Simply put, neuroplasticity is the capacity for your brain to change and grow. Do you remember learning how to change a tire? Learning how to use a smart phone for the first time? Learning how to use a new TV remote with a million buttons? This is evidence that neuroplasticity happened–your brain changed, grew, and created new pathways to get the job done. Exercise can induce neuroplasticity and can affect how dopamine is used in the brain, strengthening the brain-to-body connections. When you exercise, and work hard during exercise, you create the perfect conditions so that your brain can reinforce useful movement patterns. As people say: exercise is the pill you don’t take.

How often do we need to exercise? The more the better. General recommendations are to do some form of cardiac or aerobic exercise 5-7 times a week: at least 150 minutes of moderate intensity exercises or 75 minutes of vigorous intensity exercises. Muscle strengthening, such as resistance or weight training, should be done 2-3 times a week. Balance and flexibility should also be incorporated into your exercises 2-3 times a week. If you don’t know where to start, talk to your doctor to get a prescription for physical therapy. A good general website to reference is Go4Life at www.go4life.nia.nih.gov.

Because Parkinson’s disease affects sensory input and motor output there are changes in posture, walking, balance, and coordination. Exercise is key to prime the brain and body to reinforce proper movement pathways that are used less due to PD. With these kinds of exercises, dopamine stays in the synapse longer, the dopamine receptors are more sensitive, and your brain uses dopamine more efficiently. The best way to prevent neurodegeneration and to keep your body moving is to exercise at an intense level, working hard. Over time, the brain shows evidence of neuroplasticity because your posture is taller, walking is smoother, and balance is better.

Johnathan Lessin, MD was diagnosed with Parkinson’s disease at age 38 during his career as a cardiac anesthesiologist. In 2018 he authored a second book entitled, Tenacity: A Physician’s Struggle with Parkinson’s Disease. Lessin shares life experiences, challenges, and victories. He describes the link between exercise, Parkinson’s, and neuroplasticity in an intimate and profound way.

“Exercise, in and of itself, is both beneficial and monumentally important for people with Parkinson’s disease. This is a disease of rest, not intention. It tends to want to lock you in a stiff, unbalanced prison, but with the proper motivation, you can actually break free of the chains of Parkinson’s disease. The more you move, the more you can move.”

“The more you move, the more you can move.” Simple, difficult, and true! In other words, you get good at what you practice. If you spend most of your day sitting, transferring from sitting to standing will be challenging because you have only practiced that movement a few times a day. If you change your routines and habits to move and walk more, the sit-to-stand transfer will become stronger and easier because it is used more often.

The more you practice moving a certain way, the more automatic the movements become. This is different for everyone, but it means that you need to challenge your body to learn something new. Maybe you need to enroll in a ballroom dance class. Try a HAPS Tai Chi or exercise class. Learn indoor rock climbing. Train to hike up Guadalupe Peak. Or, you start in a chair, with focused thinking about how you move to stand up: make your body move big to the edge of the chair, position your feet properly, lean forward, stand up, and then give yourself feedback on how you did. Could you do anything bigger or better?

Neuroplasticity means you have the capacity to change your brain. Everyone’s journey is unique, but using exercise as a way to stimulate neuroplasticity, promote movement, and prevent neurodegeneration should be a key connecting factor in your understanding of PD. Make an easy start into activities you love, jump back in, or take on a new hobby for the first time ever. Science and exercise are on your side. You get good at what you practice.
