate of correction will be signed and dated by the UPL and verified by the commander or his/her representative. c. The certificate of correction will not be used to make corrections on the Unit Urinalysis Ledger.

CHAPTER 4 URINE SPECIMEN COLLECTION PROCESS

4-1. GENERAL: The following steps are the standardized PAARNG procedures for the chain of custody, collection and submission of urine specimens through the JSAPC. These PAARNG procedures will be taught and followed for all training courses. 4-2. PROCEDURAL STEPS:

a. The unit commander will receive a packet from the JSAPC to include the following: (1) Master List of personnel to be tested (2) DD Form 2624 (3) Unit Ledger (4) Bottle Labels

b. Collection Procedures:

(1) The commander will designate a collection point/holding area away from mainstream activities of the unit.

(2) Ensure an adequate number of tables and chairs are available and provide appropriate drinks to facilitate urination for the soldiers unable to provide a specimen.

(3) Follow the “Forms Checklist” ANNEX (O), items 1 through 4, to prepare forms in advance.

(4) Designate observers in the grade of E-5 or higher who are not on the Master List to be tested.

(5) Brief observers on their responsibilities, See ANNEX (G). (6) Commander briefs unit, See ANNEX (E). (7) Identify personnel to be tested and have them report to the designated holding area. (8) UPL briefs soldiers to be tested, See ANNEX (F). (9) The soldier to provide the urine specimen will remove excess garments such as BDU

jacket, coats or sweat tops. The soldier will then report to the UPL with his/her military ID card in hand.

(10) The UPL will take the soldier’s military ID card. (11) An alternate reliable method for verifying the SSN of the soldier is required, if the

soldier does not have a DD Form 2 in his/her possession. (12) The UPL initiates the Unit Urinalysis Ledger and will fill in blocks #2 and #5. (13) If the soldier is taking or has taken any prescription drugs in the last two weeks or

has had a dental procedure within the past 72 hours, the soldier will record RX and the prescription/medication in the Action Taken/Remarks block (12) of the Unit Urinalysis Ledger.

(14) The UPL initiates the bottle label. The UPL will record the date on the bottle label. (15) The UPL directs the soldier to verify the information on the bottle label, unit ledger,

and DD Form 2624. The soldier will then initial the bottle label. His/her initials are verification that all data is correct.

(16) The UPL will remove a new collection bottle from the box in front of the soldier and replace it with the soldier’s military ID card.

(17) The UPL will remove the cap, verify with the soldier and observer that the bottle is clean and that there is no foreign matter in the bottle, and recap the bottle in full view of the soldier and observer.

(18) The UPL will then affix the label to the specimen bottle in full view of the soldier and observer.

(19) The UPL will give the specimen bottle to the soldier in the presence of the observer. (20) The soldier will ensure the observer has full view of the specimen bottle at all times

until the UPL takes custody of the specimen. At no time will the observer take custody of the urine specimen.

(21) If the soldier or the observer does not maintain eye contact with the bottle at all times. The UPL will terminate the process and direct the observer to discard the specimen and destroy the bottle and the soldier will begin the process again with step 9.

(22) If the soldier is female the optional wide mouth collection cup will be issued to the soldier at this time with the bottle.

(23) The soldier and observer will move to a secure latrine. The bottle will be held by the soldier so that it will be in full view of the observer while moving to the latrine. The observer will keep the collection bottle in sight at all times.

(24) After the soldier and the observer arrive at the secure latrine, the soldier will remove the cap in full view of the observer, retaining possession of the cap or place it face up on a clean surface. The bottle and cap must be in full view of the observer at all times.

(25) If the soldier should drop or otherwise compromise the bottle cap, a cap will be taken from a new bottle.

(26) The soldier and the observer will position themselves to ensure direct observation of the flow of urine from the soldier’s body into the bottle.

(27) The soldier will fill the bottle with at least 45 ml (just over half full) of urine and recap the bottle in full view of the observer.

(28) The following procedure applies to female soldiers who utilize the wide mouth collection cup:

(a) The soldier will use the collection cup, and provide the specimen. The observer will keep the collection cup and the bottle in full view and directly observe the urine leaving the body and entering the cup.

(b) The soldier will then open the specimen bottle, and pour the urine from the cup into the bottle. The soldier will recap the bottle in full view of the observer. The observer will watch this entire procedure. The bottle must contain at least 45 ml (just over half full) of urine.

Note. If less then 45 ml of urine is collected, the entire specimen and the bottle will be destroyed. The soldier will be sent back to the holding area until he/she can provide a full specimen. Procedure will begin at step (9), original entries on the DD Form 2624 and Unit Ledger may be utilized for the second specimen collection. However, a new bottle label must be initiated.

(29) The soldier may wash his/her hands with soap after recapping the specimen as described in (17) & (28) above, but the soldier and observer must keep the specimen in full view.

(30)The soldier and the observer will return to the collection table. The bottle will be carried by the soldier so that it will be in full view of the observer while moving to the collection table. The observer will keep the bottle in sight at all times.

(31) The soldier will hand the bottle containing his/her specimen to the UPL; both the soldier and the observer will continue to keep the bottle in sight at all times until the UPL places the specimen in the collection box.

(32) The UPL will take custody of the specimen bottle, verify that the bottle cap is securely tightened, the bottle does not leak, and the specimen appearance is consistent with a valid specimen collection (e.g. temperature, color, etc.).

(33) If adulteration is suspected, the UPL will secure the specimen and order the soldier to stand fast and, ensure that the commander is notified.

(34) The UPL will then place evidence (tamper resistant) tape across the bottle cap and label. Note: Care must be taken not to cover any of the writing on the label or obscure any portion of the label. See ANNEX (H).

(35) The UPL will initial the specimen bottle label in the appropriate place in full view of the soldier and observer. The UPL’s initials signifies that he/she has received the specimen from the soldier, checked the specimen for adulteration, ensured the cap was secure, and placed tamper evident tape across the cap.

(36) The UPL will take custody of the specimen by returning the bottle to the collection box and at the same time retrieve the soldier’s ID card and return it to him/her.

(37) The observer will then sign the Unit Ledger in full view of the UPL and the soldier to verify the soldier complied with the collection process and directly observed the soldier provide the specimen and maintained eye contact with the specimen until it was placed in the collection box.

(38) The soldier will then sign the Unit Ledger in full view of the observer and the UPL, verifying that he/she provided the urine in the specimen bottle and that he/she observed the specimen being sealed with tamper evident tape and placed into the collection box.

(39) The soldier is then released from the holding area. (40) The process will be repeated until all specimens have been collected. (41) After the all specimens have been collected, the UPL will verify that all dates, and

SSNs match on the DD Form 2624, Unit Ledger and bottle label. (42) The UPL will ensure the DD Form 2624 and the bottle labels are filled out correctly. (43) The UPL will complete the backside of the DD Form 2624 detailing custody of the

specimens for the time he/she receives the specimens until they are placed in U.S. Mail. See ANNEX (L).

(44) If unable to transport to the JSAPC or mail specimens immediately, the specimens, and DD Form 2624 will be placed into temporary storage, as described in paragraph 5-1 (Storage)

(45) The UPL will prepare the specimens for shipping and mailing. See CHAPTER 5. 4-3. VERIFICATION:

a. General: After completion of the drug urinalysis collection, and prior to the packaging of the specimens for shipping, the commander, ADCO, or UPL will review the DD Form 2624 and specimen bottles to ensure all blocks are completed as per this SOP, all dates and SSNs match, all signatures and initials are completed and that all specimens are sealed with evidence (tamper resistant) tape.

b. The commander or his/her representative will sign the Unit Drug Testing Report (ANNEX N), verifying that all information has been reviewed and is correct.

c. The commander or his/her representative will ensure a copy of each Unit Ledger, DA Form 1379 and the Unit Drug Testing Report, ANNEX (N), will be mailed to the JSAPC as soon as possible following drill weekend.

d. The UPL will ensure that only original DD Form 2624s are sent to the JSAPC, along with the certificates of correction (if needed) and letter requesting the fifth drug to be tested (if commander desires). See ANNEX (M).

e. The UPL will ensure that a copy of the Unit Urinalysis Ledger is not sent to the laboratory. The Surgeon General’s SOP directs the Toxicology Laboratory to destroy any specimens that can be identified by a name.

4-4. FAILURE TO PRODUCE A SPECIMEN OR PROVIDE A COMPLETE SPECIMEN:

a. General: Effective 1 October 1993, The Surgeon General’s Drug Testing SOP requires the toxicology laboratory to test samples that contain a minimum of 45ml of urine, which is approximately just over half a bottle. Also, before directing the soldier to the latrine, he/she should be asked if they are prepared to give at least 45 ml of urine. This should eliminate the majority of problems in producing enough specimen.

b. If the soldier is unable to produce a specimen after entering the latrine, he/she will notify the observer. The soldier and the observer will return to the collection table and give the bottle to the UPL who will return the bottle to the box with the soldier’s ID card. The soldier will be placed in the holding area until he/she is able to provide a specimen. When the soldier is able to provide a specimen he/she will verify the bottle is clean and empty, and the SSN and date on the bottle label are correct. Continue process with step 4-2, (19).

c. If the soldier is unable to produce a full specimen (45 ml) after a reasonable time the soldier will notify the observer. The observer will direct the soldier to pour the urine from the specimen bottle down the urinal and recap the specimen bottle. The bottle will then be destroyed.

d. The soldier will then return to the holding area until he/she can provide a specimen. When the soldier can provide a specimen he/she will continue with step 4-2, (14).

e. The holding area will be supervised by an NCO or higher grade directed by the commander. After a reasonable time, the unit commander will be advised of the status of any soldier still in the holding area. The commander will take actions as necessary to ensure the process is completed. A soldier will not be released from duty until a specimen has been properly provided or as directed by the command authority. 4-5. REFUSAL TO PROVIDE A SPECIMEN:

a. If the soldier refuses to provide a specimen, the appropriate command authority will be notified. The soldier’s chain of command should give the soldier a direct order to provide a specimen in accordance with AR (NGR) 600-85. If the soldier refuses, it will be a violation of a direct order. Violation of a lawful order is subject to disciplinary action under the appropriate State and Federal codes. Possible actions include court martial proceedings and process for separation.

CHAPTER 5 STORAGE, PACKAGING, & MAILING

5-1. STORAGE:

a. Ensure that each specimen collected is forwarded expeditiously for testing. b. Specimens will not be placed in safe storage unless absolutely necessary. Arrangements

can be made with the U.S. Postal Service to accept the specimens after duty hours. A box of twelve (12) specimens will fit into most drop boxes. Make sure you mail the samples in compliance with U.S. Postal regulations.

c. If storage is required, block 12, on the back of the DD Form 2624 will be completed as shown, see ANNEX (L). Proper annotation must include building, room and container number as appropriate.

d. The safe storage area will be a restricted access area that requires an access roster. e. The access roster will show entries placing the specimens into storage and their removal.

5-2. PACKAGING & MAILING REQUIREMENTS:

a. Verify SSNs, dates, and initials on the bottle labels. b. Number top of bottle caps with specimen number found on DD Form 2624 block 7 in

parenthesis. c. Line through SSNs with no specimen, blacken out ¼ inch of bar code, mark “NO

SPECIMEN” over the bar code, initial and date in block 11 see ANNEX (J). d. Consolidate batches when possible. More than one DD Form 2624 and its specimens

may be shipped in a box providing the entire batch can be shipped together in that one box. e. Make two (2) copies of each DD Form 2624, Certificate of Correction (if used) and

Special Test Request (if needed) and distribute as follows: (1) Original in #10 envelope (See i. below). (2) 1 copy to Battalion ADCO/UPL (if appointed). (3) 1 copy to unit UPL

h. Make two (2) copies of Drug Urinalysis Unit Ledger and DA Form 1379, and distribute as follows.

(1) Original to State Drug Testing Site (2) 1 copy to unit UPL (3) 1 copy to Battalion ADCO/UPL (if appointed).

i. Prepare a #10 business envelope as follows for each box of specimens: (1) Annotate “Your State Base Area Code” on the front of the envelope in large print as illustrated below:

j. Seal envelope after enclosing: (1) Original (blue border) DD Form 2624. (2) Original Drug Urinalysis Unit Ledger (3) Original(s) of Certificate of Correction (if used). (4) Original of Special Test Request (if used).

k. Prepare an individual box of up to 12 specimen bottles as follows: (1) Place at least two (2) absorbent pouches in the box. (2) Close box (short extensions first). (3) Seal with “Packing Tape” around the seam of the box length-wise once and around the seams on each end of the box as illustrated in Figure (5-1) below. (4) The person who released the Chain of Custody to the U.S. Postal Service (DD Form 2624), must sign his/her name and date across the “Packing Tape” both top and bottom.

l. Place the prepared #10 business envelope (from step j. above) on top of the box. m. Secure the envelope with the tape around the box length-wise once and width-wise once (in the center each time) as illustrated in Figure (5-2).

n. Place sealed box in a water-proof mailing pouch (6530-01-304-9762) and seal the pouch. o. You may then mail the package to the JSAPC, by applying the “ship to” address and the “return” address, or you may insert the mailing pouch into a “U.S. Postal Priority Mail Pouch” (available through the U.S. Postal Service or the Mail Room at FTIG) and then mail the package. p. Be sure to follow all U.S. Postal Service regulations. q. All specimens will be mailed through the U.S. Postal Service, first class mail. Do not send specimens by certified mail. On rare occasions, when you conduct a urinalysis test for probable cause or reasonable suspicion, and a quick drug testing result is needed, the specimens may be shipped through the U.S. Postal Service Overnight Mail. The actual method of transport will be correctly annotated in block 12, d. of the DD Form 2624. r. Commander’s Briefing

• Today our unit will be conducting a drug urinalysis test for substance abuse.

• There is no probable cause or reasonable suspicion that anyone in the unit is using or abusing drugs or controlled substance.

• The personnel to be tested today were selected at random by computer.

• All samples collected will be tested for five drugs, Marijuana (THC), Cocaine, Opiates, Amphetamines, and a fifth drug tested on a rotational basis which includes LSD, PCP and Barbiturates.

• Testing procedures outlined in the PAARNG’s Drug Testing SOP will be followed. • All soldiers must be aware that all verbal orders connected with the testing are lawful and are

to be followed as such. • I am ordering everyone who has been randomly selected for this test to submit a urine

specimen IAW AR 600-85. • A refusal to comply with orders relating to this test subjects the soldier to punitive or

administrative action under AR 600-85, AR 135-18, AR 135-135, AR 135-178, AR 635-10, NGR 600-200, NGR 600-5 and the appropriate State Military Code.

Unit Briefing by UPL

• You have three major responsibilities during this drug test: 1. Provide a specimen 2. Ensure chain of custody is maintained at all times 3. Keep specimen bottle in full sight at all times

• You will be provided a clear plastic bottle, or for the females a collection cup, to submit a urine specimen. • Each bottle will have a label affixed to it with today’s date that identifies you by your SSN. NOTE: Do not fill a bottle that does not have a label affixed to it with the correct date and SSN. • Collection of the specimen will be conducted using direct observation in full view of an observer. Do not go to the control table until you feel you are ready to provide at least 45 ml of urine (approximately half of the bottle). If you are unable to provide a specimen or an adequate quantity of urine, you will be held in a holding area, until you are able to provide the quantity needed. You will be provided an adequate amount of liquid to help facilitate the urinalysis process. You will not be released from duty today until you have provided a proper specimen. • Your tasks include:

1. You will provide your military ID card. If you do not have your military ID card, you may use a driver’s license or other picture ID. If you do not have a picture ID, the 1SG or designee will verify your SSN from the DA Form 1379. You will verify that there are no errors on your ID. 2. Verify your SSN, full name, and date are correct on the DD Form 2624, the Unit Urinalysis Ledger, and the bottle label. 3. Indicate on the Unit Urinalysis Ledger in the REMARKS column (column 12) any prescription medication taken within the last two weeks and/or if you have had a dental procedure within the last 72 hours. If none, indicate none. 4. Document that all information is correct by signing your payroll signature on the Unit Urinalysis Ledger and initialing the specimen bottle label.

MEMORANDUM FOR RECORD FOR THE OBSERVERS SUBJECT: Responsibilities of Observers During Drug Testing General: 1. Observers are a critical link in the process of collecting urine specimens to be tested for substance abuse. Instances have occurred in the past where observers did not follow proper collection procedures and positive drug tests were not usable in legal and/or administrative actions. In order to prevent similar occurrences in the future, the observer will read and sign this Memorandum for Record. 2. The testing procedures do not violate a soldier's Fourth or Fifth Amendment rights, nor does the observation procedure violate the right of privacy. A refusal to produce a specimen is a violation of a direct order and may result in the soldier being processed for separation. 3. The results of tests may be used in legal proceedings and consequently the urine sample may be considered as evidence. A valid chain of custody is mandatory for a successful prosecution. As an observer, you may be asked to provide testimony at legal or administrative proceedings. You may be subject to UCMJ or administrative action if it is discovered that the specimen was altered in any way while it was under your control. Criteria for Observers: 1. Be in the rank of NCO (sergeant/E-5) or above. Unless otherwise approved by the State Drug Testing office. 2. Be of the same gender as the soldier being tested. 3. Possess sufficient maturity and integrity to preserve the dignity of the soldier being observed. 4. Not be currently enrolled within the ASAP Rehabilitation Program or currently under investigation for any substance abuse related offenses. Responsibilities: As outlined in AR 600-85, an observer must follow protocol during urinalysis collection procedures. Once assigned to a specific soldier, the observer 1. Controls the urine collection process at all times. 2. Maintains visual contact with the bottle at all times. 3. Ensures that the specimen provided is not contaminated or altered. 4. Directly observes the soldier (one soldier at a time per observer) voiding urine into the specimen bottle. 5. Supervises the soldier tightly capping the bottle. 6. Ensures the bottle is not reopened after the cap is tightened. 7. Escorts soldier back to the processing station/table with bottle in full view. 8. Observes the UPL placing tamper resistant tape over the top of the bottle, and down across the label, not to cover any printed information. 9. Will sign the Unit Ledger in full view of the UPL and the soldier to verify he/she complied with the collection process and directly observed the soldier provide the specimen and maintained eye contact with the specimen until it was placed in the collection box. Manually Completed Forms

1. Specimen Custody Document-Drug Testing DD Form 2624: A. GENERAL:

(1). One of the most important aspects of collecting urine specimens is maintaining the Specimen Custody Document - Drug Testing (DD Form 2624). It is imperative that this form be completed with extreme care and accuracy. (2). Attention to detail and the ability to write clearly are imperative in preparing the specimen bottle label, DD Form 2624 and the Unit Urinalysis Ledger. Care must be taken when copying numbers to ensure consistency. The most common errors made on the chain of custody documents that result in the sample being rejected for testing are: non-matching social security numbers, incomplete social security numbers, and improperly making corrections. Do not slash zeros. If some zeros are slashed and others are not, it may be taken as an error and/or an improper correction. Every effort should be taken to exclude those individuals who write poorly from the collection process. Data on the DD Form 2624 and label may be typed. Rubber stamps may be procured to stamp appropriate spaces in Block 12 of the DD Form 2624. (i.e. , placed in temporary storage, released to US Mail, etc.)

B. PROCEDURAL STEPS: The following steps will be followed when filling out the DD Form 2624:

(1). Block 1. Unit submitting specimens for testing and address to include street, city, state, and zip code. Block 1, unit submitting specimens, should correspond with Block 4, Unit Identification Code. (2). Block 2. State Drug Testing Coordinator, and address to include street, city, state, and zip code. (Where positive results are sent by the toxicology laboratory.) (3). Block 3. Base/Area Code (NG40) (4). Block 4. Unit Identification Code (UIC). Every PAARNG Unit has a separate six (6) digit UIC code. (5). Block 5. Document/Batch Numbers are assigned locally. Each DD Form 2624 will be assigned a batch number. The first document batch number will be 01, the second 02, and the remaining numbered sequentially. You may use 01 thru 99. X out the 1st and 2nd blocks. Enter Document/Batch number in the last two blocks. You will use only one (1) Document/Batch number per DD Form2624. (6). Block 6. Date specimen collected (year, month, day). (e.g., Collection date 1 November 1993, 1993 11 01.) (7). Block 7. Specimen number - Leave Blank. The pre-printed number on the DD Form 2624 will be used as the specimen number. (8). Block 8. (1). The Unit Urinalysis Ledger (CD-ADAPCP Form 1) is the only unit record maintained of urinalysis testing. This form will be secured and retained by the unit. The soldier and observer will sign the Unit Urinalysis Ledger. The UPL will initial the Unit Urinalysis Ledger in the appropriate block verifying the date is correct.

B. PROCEDURAL STEPS:

The following steps will be followed in filling out the Unit Urinalysis Ledger.

(1). The document/batch number, specimen number, SSN, unit identification code, date specimen collected, and test basis recorded on the Unit Urinalysis Ledger and the DD Form 2624 match. (e.g., Blocks 4, 5, 6, 7, 8 and 9 in the Unit Urinalysis Ledger and DD Form 2624 match.) (2). Block 1. Submitting Unit: Unit designation and address to include Street, City, State, and Zip code. (3). Block 2. UADC: Printed name (first, middle initial, last) of the UPL and initials.
