LETTER FROM THE CEO

To the members of the board, valued partners and supporters, respected community, and the individuals and families under our care:

Here at Jawonio, a major transformation is in progress, and I’m proud to report that we are well on our way to seeing it through.

As a whole, we’ve spent the last eighteen months taking stock. Among other activities, we’ve performed an all-encompassing evaluation of our internal infrastructure, resulting in a full-scale reinvention of organizational accountability and capacity. We’re aiming to eliminate any divisional segregation by centralizing all our programs, business units, and operational functions.

We have now turned our attention to transforming the individual experience at Jawonio, enabling integrated and intelligent care. Our ultimate goal? A seamless and coordinated system that runs through all programs and spans a person’s lifetime.

We still have a sizeable journey ahead. The transformation is a five-year strategy, and includes—among other things—the development of a revitalized campus housing diagnostic care, behavioral and wellness therapies, physical therapies, and even primary care. Integrating and coordinating these services in one centralized location will enable our care to be both strategic and holistic.

Fortunately, we are in a wonderful community at a wonderful time, and we’re on track to complete this transformation successfully and on schedule. As you know, the regulatory landscape has recently broadened to support the managed care model for our community. We now have new access to funding sources, including Medicare and private insurance companies.

In addition, we’re still Rockland County’s No. 1 employer of people with developmental disabilities and special needs. Our businesses are self-sustaining, not only employing our own adults, but also generating notable profits. We are able to reinvest these profits into the people at their core: the employees. Profits support their competitive wages and benefit packages and create a true pathway toward full community integration.

For over 65 years, we’ve been at the forefront of innovation for the physically, behaviorally, and developmentally challenged. This transformation is designed to keep us there, helping all of our people in the most visionary way possible. As always, we need continual assistance from you in order to see our vision through.

SINCE 1947, JAWONIO HAS SERVED PEOPLE WITH A BROAD RANGE OF MEDICAL AND DEVELOPMENTAL CHALLENGES. We were one of the first healthcare agencies in the country to help those with disabilities and special needs. In short order, we became known for being at the forefront of care, and for serving children and adults no matter what their diagnosis—even the most severe.

Through the decades, we’ve expanded, adding new services, facilities, and campuses. Along the way, we’ve received many notable honors and awards, including the Rockland Business Association “Pinnacle Award” for Outstanding Achievement by a Non-Profit; Agency of the Year, “Doing the Most Good Award” from the Salvation Army; “Agency of the Year” for Behavioral Health Services, given by National Alliance on Mental Illness (NAMI) FAMILYA of Rockland County, as well as countless staff recognitions and service awards throughout the region. When we added the first for-profit business in the late 1990s, the dream of community integration took a giant step closer to reality. Through it all, our mission has remained steadfast: to provide broad-based solutions for the people we serve and be a place of respite for these individuals and their circles of support.

As the nation’s healthcare landscape moves toward managed care and broadens to encompass people with disabilities and special needs of all types, we find ourselves able to lead the way into what is— for us—familiar ground. Today, we continue to meet the needs of the whole person, even as individual needs shift and change. In 2013, we were honored to become founding members of the Accountable Care Organization of Greater New York, the first network in the country to include agencies serving people with disabilities.

Our work serves children and adults across their lifespan with physical, developmental, and behavioral challenges, remaining on the cusp of both new therapies and new ways of thinking about care. Crucially, we strive for excellence in every endeavor: a creed that starts with leadership and runs to every staff member, provider, and volunteer.

Making Great Strides at JawonioWhen it comes to our lifespan capabilities, Briana personifies young success. After Jawonio’s Early Intervention services in 2012, she joined Jawonio Preschool in 2013 and began receiving special education, occupational, speech, and physical therapy, making incredible process at every step.

Now, at age 4, Briana is beginning to walk independently—without her walker—and is making fast friends with her classmates, gaining confidence with every interaction. As she continues to grow, Jawonio’s lifespan services will be there for her evolving needs, helping her at every step of her journey.

With new nationwide policies in place, the healthcare environment is currently rising to meet a philosophy long-held by Jawonio: that care should be administered to the whole person, and not to a diagnosis or any other isolated aspect of his or her being.

Whether developmentally, mentally, or physically challenged, individuals are clearly much more than their diagnosis. Far from a mere category, they’re utterly distinct in mind, body, and spirit. They have unique gifts, and independent interests and personality traits. They come from different sets of stimuli, families, and experiences. They are uniquely persistent or hard-working or intelligent or athletic, and they learn at a pace entirely their own.

Clearly, this calls for the opposite of a one-size-fits-all solution. To administer just one therapy or service would be denying any student, patient, or other person the advantage of exploration, discovery, and play.

At Jawonio, our solutions run broad and deep. The broader they are, the more exposed our people will be to their own possibilities for wellness and leading a full life. The more exposed, the more likely they are to be assisted by a new therapy, or to discover a new joy, skill, talent, or breakthrough. By encouraging this functional learning, we help our individuals gain a newfound sense of inclusion in their communities.

This is the philosophy at the heart of Jawonio’s programs. And the transformation we are looking forward to advances our philosophy one more step: consolidating, centralizing, and coordinating all support, care, and assistance.

Wellness Gateway: CENTRALIZING CARE

Early 2015 will mark a full-scale transformation in the way we deliver our services. Rather than receiving new individuals through one of many segmented services or programs, we’ll be bringing them in them via a single unified entryway known as the Wellness Gateway. This radical shift will allow individuals and their circles of support to take full advantage of the entire breadth of our services with centralized oversight.

A Care Manager sits at the forefront of this new service delivery model and will meet the individual and his or her circle of support. The Care Manager will learn about each service desired or needed, and potentially recommend other services. She will select a treatment team, and together, Care Manager, and treatment team will formulate a plan.

The Care Manager then oversees the plan, contacting and coordinating with each provider or therapist, and tracking overall progress. The Wellness Gateway will always be open to the individual and his circle of support, and afford him the opportunity to review, access, and determine the most optimal care possible, whether within or outside of Jawonio’s system.

Gaining Independence and Paying it ForwardStacy has weathered tough times and severe challenges, only to emerge stronger, healthier, and happier, thanks to Jawonio’s lifespan services. Moving from a troubled household to the Arthur Moskoff Home at Jawonio after high school, Stacy benefited from the heartfelt care of her house manager, who made her feel at home.

Now 35 and a productive Teaching Assistant, Stacy has served as a proud model for others, working through Jawonio’s work center for over 13 years. She passed an 18-month training program with flying colors. She loves connecting with children, seeing them grow, and helping them overcome their own struggles.

Jawonio has always believed in caring for the whole person—not the disability. It’s in that spirit that we’re proud to shift from a discreet service provision, which focuses only on the current identified service request, to centralized lifespan services that grow, adjust, and adapt over years as the individual’s needs evolve; this approach recognizes each person’s strengths, preferences, and goals, working towards true independence by integrating individuals into the community and celebrating their contributions. Our programs will become even more seamless, providing holistic treatment and encouragement to our constituents as they progress through their various stages of life.

Jawonio proudly assists our neighbors to live with dignity and independence through every step of their life’s journey. From helping our youngest with early intervention services and integrated preschool classes, to our six-week summer education program, to welcoming them into our myriad of adult services, employing them in our rewarding social enterprises, and always being mindful of their emotional and physical health, Jawonio is committed to enriching the lives of those we support throughout their lifespan.

Jawonio has made a number of recent advancements that reflect this shift. We’ve rolled out a unified intake document protocol that makes it easier for social workers to access crucial information; we have consolidated our Medicaid Service Coordination and Care Management departments, creating efficiencies and enabling better response times to support populations with behavioral and intellectual and developmental disabilities (IDDs). Jawonio has also introduced a Mobile Crisis Unit to serve community members with IDDs and behavioral health issues.

Revitalizing our Campus: BRIDGE TO THE FUTURE

By the year 2019, our main campus at 260 N. Little Tor Road will be completely revitalized, broadening access and housing a variety of needed services. This increases efficiencies both for the people we serve, and for Jawonio, operationally.

The new campus will contain a full-service healthcare facility, space for our Day Habilitation and Employment services and a specially designed space for our innovative Behavioral Health and Wellness Center. Jawonio is currently seeking funding from New York State for our expansion. We have received notice of our selection as a priority project. Even if we do not receive the total funding we need to effect changes that will allow us to to centralize administrative functions for greater efficiency, and build a home for our programs that support a person’s growth in an integrated service environment. This new regional plan puts us on a fast-track to providing even greater services to the community at large. If chosen by the Governor’s committee this will mean more funding to assist development.

THE JAWONIO HEALTH CENTER As healthcare continues to evolve in the wake of the Affordable Care Act, the Jawonio Health Center will look to significantly increase our patient base and services by aligning with local and regional health facilities, making an impactful difference in our community members’ lives. The Jawonio Health Center will be one of our signature service deliverables as we shift to sustainability within the changing healthcare environment. Our center is now the location for a number of our most important clinics, including primary care, psychiatry, audiology, dental, psychology, podiatry, gynecology, speech, and occupational and physical therapies.

Starting in 2014, the Jawonio Health Center began integrating processes and staff to serve our community as one business; no longer are our specialties siloed and fragmented. We’re also well on our way to implementing a modern electronic medical record (EMR) system, enabling us to seamlessly share information with other health care providers.

New York State has selected Jawonio as a priority project, giving us a potential fast-track to providing even greater services to the community at large.

JAWONIO SERVICES

Our residential community of adults continues to grow and prosper. In line with the recent initiative by New York’s Office for People With Development Disabilities (OPWDD) to transform the workforce, Jawonio has been a leader in securing jobs for our residents. Committed to providing them a life of independence and dignity, our Community Living Services (CLS) emphasizes professional development and career training now more than ever.

Our CLS division has made significant advances recently, leading a managerial training program for mid-level managers and developing a career ladder recognizing

increased levels of competence among our direct support professional staff and a choking prevention initiative. We have also created a unique CLS Behavioral Health Team, a collection of highly skilled clinicians working together to develop and maintain their expertise in handling the challenging behaviors of individuals applying for vacancies.

Jawonio day programs are open to the entire community, not just residents. Our Direct Services team works with all individuals involved in our day services to develop beneficial behavioral strategies. In our commitment to continually

Jawonio continues to set a high standard of service in our early childhood education initiatives, comprising the Infant Development Program, Universal Pre-K, and our Summer Education Program. With satisfaction surveys and referrals reflecting the rewarding success of these efforts, we’re expanding and evolving these essential services.

Our preschool program has opened two new full day classes, replacing two half-day classes, in accordance with our mission to serve children with more intense challenges. Our education center has also begun offering private pay preschool, enabling us to offer fully integrated classrooms and an enriching environment for all children.Generous gifts from our community have allowed us to provide vital transportation

raise our quality of care, all on-campus members of the Psychology Department are now trained and certified in positive intervention methods to assist our program participants in learning how to control their negative behaviors. Thanks to our in-depth staff training on behavioral plans and psychological disorders, Jawonio has seen a significant decrease in the frequency and severity of dangerous behavior, leading to a safer community.

Jawonio is also exploring the possibility of providing specialty services, such as psychiatry, to similar populations beyond our designated catchment area.

In Jawonio’s commitment to meet all our children’s needs, we’re continuing to integrate more medical and clinical services into our early education and care: we’ve recently initiated a parent/child group for infants with hearing impairments and a monthly Developmental Pediatric Clinic, with plans to open a Psychiatry Clinic for children in 2015.

In August 2011, Jawonio launched the PROmiSe (Personalized Recovery Oriented Services) program, licensed by the New York State Office of Mental Health. Designed to help individuals with psychiatric, emotional, and mental challenges identify and work towards specific goals, we now offer over 120 groups per week and services in psychiatry, nursing, social work, mental health counseling, art therapy, psychology, family therapy, alcohol and substance abuse counseling, pre-vocational opportunities, and work preparedness. From assisting with employment, to attaining new abilities, to gaining independence from substances, the PROmiSe program holistically integrates treatment, support, and rehabilitation towards self-directed aspirations—whatever they may be.

For many years, Jawonio has experienced enormous success employing developmentally, physically, and behaviorally challenged adults in our document management services and The Jawonio Cleaning Company. Thanks to the profits we generate from this business, we are able to offer competitive wages and comprehensive benefits for all employees. This allows the employee to provide for him or herself—within and outside our community housing.

In a move that reflects Jawonio’s increasing centralization of resources, PROmiSe is now simultaneously graduating its first participants and co-enrolling the first individuals from existing residential and day services. Admissions have increased 150% over the last two years, while our evening programs now boast 20 individuals—up from five. Over the next year, we aim to increase enrollment in PROmiSe by 10-15%, helping more of our community members realize their dreams.

Over the last 15 years, 4,500 individuals have been placed in competitive employment. Businesses throughout Westchester and Rockland are partners in this effort, and we plan to expand our employment services into Orange County in the fourth quarter of 2014. Our employment training program continues to provide robust support for our members, including on-the-job training from supervisors and employment counselors.

Over the last year, Jawonio ‘s Document Imaging business forged a new partnership with eBizDocs, an award-winning service provider of electronic document management. Jawonio will be launching a new pre-employment service called Pathways to Employment this fall, in conjunction with the New York State Office for People with Developmental Disabilities. Other new activities include expanding our cleaning company’s work at government facilities, creating a minimum of 16 new jobs for veterans and the disabled, and, for the first time, into commercial cleaning, as well. We’ll also be opening a thrift shop in Yonkers for prevocational training and launching more job development programs.

The PROmiSe program takes great pride in our diverse staff consisting of clinical professionals with backgrounds in music, art therapy, and performance. Once our participants see these talents, they are inspired to develop their own. After a few productive group sessions, the idea for a talent show quickly evolves and flourishes. The first show garnered so much enthusiasm, passion, and growth that it’s now become a quarterly event. The show has been performed for other groups, and has sparked incredibly positive reviews throughout the community.
