    1.0 HOW TO USE THIS FORMULARY

    Medicines are arranged according to alphabetical order. Only medical practitioners from respective

    departments are authorised to initiate the prescription as listed in the formulary. For medicines where

    Department names are not specified, medicines are not confined to a specific department, but it shall be

    prescribed in reference to FUKKM system/group.

    Category of prescribers that are authorised to initiate the prescription for the medicine in this formulary is

    listed as:

    CATEGORY OF PRESCRIBER DEFINITION

    A* Consultant/specialist from disciplines related to the listed indication only

    A Consultant/specialist

    A/KK Consultant/specialist/family physician specialist

    B Medical officer

    C Paramedic staff

    C+ Paramedic staff doing midwifery

    Dental consultant, dental specialist and dental officers are also included in this category but only for the

    medicines used in dental treatment. Medicines should be prescribed when they are necessary, and in all cases

    the benefit of administering the medicines should be considered in relation to the risks involved.

    Medicines will be dispensed generically. The use of trade name (if any) in this formulary is only given as an

    example; it does not indicate that the particular brand should be dispensed.
