program when the Courts and all parties agree the youth is unlikely to attain

Family & Children’s Behavioral Health Programs

Target Population Level of Care

Treatment Focused Services Youth up to the age of 19 (with full scope Medi-Cal) who have been referred to the Probation Department for low-level offenses but are not on formal probation supervision.

4.2 hours per child/month4.5 months length of service

James Ranch Behavioral Health Services

All youth residing at James Ranch in need of Behavioral Health services.

4.5 hours per child/month8 months length of service

Juvenile Justice Support and Enhancement Services (SES)

Probation involved youth with serious emotional disturbance and at risk for out-of-home placement, up to age 18 referred by JPD with Full Scope Medi-Cal. This is a community-based and family-centered program.

8 hours per child/month9 months length of service

Juvenile Justice Pro-GripGang involved youth, in need of behavioral health services, up to age 18 referred by JPD, with Full Scope Medi-Cal.

8 hours per child/month9 months length of service

Multisystemic Therapy

Youth ages 10 to 14 who have committed a serious offense; are at-risk of out of home placement due to anti-social or delinquent behaviors; and are involved with the justice system. This is a community-based and family-centered program.

17 hours per child/month (24 hours/7 days a week crisis response)5 months length of stay

Therapeutic Foster Care Youth that would benefit from stabilization in a therapeutic foster home which would provide support to transition to a permanent plan (one slot funded by Probation)

Daily support and from foster parents 6 months

TARGET POPULATION

Pro-CSR

• Youth with Full Scope Medi-Cal, 13 to 21 years of age, who are exiting the Juvenile Rehabilitation Facilities (James Ranch); exiting Juvenile Hall following a commitment of more than 60 days, or exiting a Foster Care Placement (Short Term Residential Therapeutic Program or similar foster care environment), and who are considered moderate or high risk offenders.

JPD Aftercare

• High risk youth with Full Scope Medi-Cal exiting Juvenile Hall or the James Ranch, program will also accept youth with one year post JPD dismissal, and with Full Scope Medi-Cal insurance up to 21 years of age

Ranch Reentry and Wraparound

• High risk youth under the age of 18, with complex needs, existing the Ranch

•Youth who are (or are in the process of becoming) dependent wards of the Juvenile Justice Court, and who have full-scope Medi-Cal.
