1.0 Event 1 - Reported lack of architect’ progress

1.1 Response – Formal letter

30th August 2014 Our Ref: COP-DES-001 Chris Steve Design Director Architect PO Box 0000 Project: Copham Hospital Subject: Lack of Progress Dear Sir, It has been recorded by our side that there is a slow progress from the architect side, which resulted in supplying late the required information needed by the structural consultant, OS and services engineer. The delays in providing the required information affected negatively the design programme. As per the approved design programme, the design works lay on the critical path, which means any delay in the design works will affect directly the hospital occupancy date that planned to be on 27-March-2017. Concept Project Management Ltd. requests that you submit a recovery programme to accelerate your design to recover the delays in order to meet the approved programme dates and to provide the required technical information required by other professional teams on time.

Moreover, Concept Project Management Ltd. requests that;

• You attend the weekly meeting to discuss the progress of design and highlight the significant issues in order to be resolved at the earliest.

• You submit a weekly a progress report that includes the work programme update and executive summary that show the progress during a week period. Also the staff mobilization schedule has to be included to show us the status of your staff’s mobilization plan.

2.0 Event 2 – Appearance of a large hole in an adjacent field

2.1 Response – Press Release

Copham Hospital and Mineral History

The Copham Hospital Project’s team got to know about the large hole that has appeared in the adjacent field to the new community hospital site and the subsequent public worries and queries about the effect of Copham hospital on the mining history/future and the safety of the hospital building which will be built on a mining area.

Based on our responsibility towards the community and as part of our commitment towards our corporate social responsibility, this message is being written in this local newspaper to the whole community in order to release some information and evidences that will prove and enhance our social commitments.

An integrated geotechnical study was performed before purchasing the hospital land to ensure that the construction of Copham Hospital will not affect the mining history of the area and the project site doesn’t contain any minerals or other geological materials. Also the safety of the hospital buildings has been taken into consideration from the engineering point of view by designing the hospital to be rested on bored piles in rocks to avoid the risk of constructing the hospital on hidden mining holes.

For more commitment towards the community, a project web site www.cophamhospital.com is available now to let the community members know about all project aspects and to get their queries answered online. Also Copham project’s news and event can be seen on the social media such as; face book and twitter.

3.0 Event 3 – Groundbreaking ceremony timed out to suit board chairman’s diary

3.1 Response – Email Dear Sir, I got to know that you are planning to organize the ground-breaking ceremony after starting the piles work in the week 5 of starting the contractor mobilization. I would like to draw your attention to some factors that will make organizing the event in week 5 not possible. I will list below these factors in brief;

• In week 5 according to the national weather forecast, heavy rains will be expected during the whole week.

• I contacted the guests (officials, prominent business people, senior management persons and media) who will attend the ceremony to find out the most suitable time for them and I found that most of them are free to attend by end of week 3 rather than week 5. Kindly find attached a list that contains the suitable date for each guest to attend the ceremony.

• The ceremony has to be held before the start of piling works. It is more convenient to have enough space to do the ceremony especially if a lot of participants are expected to attend plus the need for more space for fixing the raised platform.

Based on all the above reasons, I would recommend changing the ceremony date to week 3 after completion of the mobilization works. I look forward to hearing from you. Regards, PM

4.0 Event 4 – Injury of a trespasser to the site

What: Falling off the roof

When: 22 January 2016

Where: Copham Hospital Project Site

Incident Summary

On 22 January 2016, at around 20:00 pm the mobile crane collapsed during the lifting of heavy equipment for the plant room. The mobile crane collapse caused a subsequent collapse to the roof slab as the carne was positioned near to the building’s slab.

A proper lifting plan was in place and the 85-ton crane was set up correctly as per the inspection report. The crane started to shift up 6 types of different equipment to the roof successfully. However, during the 7th shifting up, the crane boom reached the 90 degree angle to the crane bed and before the crane operator began to lower down the equipment on the roof slab, a loud noise was heard followed by crane overturning on the slab.

The crane operator escaped from his cap door without any injuries but unfortunately some children came into the site and one of them was seriously injured because he was very near to the collapsed slab.

Analysis of what went wrong

• During investigation, it was proved that there was no mechanical fault in the crane and it was positioned correctly.

• The cause of incident was due to extending the boom by the operator outside the allowable limits, which was mentioned in the lifting plan.

4.2 Event Commentary

• Instruct the contractor to hire experienced crane operator/supervisor.

• Restrict the entering of children to the site by installation of more safety barriers.
