By signing this form, both parties are agreeing to work together to meet the goals laid out in the plan discussed above. Let it be known that if improvements in performance agreed to above be not met in a timely manner, further action may be taken by management to ensure that the patrons of the Glen Oaks Community College Fitness/Wellness Center are given the best service possible. Action may include but is not limited to reduction of hours or termination from lab tech position. By signing this document both parties are agreeing to these terms.
