LABORATORY DATA REVIEW CHECKLIST CS Report Name: 2012 Confirmation Sampling, Well Installation, and Groundwater Sampling, 4501 Lake Otis Parkway, Anchorage, Alaska Date: July 2012 Laboratory Report Date: 7/11/2012 Consultant Firm: Shannon & Wilson, Inc. Completed by: Jake Gano Title: Environmental Engineer Laboratory Name: SGS Environmental Services, Inc. Work Order Number: 1121984 ADEC File Number: 2100.38.511 (NOTE: NA = not applicable; Text in italics added by Shannon & Wilson, Inc.)
1. Laboratory a. Did an ADEC CS approved laboratory receive and perform all of the submitted sample
analyses? Yes / No Comments:
b. If the samples were transferred to another "network" laboratory or sub-contracted to an alternate laboratory, was the laboratory performing the analyses ADEC CS-approved? NA / Yes / No Comments:
2. Chain of Custody (COC)
a. COC information completed, signed, and dated (including released/received by)? Yes / No Comments:
b. Correct analyses requested? Yes / No Comments: Changes to the COCs included analyzing samples PP10-2 and PP10-10 for PAHs; removing PAH analysis for sample PP2-4; and analyzing three samples which were submitted on “hold” for GRO/BTEX, DRO, and VOCs.
3. Laboratory Sample Receipt Documentation
a. Sample/cooler temperature documented and within range at receipt (4° ± 2° C)?
Yes / No Comments: Sample cooler temperature blanks were 6.5° C and 6.2° C, respectively.
Work Order Number: 1121984
32-1-17172-014, 4501 Lake Otis Parkway, Anchorage, Alaska Page 2 of 6
b. Sample preservation acceptable - acidified waters, Methanol-preserved VOC soil (GRO, BTEX, VOCs, etc.)? NA / Yes / No Comments:
c. Sample condition documented - broken, leaking (soil MeOH), zero headspace (VOC
vials)? Yes / No Comments:
d. If there were any discrepancies, were they documented (e.g., incorrect sample
containers/preservation, sample temperatures outside range, insufficient sample size, missing samples)? NA / Yes / No Comments: The lab noted that the methanol did not completely submerse the soil for Samples B9-2, B9-22, and B9-11. The septa lid jar for Sample PP8-6 had the wrong time. The laboratory used the notation on the sample lid to confirm the correct sample ID and used the time on the COC.
e. Data quality or usability affected? Explain.
Comments: Based on conversations with the lab, the soil samples where the methanol was not covering the soil sample were swirled and the soil was settled back into the jar (likely due to shifting during transport). The data usability is not affected.
4. Case Narrative
a. Present and understandable? Yes / No Comments:
b. Discrepancies, errors or QC failures noted by the lab? None Noted / Yes Comments: Surrogate recoveries were outside of QC criteria for several samples either due to matrix interference or sample dilution. LOQs for DRO were elevated for several samples. LCS recovery for chloromethane and vinyl chloride are outside QC criteria (biased high). MS/MSD RPD for vinyl chloride does not meet QC criteria (biased high). For Sample B9-22, PCE results are outside of the lab calibration range. Reanalysis was not conducted due to a lab scheduling error. CCV recovery for several analytes do not meet QC criteria (biased high).
c. Were corrective actions documented? None Noted / Yes Comments:
d. What is the effect on data quality/usability, according to the case narrative? Comments: The case narrative notes that LOQs are elevated for samples run at dilution. Also, the results for B9-22 are considered an estimate.
Work Order Number: 1121984
32-1-17172-014, 4501 Lake Otis Parkway, Anchorage, Alaska Page 3 of 6
5. Sample Results
a. Correct analyses performed/reported as requested on COC? Yes / No Comments:
b. All applicable holding times met? Yes / No Comments:
c. All soils reported on a dry-weight basis? NA / Yes / No Comments:
d. Are the reported LOQs less than the Cleanup Level or the minimum required detection level for the project? Yes / No Comments: The detection limits (DLs) exceed the ADEC migration to groundwater cleanup level for the following analytes in the following samples:
Vinyl chloride in all of the project samples. It is noted that the VC DL is less than the interim threshold concentration.
Methylene chloride, 1,2-dibromoethane, 1,2,3-trichloropropane, and 1,1,2,2-tetrachloroethane in Samples B9-2 and STB2.
e. Data quality or usability affected? Explain.
Comments: The potential presence of these compounds at a concentration greater than the ADEC migration to groundwater cleanup level but less than the DL cannot be determined. It is noted that the limit of detection for vinyl chloride is less than the interim threshold concentration for the project samples.
6. QC Samples
a. Method Blank
i. One method blank reported per matrix, analysis, and 20 samples?
Yes / No Comments:
ii. All method blank results less than LOQ? Yes / No Comments:
iii. If above LOQ, what samples are affected? NA
Comments:
iv. Do the affected sample(s) have data flags? NA / Yes / No Comments:
If so, are the data flags clearly defined? NA / Yes / No Comments:
Work Order Number: 1121984
32-1-17172-014, 4501 Lake Otis Parkway, Anchorage, Alaska Page 4 of 6
v. Data quality or usability affected? Explain. NA Comments:
b. Laboratory Control Sample/Duplicate (LCS/LCSD)
i. Organics - One LCS/LCSD reported per matrix, analysis, and 20 samples? (LCS/LCSD required per AK methods, LCS required per SW846) N/A / Yes / No Comments:
ii. Metals/Inorganics - One LCS and one sample duplicate reported per matrix, analysis and 20 samples? N/A / Yes / No Comments:
iii. Accuracy – All percent recoveries (%R) reported and within method or laboratory limits? And project specified DQOs, if applicable. (AK petroleum methods: AK101 60%-120%, AK102 75%-125%, AK103 60%-120%; all other analyses see the laboratory QC pages) Yes / No Comments: LCS recovery for chloromethane and vinyl chloride are outside QC criteria (biased high)
iv. Precision – All relative percent differences (RPDs) reported and less than method or laboratory limits? And project specified DQOs, if applicable. RPD reported from LCS/LCSD, MS/MSD, and or sample/sample duplicate. (AK Petroleum methods 20%; all other analyses see the laboratory QC pages) Yes / No Comments:
v. If %R or RPD is outside of acceptable limits, what samples are affected? NA Comments: Lab sample numbers 3-12, and 15-20.
vi. Do the affected samples(s) have data flags? NA / Yes / No Comments: If so, are the data flags clearly defined? NA / Yes / No Comments:
vii. Data quality or usability affected? Explain. Comments: The analytes with LCS recoveries outside of QC criteria were not detected in the associated project samples. Therefore, no samples are affected.
c. Surrogates - Organics Only
i. Are surrogate recoveries reported for organic analyses, field, QC and laboratory samples? NA / Yes / No Comments:
Work Order Number: 1121984
32-1-17172-014, 4501 Lake Otis Parkway, Anchorage, Alaska Page 5 of 6
ii. Accuracy – All percent recoveries (%R) reported and within method or laboratory limits? And project specified DQOs if applicable. (AK Petroleum methods 50-150 %R; all other analyses see the laboratory report pages) NA / Yes / No Comments: Recovery was outside of QC criteria for GRO by AK101 for Samples PP7-5, PP8-6, PP9-3, PP8-7, PP10-2, PP10-5, PP2-3, PP2-4, PP2-6, PP3-6, PP5-6, AND PP6-4. Surrogate recovery for PAHs by EPA 8270D SIMS in Samples PP7-5 and PP10-2, and VOCs by EPA 8260B in Sample PP10-5 were also outside of controls.
iii. Do the sample results with failed surrogate recoveries have data flags? NA Yes / No Comments: Failed surrogates are flagged but the associated samples are not flagged in the laboratory report. The data are flagged in our report tables. If so, are the data flags clearly defined? NA / Yes / No Comments:
iv. Data quality or usability affected? Explain. Comments: The surrogate recoveries were outside of controls due to either matrix interference or sample dilution. The associated samples are flagged in the project tables.
d. Trip Blank - Volatile analyses only (GRO, BTEX, VOCs, etc.) [soil and water]
i. One trip blank reported per matrix, analysis and cooler? NA / Yes / No
Comments:
ii. Is the cooler used to transport the trip blank and volatile samples clearly indicated on the COC? NA / Yes / No (if no explain): One trip blank was submitted with each cooler
iii. All results less than LOQ? NA / Yes / No Comments: Although less than the LOQ, GRO were detected in STB1 (0.930J mg/kg) and STB2 (1.07J mg/kg).
iv. If above LOQ, what samples are affected? Comments:). Sample PP6-1 had a GRO detection within five times of the associated trip blank (STB1), and Samples B9-2 and B9-22 each had GRO detections within a factor of five of the associated trip blank (STB2). The remaining GRO results are either non-detect, or greater than five times the detected concentrations.
v. Data quality or usability affected? Explain. Comments: The GRO data are flagged as less than the detected concentrations for the three samples in our project tables. It is noted that these concentrations are less than the most stringent ADEC cleanup levels, and therefore the DQO failure does not affect the regulatory status of these samples. The remaining results are unaffected.
Work Order Number: 1121984
32-1-17172-014, 4501 Lake Otis Parkway, Anchorage, Alaska Page 6 of 6
e. Field Duplicate
i. One field duplicate submitted per matrix, analysis and 10 project samples? Yes / No Comments: Sample B9-22 is a duplicate of Sample B9-2, and Sample PP10-10 is a duplicate of Sample PP10-2.
ii. Were the field duplicates submitted blind to the lab? NA / Yes / No Comments:
iii. Precision – All relative percent differences (RPDs) less than specified DQOs? (Recommended: 30% for water, 50% for soil) NA / Yes / No Comments: RPDs exceed the DQO for PCE in Samples B9-2/B9-22 (68 percent) and cis-1,2-DCE, fluoranthene, and pyrene in Samples PP10-2/PP10-10 (up to 80 percent). For the other analytes which were detected in both samples, DQOs ranged from 0.6 percent to 24 percent.
iv. Data quality or usability affected? Explain. Comments: The PCE result for Sample B9-22 are within 3 times of each other and both contain significant concentrations of PCE. The cis-1,2-DCE failure in PP10-2/PP10-10 are likely due to the non-homogenous distribution of contaminants in the sample. Both results are considered useable for the purposes of this project.
f. Decontamination or Equipment Blank (if not applicable, a comment stating why must
be entered below) No decontamination or equipment blank used per the ADEC-approved work plan.
i. All results less than LOQ? NA / Yes / No Comments:
ii. If results are above LOQ, what samples are affected? NA Comments:
iii. Data quality or usability affected? Explain. NA Comments:
7. Other Data Flags/Qualifiers (ACOE, AFCEE, Lab-specific, etc.)
a. Are they defined and appropriate? NA / Yes / No Comments: Lab-specific qualifiers are defined on page 6 of the lab report.

NA Comments:
vi. Do the affected samples(s) have data flags? NA / Yes / No Comments: If so, are the data flags clearly defined? NA / Yes / No Comments:
vii. Data quality or usability affected? Explain. Comments: No.
c. Surrogates - Organics Only
i. Are surrogate recoveries reported for organic analyses, field, QC and laboratory samples? NA / Yes / No Comments:
ii. Accuracy – All percent recoveries (%R) reported and within method or laboratory limits? And project specified DQOs if applicable. (AK Petroleum methods 50-150 %R; all other analyses see the laboratory report pages) NA / Yes / No Comments: The GRO surrogate recovery for Sample B4MWM is outside QC goals (high).
iii. Do the sample results with failed surrogate recoveries have data flags? NA Yes / No Comments: The failed surrogate recovery is flagged, but the sample result is not flagged in the lab report. The result is flagged in our report tables. If so, are the data flags clearly defined? NA / Yes / No Comments:
iv. Data quality or usability affected? Explain. Comments: The reported GRO concentration for Sample B4MWM may be biased high, the GRO result has been flagged in the project tables.
d. Trip Blank - Volatile analyses only (GRO, BTEX, VOCs, etc.) [soil and water]
i. One trip blank reported per matrix, analysis and cooler? NA / Yes / No

ii. Is the cooler used to transport the trip blank and volatile samples clearly indicated on the COC? NA / Yes / No (if no explain): Only one cooler submitted.
iii. All results less than LOQ? NA / Yes / No Comments:
iv. If above LOQ, what samples are affected? Comments: No results above LOQ
v. Data quality or usability affected? Explain. Comments: No.
e. Field Duplicate
i. One field duplicate submitted per matrix, analysis and 10 project samples? Yes / No Comments: Sample B4MW11 is a duplicate of Sample B4MWD.
ii. Were the field duplicates submitted blind to the lab? NA / Yes / No Comments:
iii. Precision – All relative percent differences (RPDs) less than specified DQOs? (Recommended: 30% for water, 50% for soil) NA / Yes / No Comments: Toluene and ethylbenzene had RPDs greater than the DQOs, 150 and 120 percent respectively. Note that at least one sample contained estimated concentrations of these target analytes.
iv. Data quality or usability affected? Explain. Comments: No. The failed RPDs were for concentrations that are significantly less than the applicable cleanup levels, the data is considered usable for the purposes of this project.
f. Decontamination or Equipment Blank (if not applicable, a comment stating why must
be entered below) No decontamination or equipment blank used per the ADEC-approved work plan.
i. All results less than LOQ? NA / Yes / No Comments:
