MMU has reversed into unoccupied MMU parked to the rear, clipping driver's side of vehicle.
Vehicle Collision Low speed ­ Prime Mover operated by contractor clipped wheel on trailer damaging the rim.
An MPU vehicle suffered a disc brake failure on the 2nd steer axle driver's side wheel resulting in a small fire.
Light vehicle lost braking and travelled approximately 250m down a ramp before going into the high wall windrow to stop.
Smoke was observed coming from a wheel on the dolly of vehicle transporting Ammonium Nitrate Prill on a road train.
Transport vehicle left the road but remained upright. Vehicle was jack-knifed when it came to rest.

Blasting explosives Isolation of scene and inspection of vehicles to ensure no possibility of ignition. Damage to indicator assembly and door visor.

Operator has connected the tanker to a reload pump but has not opened the delivery valve causing a burst disk to rupture.
During manufacturing a burst disc ruptured due to pressure build up, releasing approximately 1 Skg product onto the floor.

Smoke noticed on mobile manufacturing unit due to brake overheating. No flame present.
Blasting Explosives Emergency response team responded and doused area with water. No injuries. Faulty brakes replaced.

Defective moulding found on surface connector of a batch of detonators
Detonators Contact made with manufacturer who issued a product safety bulletin
Vehicle incident (2 incidents) 100kg of ammonium nitrate emulsion spilt during transfer from trailer storage tank to mobile manufacturing unit.
Ammonium Nitrate Emulsion
Scene secured and reported. Emulsion product to be mixed with gravel and used at the next shot.
Prime mover suffered major engine failure.
Ammonium Nitrate Fuel Oil
Load secured and was not affected. Mechanic dispatched to location. Additional prime mover sent to recover loaded trailers.

Two packages of fireworks seized from purchaser. Did not have authority to possess.

Foreign metal object has lodged in static mixer of emulsion pump, reducing emulsion pumping capacity.

Investigation ongoing. Object believed to be part of tank breather.
Burst disc ruptured when operator started loading pump without opening tanker valve
Ammonium nitrate emulsion
Investigation ongoing. Ruptured disc replaced and spilt material used in rework process.
Driver has closed discharge valve before pump has stopped causing the inline burst disc to fail.
Ammonium nitrate emulsion
Investigation ongoing. Maintenance and clean up carried out of small amount of spilt product.
Vehicle incident (1 incident) Mobile processing unit was left parked outside of a locked security compound overnight.
