accommodation, such as a business, recreation, lodging, entertainment, or transportation facility that is open to the public in Illinois.

3. Illinois Department of Human Rights, Housing/Real Estate Transactions Complainant Information SheetDescription: This form is used to file a claim of discrimination in the sale or rental of residential or commercial property in Illinois.

4. Idaho Nez Perce Tribal Court, Civil ComplaintDescription: These forms are used to file a civil complaint with the Nez Perce Tribal Court in Utah. These forms and instructions will be
