Chef, author, and host of Travel Channel programs, Anthony Bourdain said that in the late 1980s, he was a recovering drug addict and had wrecked the first half of his career and any professional reputation that he had.

He got a job as a lunch cook, the same job he had at the beginning of his career. In his book Kitchen Confidential, Bourdain calls the boss Bigfoot whose orders went like this: "If you're going to work for me, the most important thing is that you show up on time." To him, that meant 15 minutes before your shift started.

Bourdain said showing up, on time, every day not only disciplined him but respected every other person in the organization. It's a rule he has lived with.

Now a top chef again, Bourdain is his own Bigfoot, pounding into every new employee that getting there, on time, every day is the first thing they have to do. The skills necessary to do the job can be taught to people who show up.

For his TV shows, he says it's unthinkable that anyone will be late.

Bourdain says his behavior sets a tone. Because he's there on time every day, his people show up on time too.

Douglas Hoey, RPh, MBA, issued the following statement in response to the annual Gallup survey regarding professional honesty and ethics:"Congratulations to America's pharmacists for setting a record high in trustworthiness in Gallup's annual survey for 2012. An impressive 75 percent of respondents categorized pharmacists very high/high by the honesty and ethics standard. Since the audience is the American people, this ranking is a testimony to the good will generated by the patient-friendly approach of pharmacists who are accessible, responsive and take tremendous pride helping patients achieve the best health outcomes at the lowest costs.

SILVER SPRING, Md. — The Food and Drug Administration has approved a new use for a cancer drug made by Johnson & Johnson, the agency said Monday.The FDA announced the approval of Zytiga (abiraterone acetate), made by J&J subsidiary Janssen Biotech, for men with castration-resistant prostate cancer that has spread to other parts of the body, for use prior to receiving chemotherapy. The drug was already approved for castration-resistant prostate cancer in men who had already undergone chemotherapy treatment.In castration-resistant prostate cancer, the cancer cells continue to grow even when men have undergone drug treatment or surgery to block testosterone, which stimulates the growth."Today's approval demonstrates the benefit of further evaluation a drug in an earlier disease setting and provides patients and healthcare providers the option of using Zytiga earlier in the course of treatment," FDA Office of Oncology Drug Products director Richard Pazdur said.

But that may not happen if pharmaceuticals are seen first and foremost as a ready source of near-term revenues.

One particularly troublesome idea, included in the President’s proposals, calls for extend-ing Medicaid price controls on medicines into Medicare Part D. The Congressional Budget Office estimates that such a proposal would cut revenues for research-based pharmaceuti-cal companies by well over $100 billion over the next 10 years.

Keep in mind that it takes well over $1 billion for companies like ours to bring a new medicine to patients. In a March 2011 report, the CBO cites a key “disadvantage” of extending Medicaid rebates to Part D would be to “reduce the amount of funds that manufacturers invest in research and develop-ment of new products.” The potential cost is real, as R&D cutbacks delay or derail the next breakthrough treatment for diabetes or Alzheimer’s.

And it would impose costs on the economy, as well. A 2011 analysis by Battelle Memorial Institute estimated that such a $10 billion to $20 billion per year reduction in pharmaceu-tical industry revenue would result in 130,000 to 260,000 lost jobs.

Ironically, Medicare Part D happens to be the rare example of a government program that has cost far less than anticipated. According to the CBO, it is coming in 43 percent – or $435 billion – below initial projections for its first seven years. Part D shows how the power of competition can improve government health care programs – but that’s a subject for another day.

Policymakers face a daunting task in closing the yawning gap between revenues and ex-penditures in the coming years, but targeting medical innovation is a particularly counter-productive approach. The trends are clear: Starving medical innovation to hit short-term fiscal targets will only make the next round of budget talks that much harder.source: www.forbes.com

Beyond The Fiscal Cliff, Pharmaceutical Innovation Is The Key To Long-Term Fiscal HealthWhen you look beyond the current “fiscal cliff ” brinksmanship, the long-term budget deficit will depend in large part on two key trendlines: health care costs and economic growth. I’d like to explain how pharmaceuti-cal innovation can be a key positive factor in both areas – if it isn’t choked off by short-sighted efforts to close the budget gap.

The first trendline: health care costs.

Back in 2009, then-budget director Peter Orszag wrote, “Over the long run, the deficit impact of every other fiscal policy variable is swamped by the impact of health-care costs.” That hasn’t changed. One big reason: 10,000 Americans will reach retirement age every day for the next 19 years. Medicare is the fastest-growing major entitlement, growing 68 percent since 2002, according to the Heri-tage Foundation. And these folks, as a whole, can be expected to live longer than those who started receiving benefits when Medicare was enacted in the 1960s.

So, while pharmaceuticals account for only 10 percent of health care spending, medicines are inevitably caught up in efforts to tame its growth. In countries around the world – in-cluding the U.S. – our biggest customer is the government, operating a health care system faced with relentlessly rising costs of caring for an aging population.

Yet those medicines often represent the most cost-effective approach to preventing and treating disease. That’s the true value of phar-maceutical innovation.

To cite just one example, Columbia University economist Frank Lichtenberg has estimated that every dollar Medicare spends on new medicines saves six dollars in other health care costs, on things like hospitalizations and physicians’ services.

This finding was borne out by a 2011 study of the Medicare Part D prescription drug pro-gram, published in the Journal of the Ameri-can Medical Association. The study found that older Americans who previously lacked comprehensive drug coverage saved about $1,200 in medical costs the year they signed up for Part D. If we apply that to the 11 mil-lion seniors who have gained comprehensive coverage through Part D, total savings exceed $13 billion.

And new medicines ultimately yield a legacy of cost-effective generics, which today ac-count for 80 percent of U.S. prescriptions and are actually less expensive here than in other countries around the world.

But we need continued innovation to ad-dress serious medical needs unmet by current medicines or by any other medical interven-tion. For example, a recent study found that new treatments that could delay the onset of Alzheimer’s disease for five years would save U.S. government health care programs $140 billion annually by 2030.

As Washington grapples with the difficult trade-offs of entitlement reform, innovation is the best hope of making those choices less painful – of providing better health care with-out busting the budget.

The second trendline: economic growth.

As President Obama has noted, “The single most important thing we can do to reduce our debt and deficits is to grow.” Here’s an area where our industry would appreciate more attention.

Innovative pharmaceuticals are a U.S. eco-nomic strength, a 21st century industry where the U.S. has gained world leadership in the past 30 years. The U.S. share of new medicines during the decade 2001-2010 was 57 per-cent, compared with 33 percent for France, Germany, Switzerland, and the UK combined – reversing the relative positions in the 1970s.

The biopharmaceuticals sector accounts for about 650,000 jobs nationwide, and contrib-utes around $917 billion to the economy each year, according to a 2011 report by Battelle. And if we include clinical testing centers, supply chain managers, and other partners, our industry supports 4 million jobs across the nation. The value of U.S. biopharmaceuti-cal exports totaled $232 billion between 2005 and 2010 and grew 61 percent over six years. It’s hard to find another U.S. industry that can match that record.

In sum, innovative pharmaceuticals can con-tribute to both sides of the long-term fiscal ledger by helping hold down health care costs and driving economic growth.

It's not easy to find new connections and friends to add to your inner circle, people who will support you, help you, and encourage and motivate you.

Writing for Inc., business expert Jeff Haden says it's not difficult to spot the people who are holding you back. If you have any of these in your inner circle, remove them:

Egocentric advocates: They rarely advocate for anything other than their own ego and points of view. Their "perspective" is often just bragging in disguise.

Keep the people who ask smart questions, share lessons learned and ways to do things differently.

Your big fans: It's nice to tell someone that he or she is great. It's much, much harder, and it takes a real friend, to tell someone he or she can do better.

Be nice to raving fans, but don't pay much attention to what they say.

Inside Scoopers: It's hard to resist hearing the reasons behind someone's decisions, but the person who gives you the inside scoop on other people is also giving other people the inside scoop on you. Those you want in your inner circle are willing to share the inside scoop only on their own thoughts.

Roadblock predictors: They believe they have the ability to foresee a long list of potential barriers and problems that in reality will not appear. Someone who counters every idea with a list of reasons it won't work has to go.

Schmoozers: Networking isn't a numbers game. Connections are just a beginning. Keep the schmoozer out of your inner circle.

Needy inner circle members: They want regular check-ins and contacts so they can feel reassured that you still "care." The people you want in your inner circle are there when you really need them, just like you are for them.

The dead brained: They just drift like zombies and can kill your motivation and enthusiasm.

Surround yourself with people who have ambitious plans, purposes, and big goals. Even if their goals are different from yours, you'll feed off their energy, and they'll feed off yours.

More States are Collecting Sales Taxes on Internet PurchasesThey are coming for your money. And you knew they would.

You have probably noticed that some online vendors charge state sales tax, but many do not. That is going to change as cities and states eye big new piles of potential revenue from the Internet.

Politicians at state and federal levels have been trying to overcome objections and put in place a structure to collect taxes online.

Key to the effort is Amazon.com's new support of a national sales tax, according to Kiplinger's personal finance.

Until now, the question has been one of how to collect the taxes. Retailers would have to somehow collect and remit taxes to thousands of juristictions, each with its own rates and rules.

Now with big Internet retailers behind the tax, the playing field is changng.

At the moment, retailers have to be located in a state to be required to collect taxes. But the states, who want to collect some $23 billion a year in Internet sales taxes, are broadening the rules.

In late 2012, 18 states passed "Amazon" laws, according to tax tracker CCH. Such laws might require retailers to collect taxes if they derive revenue from online links to an in-state business, or if they do business with a local distributor or warehouse. Ten other states have such legislation pending.

Until a federal law is passed, and that could be very soon, Amazon has made its own agreements with six states with six more to come in the next three years.

NCPA Meets With FDA on Compounding FDA Concerned About Noncompliant Isotretinoin

Dispensing

Oversight of the 10-year-old iPLEDGE Program Noncompliance Action Policy, established to eliminate the dispensing of isotretinoin to female patients who are or may become pregnant, is being tightened. Pharmacies suspected of not obtain-ing a Risk Management Authorization, signifying that the patient has met all program requirements and is qualified to receive isotretinoin, will be investigated and potentially face suspension from the program. NCPA and representatives of about a dozen other pharmacy groups met last week with the Food and Drug Adminis-tration to discuss compounding issues in light of the New England Compounding Center meningitis tragedy. Each group was asked to give its perspective on the current compounding situation and react to the FDA’s risk-based proposal to fur-ther regulate pharmacy compounding. Under the proposal, FDA would define traditional pharmacy compounding as what most community pharmacies do, which would continue to be regulated by the states. Non-traditional pharmacy compounding, such as large scale com-pounding, would be regulated by the FDA.

The general consensus among the phar-macy groups seemed to be that the pro-posal could create more, rather than less, confusion. However, there was agreement that for the most part the FDA should fo-cus on those entities that are acting more like manufacturers rather than traditional compounders. FDA seemed to place significant empha-sis on how to assure the quality of sterile compounding, wherever such com-pounding was being done. The agency is scheduled to meet this week with state regulators responsible for the oversight of compounding. Congress is expected to consider compounding legislation next year.

Once again the new year and the inevitable resolutions are upon us. Lose weight. Exercise more. Be more responsive to the family.

Those are the typical goals that people make and break in January. Sometimes it seems that the only new years resolution that works is resolving not to make one.

Still, these are good commitments. If you want a better chance of making them stick, you might heed the advice of Roy F. Baumeister and John Tierny, authors of “Willpower: Rediscovering the Greatest Human Strength.”

First, remember that every exercise in mental self-control takes a physical toll.

The authors give an example of an experiment in which hungry students were immersed in the yummy smells of baking chocolate cookies. Some of the lucky hungry kids were put in rooms by themselves and told to eat the warm cookies if they wanted to. Other unlucky students were left alone in a room and told they could not eat the cookies but they could eat radishes. The radish eaters used their willpower to not eat the cookies. Later, the radish eaters, having exhausted themselves by not eating cookies, were much less able to spend time on other tasks.

Willpower, the author says, is diminished with use. Resisting temptation costs willpower and so does controlling strong emotions or focusing on a mental task. So if you are tired, overworked and overwrought, you are in danger of breaking resolutions because you don’t have any willpower left to keep them.

Luckily, willpower is also replenished by rest. So if you want better family relationships, come home early or give yourself time to rest after work. If you want to exercise more, make sure you aren’t mentally exhausted before you get on the treadmill.

You can help yourself by doing certain things too. If you are on a diet, you can plan ahead as to what you will do when confronted with a buffet (I’ll eat the grilled chicken but not the cake). You can promise yourself cake later (I’ll have a piece but not today).

Winter Skincare Tips, From Head to ToeIt’s everyone’s dream in the dead of winter: to have dewy skin that’s immune to the effects of icy temps, whipping winds, and Sahara-like heating. Good luck with that, right?

“The air is frigid and dry outside, and any kind of indoor heat leaves it even more parched. Your skin’s protective barrier cracks, making it less able to repair itself,” says San Francisco dermatologist Katie Rodan, MD. “It becomes a vicious cycle unless you do some-thing to prevent it—or treat it fast.”

Here, just in time: a guide to protecting your most moisture-starved parts so you can stay soft and smooth all season long.

Try These:

Handle with careTo prevent chapping parched hands, embrace wipe-off, soap-free cleansers and alcohol-free hand sanitizers.

“They’re less drying than washing repeatedly with soap and water,” Dr. Rodan says. “It’s the wet-dry, wet-dry that really sucks the mois-ture out of your skin.”

When you do wash, choose a moisturizing soap-free cleanser or a hydrating antibacterial gel, then slather on lotion right away

Glove upAnything that creates an occlusive barrier (i.e., traps moisture) on skin helps lotions and creams soak in. So slip on some cotton gloves over lotion to help moisturize parched hands.

It seems that every few years a new set of trends assumes prominence over the retail consumer healthcare market—as in other markets—and the nature of the business changes to adapt to emerging consumer preferences. The first decade of this century is no exception, particularly given the challenging financial conditions of the last three years. Following are three such trends of special significance to independent retail pharmacies.

1. Disease state management. The current and projected rise in instances of conditions such as diabetes and hypertension, and all of their ensuing complications, opens an opportunity for community pharmacies to partner with parents to manage their health for optimal outcomes, including the recommendation of OTC medications. The 2010 NCPA Digest points out that community pharmacies have been leading the nation’s other healthcare resources in providing disease state management for many years. The tremendous strain that chronic conditions are expected to exert on the healthcare system as well as an aging population means that independent pharmacies will have many opportunities to have a positive impact on individual consumers and healthcare delivery in general.

2. “Do it yourself” healthcare. As already mentioned, more and more consumers are seeking information and resources to proactively manage their own health. Two primary motivators are driving the trend: first, a dramatic increase in the amount of reliable health-related information easily accessible to consumers via the Internet. Consumers are no longer strictly limited by what their healthcare professionals tell them, and this encourages a curiosity to delve deeper into the nature of their symptoms and conditions. This freedom can be a double-edged sword, because plenty of readily available information is at best incomplete and at worst dangerously inaccurate. Pharmacists are positioned to help guide consumers to trustworthy sources of health information and to answer the questions that naturally arise. Second, cost consciousness clearly motivates many consumers to seek OTC solutions to avoid the cost and inconvenience of a visit to a physician. Because of the variety and quantity of available OTC medications, consumers rely on the pharmacist for recommendations and advice. Independent community pharmacists talk with consumers about non-prescription items an average of 11.6 Times per day—outstanding opportunities to discuss non-prescription remedies and build customer loyalty.5Pharmacists can also advise when parents should not rely upon self-care and should seek a physician’s advice.

3. Web presence. An increasing number of independent pharmacies are actively researching or launching websites that provide their customers with around-the-clock access to the healthcare solutions they offer. LAWRENCEVILLE, Ga. - By Tacoma Perry, FOX 5 reporter A Lawrenceville mother turned her son’s stressful medical condition into a successful business. Using her own recipe, she cooked up a cure for her son’s skin symptoms.

Anita Redd’s middle son is now a teen, but she says she’ll never forget the day he was born.

“He was born in my arms and his skin was flaying off of his lower body,” said Redd.

Doctors told her it was an incurable genetic condition, eczema. It’s is a skin condition that causes symptoms like flaking, redness, itching, dryness and in some extreme cases - bleeding.

Redd says doctors tried everything from steroids to cream. She worried about the effects it would have on her son and thought there had to be a better way to treat his condition.

She did her research and looked into natural skin conditioners. She came up with olive oil and beeswax.

“My very first container that I made for him was in his dad’s roll-on deodorant, Mennen stick. I melted beeswax and added olive oil until it got to be just the right consistency,” said Redd.

It worked and Anita’s Balm was born. But Redd says she didn’t consider it a business until she heard similar skin stories from others in her parents’ group. She’s now making it for others and delivering it.
