Decision Assist

New program enhances pallia ve care for older Australians Health professionals and aged care staff working in residen al and home care aged services can now access Decision Assist, a new program to improve their capacity to deliver pallia ve care to their older residents, clients or pa ents. Funded by the Australian Government, Decision Assist is a mul faceted program of services and ac vi es that supports health professionals and aged care staff to develop their knowledge and skills about pallia ve care and advance care planning prac ces.

From the CEO It is with great sadness that I write this final message to you, as I will be moving on from my posi on as Chief Execu ve Officer on 13 August.

It has been an absolute joy and honour to work in this amazing sector. I con nue to be astounded at the dedica on and passion shown by those who work each day to ensure quality care at the end of life for all and would like thank everyone who has supported me in this posi on. There have been so many highlights during my four years at PCA that I couldn't possibly list them all here. Pallia ve care has become well supported as a mul par san issue which has been humbling to witness, and to see first hand the ground breaking work that has happened on the interna onal stage, culmina ng in the World Health Assembly adop ng a resolu on on pallia ve care for the first me it its history, has been wonderful.

I truly believe the founda ons have been laid for transforma onal change in our sector.

PCA Update Vale, Tristan It is with sadness that we share the news that Tristan Heaton died peacefully at home at the end of July. You will remember Tristan’s great smile from our Na onal Pallia ve Care Week posters this year, and for those of you who were able to join us , the passion and honesty of his mother, Maria, who spoke at our parliamentary breakfast during Na onal Pallia ve Care Week. We are grateful that carers such as Maria open their hearts and share their stories in order to raise awareness and improve understanding of pallia ve care for others. I am sure you join all of us here at Pallia ve Care Australia in extending our sympathy to Maria and Fabian, their family and loved ones.

Call for Syringe Drivers Professor Margaret O’Connor, Emeritus Professor of Nursing at Monash University, is visi ng pallia ve care services in Chengdu, China in early September and is seeking to contact Australian pallia ve care services that may have syringe drivers they are able to donate. With the phasing out of Graseby syringe drivers in Australia and the introduc on of alterna ves, these useful devices would be welcomed by pallia ve care services in China. Australian services with spare devices should contact Margaret as soon as possible to organise the dona on via [email protected] or on 0417 372 331.

These services include: The Decision Assist website The Decision Assist website – www.decisionassist.org.au ‐ is now ‘live’ and is the central hub of informa on about Decision Assist resources. Visit the website today for informa on about how to register for workshops, the Advance Care Planning Phone Advisory Service, and the full range of programs that Decision Assist will provide. The Advance Care Planning Phone Advisory Service One of first ini a ves to be offered through Decision Assist is the Advance Care Planning Phone Advisory Service – a dedicated telephone advisory service for advance care planning in Australia. It’s available seven days a week from 8am – 8pm. For advice about advance care planning, call 1300 668 908 now. The phone advisory service provides informa on, resources and mentoring regarding advance care planning. This includes assistance with advance care direc ve paperwork, informa on about state/territory advance care planning legisla on and referral to specialised resources. It is staffed by specially trained professionals who can give advice about any aspect of advance care planning, including case specific discussion. Workshop for aged care professionals Workshops specifically designed for aged care professionals will be delivered by experts in the field. They cover the range of processes involved in mee ng pallia ve care needs – from how best to ini ate the important conversa ons that need to be had, to developing an appropriate advance care plan, understanding legal frameworks, and implemen ng a comprehensive model of pallia ve care. Future resources Other resources, such as a Specialist Pallia ve Care Phone Advisory Service, a mobile applica on based on standard clinical prac ce guidance, workshops and educa onal resources for General Prac oners and support to build be er links between aged care, General Prac oners and specialist pallia ve care services, will follow in the coming months.

Dying to Know Day Are you prepared? The second Dying to Know Day (D2K Day) takes place on Friday 8 August. There are over 50 events being held across the country to celebrate the Day, you can view a complete list of them here. If you can’t a end an event in person don’t worry, there are three online events, including a short film premiere, that you can also par cipate in. D2K organisers, the Groundswell Project, are also encouraging people to log personal ac ons describing what contribu on they will make to learning more about death, dying and end of life issues. the PINwise ini a ve comes as a relief to people with demen a and their carers,” Mr Samuel said.

“Memory loss is one of the common symptoms of demen a, and without the op on of a sign only card, the PINwise ini a ve could bring further barriers for people living with demen a to par cipate in everyday ac vi es.” Banks will implement their own eligibility criteria for customers seeking to obtain a sign only card. “There are going to be some func onal differences with the sign only cards between the banks. For example, some sign only cards may not be able to be used at Automated Teller Machines,” Mr Samuel said. “I urge people with demen a and their carers to consult their banks about what process are in place for obtaining a sign only card as the processes will differ between banks.” Alzheimer's Australia has further informa on about the responses of the major banks to the needs of people with demen a. Heart Founda on New app helps pa ents to stay on their meds Hundreds of thousands of Australians who don’t take their heart medicines as prescribed can now be er manage their dosage and rou ne through a new Na onal Heart Founda on smart phone app.

The free My Heart, My Life app will help people keep track of and remember to take their medicines, while also providing health stats, enabling people to learn about heart a ack warning signs and offering healthier recipes.

The Heart Founda on’s Na onal Director of Cardiovascular Health, Dr Rob Grenfell said the app will help pa ents manage their medica on rou ne, increase their understanding of their condi on and ul mately improve their health.

“We know the app will assist GPs, pharmacists and primary care nurses in ge ng their pa ents to adhere to medicine and lifestyle changes,” Dr Grenfell said.

“Around 3.7 million Australians are living with cardiovascular disease ‐ the number one killer of men and women.

“We know the more medica ons people take, the more difficult it is to remember to take them.

“People can use the app to search a database for medicines they are taking, which lists the dose and frequency for each medicine and sets reminders to help people to take them.”

A Heart Founda on survey showed 5.5 per cent of people are prescribed blood pressure medicines and 16 per cent of people on cholesterol medicines are not taking them regularly.

“This equates to around 600,000 people on cholesterol and blood pressure medicines alone,” he said.

“Even more alarming is the fact that approximately half the pa ents prescribed a cholesterol lowering drug, commonly known as a sta n, will stop taking it within six months of star ng their medicines.”

PCEHR Consulta on Survey The Department of Health has launched an online survey to seek feedback from healthcare professionals, consumers and so ware vendors about the Personally Controlled Health Record. The survey follows a review of the PCEHR earlier this year which made 38 recommenda ons about how to make electronic records more effec ve.

The survey focuses largely on the recommenda on that the PCEHR should transi on to an opt‐out model from next year. To par cipate in the survey, click here.

Na onal Missing Persons’ Week 3—9 August The links between demen a and missing people are highlighted in Na onal Missing Persons’ Week. With one in 15 people in Australia over the age of 65 suffering from moderate to severe demen a, the need for that awareness is paramount.

The Na onal Missing Persons Coordina on Centre (NMPCC) says carers should consider registra on in a safe return program of demen a sufferers who may be at risk of wandering or going missing. Other safeguards include an ID bracelet or other iden fica on.

The centre has also developed a range of merchandise specifically targeted at carers and demen a sufferers, available here.

Pallia ve Care WA Farewell Amanda Execu ve Officer, Amanda Bolleter, is leaving PCWA to take up a project role with the Pallia ve Care Network at the WA Health Department. In her 15 months with the organisa on there has been some excellent work done, including developing a new strategic plan and priori es for the organisa on, holding important events like the Annual Commemora on, Pallia ve Care Breakfast and pallia ve care conference and the review of ‘Pallia ve Caring at Home’ a resource for carers which will be launched at the PCWA conference on 31 October. Na onal Standards Assessment Program

NSAP is celebra ng the publica on of its first peer reviewed journal ar cle! Suppor ng service change in pallia ve care: a framework approach was published in the Interna onal Journal of Pallia ve Nursing and explains a Change Management Framework developed in collabora on with CareSearch and PCOC which was piloted na onally with the pallia ve care sector. Background Pallia ve care services are increasingly iden fying areas for improvement, then trying to create appropriate changes in response. Nurses in par cular are o en expected to take leading roles in quality improvement (QI) but are not necessarily trained or supported in these processes.

Methods A framework approach to change was developed to guide services through a change cycle and delivered via workshops by representa ves of three Australian na onal projects. Par cipants were predominantly nurses (80%), with the majority (63.7%) over the age of 50.

Findings The workshops and the framework were posi vely evaluated, with par cipants feeling confident in a number of QI‐related ac vi es following workshop training.

Conclusion Recognising and addressing problems in clinical prac ce and service delivery is an important way for nurses to ensure quality care for pa ents; however, they need support in developing the skills and knowledge that are essen al to successful QI ac vi es. Sector News Caring for a loved one with a life‐limi ng illness: new factsheets available To help carers iden fy and find support, CareSearch is producing and distribu ng a set of informa on sheets that focus on carers and families. There are now three factsheets available. The first asks ‘Are you a carer?’, the second discusses how carers are not all the same (254kb pdf), and the third factsheet focuses on grief and loss (204kb pdf). Breast Cancer Network Australia Hope & Hurdles Breast Cancer Network Australia’s (BCNA) Hope & Hurdles is a comprehensive informa on pack for women diagnosed with secondary breast cancer. It was developed through extensive consulta on with women living with secondary breast cancer and the health professionals who treat and care for them.
