Academic eligibility requirements: Students must have passed 5 classes for the year/1st semester. Students must have a 2.0 for the 2nd semesters/1st semester. Waivers are only available for the 2.0 requirement not pass 5. Waivers may be used one time during middle school years. 6. GRADING SCALE 7. INSURANCE: VBCPS is a supplemental insurance to your current provider Notify coach immediately if athlete is being seen by a doctor or emergency room due to accident during practice or game Claim forms may be issued by the SAC. Treatment for any injury must be received and the claim form filed within 90 days of the date of the injury. Plan provides 70% of Usual & Customary coverage $25,000 maximum benefit The deductible is $100 per injury. There is a $500 limit on physical therapy claims per injury. 8. Claim Form Example: 9. Athletic Trainer Lauren Crockett, ATC On site approximately every other day Inhalers/epi-pens must be given to coach for med kit. Self carry forms must be completed for inhalers & epi- pens and should state they may carry and self- medicate Emergency Cards will travel with team and should be completed this evening. 10. Parent/Spectator: The following are required of all parent/spectators: 1. Realize you represent LKMS, so encourage through positive behavior 2. Being a good sport is more important than victory: 3. Recognize and show appreciation for an outstanding play by either team. 4. Be a team fan as well as a my kid fan. 5. Remember that a ticket to a school athletic event is a privilege to observe the contest. 6. Treat visiting teams and officials as guests 7. Be modest in victory and gracious in defeat. 8. Respect the judgment and integrity of officials, realizing that their decisions are based upon game conditions. 9. Bottom line be a positive role model 11. Athlete/Parent/Coach Communication Appropriate concerns to discuss with the coach include issues of mental and physical well being, strategies for improving individual performance, and issues of behavior. Issues not appropriate for a parent to discuss with the coaches include playing time, starting positions, team strategy, play calling, and other athletes. It is NEVER ok to approach a coach after practice or competition to discuss any of the above. Please refrain and call the next day. 12. TICKET PRICES Football, Basketball, Volleyball, & Wrestling $2/$1 per regular season game/match All games/matches for playoff/championship will be $4/$2. Wrestling tournament is $6/$2. Coach may have a non mandatory morning practice. Important dates: 16. For Your Information: Any student receiving OSS will automatically be removed from the team. After 1st ISS student will not start next competition. After the 2nd ISS the team member will be removed We will provide transportation to away games. We ask that you allow your child to ride the bus back to LKMS from away sites. Only a parent/guardian may take a child from an away site. 17. More Important Info o Cancellations of games and practices will be made after 2pm. Please refrain from calling after the first rain drop/snow flake o There are few dates for make ups, so if at all possible we will play. o Be sure coaches have your email address and/or sign up for Remind 101 tonight o Uniforms are the responsibility of team members as soon as they take possession. Please help ensure we receive them back in the same condition they were issued. 18. Check out this Info Transfer rule: 6 or 8 games Students are eligible for 6 semesters, which begins when they enter 6th grade for the first time. CONCUSSION INFO: If your child is suspected of having a concussion during practice or a game and you are not present the trainer or coach will call you. It is mandatory that you come pick your child up, they will NOT be put on the bus. Please be available by phone while your child is practicing or competing. You will receive home instructions from person signing your child over to you. If appropriate your child's teachers will be notified of their condition. 23. MORE CONCUSSION FACTS: Your child will begin the Graduated Return to Play progression only after they are symptom free FOR 24 HOURS. There are 5 stages and each stage last a minimum of 24 hours. If at any time during the stage process they experience any symptoms, the process stops for 24 hours and they start with the last stage where they had no symptoms. 24. If symptoms resolve w/in 15 min: If the original symptoms resolve within 15 minutes of the incident AND the student has experienced NO other symptoms overnight the ATC can clear the student to begin the gradual return to play progression the day after the incident without seeing a physician. The gradual return to play progression is a minimum of 5 days of gradual progression before full return to play will be considered 25. If symptoms last longer than 15 min and/or return overnight: The student MUST see a physician There is a specific form that must be completed by the physician in order for the student to be considered cleared to start the 5 day progression This form can be obtained from the school ATC or SAC 26. REMEMBER These guidelines and activity restrictions are for any and all physical activity to include but not be limited to Physical Education Classes, Rec. games and practices, club sports - games and practices. Rules are in place to protect the student athlete As a parent help the student/athlete take time to get better Your support as parents is vital to the success of the program and safety of the students 27.
