JOB DESCRIPTION

Job Title: Dementia Connections Co-ordinator

Hours: 37 per weekFixed term contract until 31st March 2017

Postholder reports to Operations Manager

Posts reporting to Dementia Connections VolunteersPostholder:

Dementia Connections is a time limited service providing encouragement and low level support for a person with dementia , encouraging them to pursue activities of their choice and to engage in new ones within the community.

Duties and key responsibilities:● To receive and acknowledge referrals to the service

● To make initial contact with clients following referral and prior to assessment

● To visit clients in their own home

● To write to the client, health and social care professionals and other voluntary agencies confirming the start of intervention within given timescales

● To communicate with those clients on the waiting list on a weekly basis

● To accompany clients who have yet to be matched with a volunteer to help them pursue their preferred activities

● To provide information or signpost to other local support services as appropriate

● To work effectively and collaboratively with relevant statutory and voluntary agencies as appropriate

● To assist in the recruitment, induction, training and supervision of volunteers

● To record and maintain client database and statistics as required and provide reports as necessary

● To assist in the monitoring of the service, including service user satisfaction, and contribute to its evaluation

● To gather the views and opinions of people with dementia and carers to understand whether needs are being met and to inform any developments of this service

● To develop and maintain and up to date resource library to include information sheets, publications, videos, DVDs etc

● To comply with the data protection regulations, ensuring that information on clients remains confidential.

● To be responsible for personal learning and development, and to support the learning and development of others and the whole organisation.

● To work collaboratively with fundraising and media and campaigns teams, sharing responsibility for the achievement of jointly agreed objectives, as appropriate.

● To work in a manner that facilitates inclusion, particularly of people with dementia

● To carry out any other duties consistent with the responsibilities of this post as requested from time to time by the Team Leader

All staff, directors and volunteers are required to work within Age UK Essex policies and procedures, ensuring they are carried out in relation to their job and responsibilities

The post may involve occasional evening or weekend work for which time off in lieu will be given

Skills and experience Application InterviewPersonal or work experience in a care environment ●

An understanding of dementia ●

An understanding of the needs of people with dementia and their carers

Good time management skills ●

Good listening skills

Experience of leading a team

Experience of managing & motivating volunteers

Knowledge of relevant legislation including the Health & Safety Act and Mental Capacity Act

Knowledge of the Essex, Thurrock & Southend Dementia Strategy

Experience of working with statutory and voluntary agencies ●

An understanding of the need for client confidentiality ●

Basic IT skills including Word and Excel ●

Experience of managing the learning and development of others

An understanding of collaborative working * ●
