What do I pay?The HACC program seeks a contribution from clients toward the cost of services provided that is fair and affordable and in accordance with the WA HACC fees policy, which can be located on the HACC WA website. Your capacity to contribute to the cost of the support received is based on your level of income. This will be discussed in confidence during assessment.

Home and Community CareJuniper’s Home and Community Care (HACC) program provides a range of services delivered with compassion and respect to older people who require assistance to remain living safely in their own homes.

The care and services we deliver places great importance in maintaining personal dignity, wellness, independence and quality of life. Juniper HACC also offers support to carers.

EligibilityYou may be eligible for Juniper’s HACC services if you:

• areolderand/orfrailandhavedifficulty with everyday tasks;

• haveadisability;or• arethecarerofafrailolderperson

or someone with a disability.

Clients can receive assistance from our support workers to keep up with essential activities outside the home, such as bill paying, banking and shopping.

Good Life ClubFor residents in the north metro area, our staff plan and facilitate a range of activities which are conducted at the ‘Good Life Club’ located in Karrinyup. Activities are catered to clients’ needs and interests and also include some group excursions held away from the centre. If required, transport can be provided to and from the centre.

Respite*Respite service option provides carers with the ability to maintain their own wellbeing and take a break from their caring role, whilst a support worker will provide assistance either in or outside the home.

Other food services* We offer assistance with meal preparation and cooking and can give advice on nutrition, menu planning and storage, or food preparation.* These services are currently not offered

in the Busselton region.

Juniper offers a range of personalised HACC services designed to help meet individual needs:

Domestic assistanceJuniper can support you at home and assist with everyday household tasks. We can also help you keep up with essential activities outside the home, such as bill paying, banking and shopping, on your behalf.

Personal careOur trained Community Support Workers can provide support with daily self-care activities.Juniper’s personal care includes assistance with: • showering; • toileting; • grooming; and • medication prompts.

Social supportWe can help individuals to engage in social activities and participate in their community. Juniper offers a variety of assistance ranging from one-on-one, small group outings by car, bus group outings* and Community Chat to connect up to 10 people at a time via telephone link up.
