Implant Therapy in the Periodontally Involved Patient: Aesthetics and Function

About the Speaker: Dr. Diego Capri - Italy

Dr. Capri graduated with honors in Dental Medicine from the University of Bologna in 1996. In 2001 earned the Certificate of Advanced Graduate Study in Periodontology after a 3-year program at Boston University - Henry M. Goldman School of Dental Medicine. In 2002 passed the exams of the American Board of Periodontology becoming a Diplomate. He is an active member of the American Academy of Osseointegration. He has published several articles in the fields of periodontology and implantology in international journals and lectures extensively both nationally and internationally. He is the author of the chapter titled “Soft Tissue Management Around Dental Implants” in Practical Periodontal Plastic Surgery edited by Blackwell Munksgaard. From 2007 to 2008 he was the faculty in charge of the implantology course for the post graduate master in fixed prosthodontics at the University of Bologna. He currently practices as a Periodontist in Bologna.

Course Description:

Implants are often used in adults to replace missing teeth lost due to periodontal disease. In this vast category of patients several authors have, in the last decade, reported a decreased survival of osseointegrated implants and have warned us about an increase in the prevalence of biological complications, when compared to non-periodontally susceptible patients. Nonetheless, conflicting data can be found in the literature and some research groups are reporting good success rates in periodontally susceptible patients. This lecture will address this topic highlighting which factors seems to be more relevant in influencing the success versus failure of implant rehabilitations in periodontal patients. Functional and aesthetic considerations will also be examined as to how they play a role in the treatment planning of these often complex and challenging cases. This lecture is intended for all implant surgeons and restorative dentists.

At the end of the lecture the participant will have gained insights on:

• Determining if a periodontal patient is a candidate for implant therapy and how to prepare these patients for such therapy

• Evaluating and minimize “risk factors” that may influence long term success of implant therapy

• Properly handling the peri-implant mucosa during surgery in order to improve long term implant health

Course Description:

Digital Planning in implant surgery is used for implant placement. A digital work-flow has been developed so as to mill the bone block manually and to screw it on the host site with a tunnel technique. Potential advantage of this innovative work-flow is to maximize clinical outcomes and improve new bone formation. The progress in imaging and three-dimensional reconstruction are at the base of the evolution for this type of treatment option. The placement of bone grafts using the tunneling technique is common however provides significant advantages to the patient. The use of digital technology provides new tools for planning, simulation and execution of these minimally invasive techniques. This lecture is intended for all implant surgeons and restorative dentists.

At the end of the lecture the participant will have gained insights on:

• Potential of the tunnel technique and the related digital work-flow for bone grafting

• Advantages of cortico-spongious versus spongious block grafts

• Proper graft preparation and positioning

• How to maintain optimal vascularization

Case Presentation Description:

Recent technological and clinical advances are transforming the practice of implant dentistry. The inability of certain historical implant connections to provide a harmonious biologic response will require deviations from the ideal treatment plan to improve the final restorative result. A contemporary implant system should have the ability to improve the aesthetic outcome by maximizing bone support and enhancing soft tissue aesthetics. In addition, it should offer a wide variety of prosthetic options to address various clinical situations.

TiLobe Connection in Full Arch Restoration

About the Speaker: Dr. Marco Josch - Germany

After graduating, Dr. Marco Josch has participated as an instructional assistant at several implant courses and this knowledge is constantly expanding. From 2006 to 2007 he completed a curriculum implantology at the University of Cologne (Prof. Dr. J. E. Zöller). Dr. Josch is a member of BDIZ, DIZ and DGOI. In 1998 he founded a dental practice in Langenhagen near Hannover and has since worked as a dental practitioner concentrating on implantology, dental surgery and aesthetics. Dr. Marco Josch has used different implant systems successfully for many years and provides them with the corresponding prosthetics from his dental laboratory.

Case Presentation Description:

The European full arch treatment concept was developed to provide edentulous and soon-to-be edentulous patients with an efficient and effective restoration that uses only four implants to support immediately loaded, fixed full-arch prostheses. By tilting the two posterior implants, longer implants can be used in reduced bone volume, increasing bone-to-implant contact and reducing the need for vertical bone augmentation. As tilted posterior implants can be anchored in better quality anterior bone, cantilevers are reduced, improving support for the prostheses. Tilting also reduces the need for bone augmentation. It’s an approach that is less invasive and cost-intensive for the patient than conventional implant restoration (based on more implants), more efficient for the clinician and, most importantly, is clinically validated to work.

Case Presentation Description:

“Teethris” is the concept that helps to manage complex cases. All the patients’ medical and dental data are like units in Tetris and can be used by Decision Support System. We can integrate different materials, techniques, existing teeth, dentures, and implants by using evidence-based medicine in order to gain the best possible result. A well prepared diagnosis is fundamental in the planning process and can simplify the choice of the treatment. The comfort zone is what every patient needs. We can achieve it if we connect patients’ expectations, practitioners’ experiences and capabilities of the art of dentistry.
