On the cover: LDA members joined together in Baton Rouge on April 28 for Dentists’ Day at the Legislature. This is a grassroots event celebrating the dental profession that offers dentists the oppor-tunity to meet their lawmakers. The event began with briefings on legislative matters and bills of interest. Dentists then visited with their legislators at the Capitol prior to the afternoon reception at the Pentagon Barracks. This is one of many steps that the LDA takes each year in advocating for the dental profession. For more information on the LDA’s successful legislative session and photos, see pages 10-13.

What? What? What? Is it true that the LDA and NODA recently completed a

wildly successful, live, in-person New Orleans Dental Conference/LDA Annual Session. Say it ain’t so! I am happy to report that the aforemen-tioned conference did indeed take place the first week of May and it was well received by all. Attendance was great and the continuing educa-tion offerings were top notch. Kudos to all who were involved in the ever-evolving planning for the meet-ing, and also to all who attended to help make Louisiana’s dental meet-ing a success.

So, it appears that we may be well on our way back to a sense of normalcy, in the dental commu-nity, and in the world at large. People are anxious to have more face-to-face interactions with others and only use Zoom sparingly for a while. Patient after patient has eagerly told me of upcoming travel plans for the summer as the world seems to be opening back up again. And, actu-ally, as I write this, I’m traveling to Orlando with my family to spend a few days with Mickey and friends. Originally planned for October before the ADA Annual Session went virtual, it’s a chance for fun and some family time with kids that are getting older and whose sched-ules rarely match up now. And with the mandate of wearing masks while outside now lifted, it’s going to feel a bit more normal.

Normal. Now that’s something that we can all get used to again.

Has anyone else gotten tired of hear-ing the phrase the “new normal?” I’m not a fan. About 2 seconds after the pandemic began, the first of a bazillion articles came out about the “new normal” and how things would never be the same again. From school settings to work environ-ments to sporting events, everything was going to change. And not just for now, but for all time. Really?! Isn’t that a bit of an exaggeration? After being totally virtual for the

last nine weeks of school last year, my children were able to safely go to school this year. While in some workplaces work-ing remotely is something that can happen success-fully and people can still be productive, in many more settings people have discovered that the inter-action with customers and coworkers is something they crave and is neces-sary. Anyone tuning into a Major League Baseball game lately is likely to have seen a stadium full of fans now and not card-board cutouts.

Some of the measures and changes put in place during the height of the pandemic were necessary at the time, but not for all time. The New Orleans Dental Conference/LDA Annual Session was one of the first casual-ties on the calendar last

year followed by in person dental meetings across the country being cancelled, including the ADA meet-ing going virtual. Louisiana just showed the rest of the country (to paraphrase George Costanza) “We’re back baby!” Thanks for leading the way as we all try to get back to doing what we love to do with the ones that we enjoy being with!

Things are finally getting back to normal. I began my presi-dency on March 20 with the

first virtual House of Delegates at the LDA office. We are still doing Zoom but by a lot less than last year. Then on April 24, my daughter Calette got married. After months of plan-ning and stressing, especially during the pandemic, everything turned out great. Congratulations to Dr. and Mrs. Zach Lee. Needless to say, it has been an eventful year.

On April 28, we were allowed to have LDA Dentists’ Day at the Legislature. We had some great guests speak including Dr. Robert M. Laughlin, Dean, LSU School of Dentistry, who gave us an update on the Dental School and the Rural Scholars Track Program. Senator Bret Allain, chair of the Senate Revenue and Fiscal Affairs Committee, spoke to us about the legislative session. Representative Jerome “Zee” Zeringue, chair of the House Appropriations Committee, spoke about upcoming fiscal fund-ing. Dr. Charlie Foy also spoke about the Louisiana Mass Disaster Team, a program in honor of the late Dr. Robert Barsley. The LDA has asked the state for an appropriation to pay for the equipment for the Mass Disaster Team. Read more in the legislative report on pg. 10.

Several legislative bills are import-ant to dentistry this session.

HB387 is the LDA’s bill for trans-parency in network leasing by Rep. Kyle Green. Dentists inadvertently sign into contracts with dental plans

or leasing companies without under-standing they have agreed to a leasing arrangement that opens their office to both existing and new patients at a fee level that is below their usual and customary fee. Some insurance carriers lease their dental provider networks to other insurance compa-nies without the dentist’s knowledge or authorization. This results in lower reimbursements even though the provider never agreed to the new terms. This bill would allow dentists the opportunity to choose to not participate in leased networks.

HB 172 provides for Medicaid coverage of dental care for certain adults with developmental and intellectual disabilities by Rep. Rhonda Butler.

SB 82 is also the LDA’s bill and provides for review of dental insur-ance claims under the Health Insurance Issuer External Review Act (by Senator Stewart Cathey).

The LDA sends out alert texts and emails and you can contact your legislator through Voter Voice to request support on these important issues for our profession.

We were allowed to enter the Capitol, of course with masks on, and send notes to our Senators and House of Representatives members regarding legislation and invite them to the reception that evening at the Pentagon Barracks. It was a great time. Thanks to the LDA staff, the CGA committee, Pelican Partners (our lobbying team) and especially CGA chair Dr. Gizelle Richard for a job well done.

On May 6-8, the New Orleans Dental Conference/LDA Annual Session was held at the Hilton New Orleans Riverside. There was a lot of speculation on how every-thing would turn out because of the pandemic. However, with masks and social distancing in classes, every-thing turned out great. Everyone was excited about getting CE in person and seeing friends and colleagues. Thanks to the NODC chairman Dr. Vincent Dileo (also a director on the LDA Board of Directors), Dr. Keith Kyler (Chairman of the LDA Council on Annual Sessions), and LDA staff for another job well done.

On May 11, the LDA hosted a virtual signing day for LSUSD gradu-ating seniors. The event was hosted by Colin Zvosec, LDA Director of Membership. LDA Executive Director Annette Droddy and myself were able to explain the benefits of being a member of organized dentistry to the students. The graduates were also given information from Stormy Blair with Brown & Brown Association Services Professionals regarding malpractice insurance and other business office type policies. Also, speakers from the ADA spoke to the students about student loan refi-nancing, the ADA practice transition program, and information on licen-sure from the ADA.

Hopefully, you can see things are starting to get back to normal. Hope to see ya’ll at the LDA Summer Education Conference in Pensacola, Florida on June 23-26. Have a great summer!

The LA Department of Health updated its policies to allow dental providers contracted with Managed Care Organizations (MCOs) as professional services providers to bill for the administration of COVID-19 vaccines using a hard copy or electronic professional services medical claim form.

Shreveport Dentists Administering the COVID-19 Vaccine

Early this year, the LDA was hard at work prioritiz-ing COVID-19 vaccines for

dentistry. After successfully getting the Louisiana Department of Health (LDH) to designate dentistry as an essential health care service, the LDA worked just as hard to make sure Louisiana dentists are on the vaccine administrator list. LDA member, Dr. Jeffrey Kerst did something he never thought would happen in his dental career by administering the vaccine for the first time in April.

“Receiving the vaccine myself was one of the most relieving moments during the midst of the pandemic,” said Dr. Kerst. “So, it felt phenome-nal to be able to give the vaccine to my patients and potentially change the world.”

Before dentists could adminis-ter the vaccine, Dr. Kerst participated in hours of continuing education courses, hoping dentists would even-tually be authorized to give the vaccine. Once dentists were eligible, Dr. Kerst immediately partnered with the LDH and the Centers for Disease Control (CDC) to administer the Moderna vaccine at his dental office, Kerst & Caskey Family Dentistry in Shreveport.

“Since many people have already received the vaccine, we are primar-ily appealing to those who are on the fence of traveling due to the pandemic,” said Dr. Kerst.

The quarantine prevented Dr. Kerst from visiting his elderly family members for over one year, so he understood that many individuals

were keeping travel to a minimum. Administering the vaccine at his dental office created a sense of convenience by providing dental services and vaccines all in one place.

After one month, Dr. Kerst vacci-nated nearly 20 individuals and has more than 12 vaccine appointments

scheduled throughout June. Dr. Kerst is beyond grateful to be able to assist in vaccine administration and suggests that everyone consider getting vaccinated to remain moving in a direction of normalcy.

The 2021 regular session of the Louisiana Legislature adjourned sine die on

Thursday, June 10, with one of the final acts being approval of a $43 billion spending plan for the state budget for the fiscal year that begins July 1. The session was chock full of wins for dentistry and the LDA. Both LDA sponsored bills have already been signed by the Governor prior to the close of session. Here is the breakdown:

LDA Bills:

HB 387 (Act 26) by Rep. Kyle Green – This is the LDA’s bill requir-ing transparency in network leasing. HB 387 has been signed by the Governor and is effective 8/1/21.

The National Coalition of Insurance Legislators (NCOIL) adopted model legislation regarding network leasing. Network leasing by dental insurers has become a challenge in a number of states, including Louisiana. The LDA filed a bill that modeled NCOIL’s suggestions. The model act is all about transparency for the dentists who often inadvertently sign into contracts with leasing compa-nies and/or dental plans without understanding they have agreed to a leasing arrangement that opens up their office to both existing and new patients at a fee level that is below their usual and customary fees. The LDA introduced this model

legislation to bring transparency to network leasing and to provide consumer protection.

A full copy of Act 26 can be down-loaded from the LDA website, but a few parts of the law worth pointing out include:

• The contract specifically states that the contracting entity may enter into an agreement with a third party allowing the third party to obtain the contracting entity’s rights and responsibilities as if the third party is the contracting entity.

• If the contracting entity is a dental carrier, the provider chose to participate in third-party access

at the time the provider network contract was entered into or renewed, and for contracts with dental carriers, the dentist has the right to choose not to participate in third-party access.

• If the contracting entity is an insurer, the third-party access provision of any provider network contract specifically states that the contract grants third-party access to the provider network.

• The third party accessing the contract agrees to comply with all terms of the provider network contract.

• The contracting entity identifies, in writing or electronic form to the provider, all third parties in existence as of the date of the contract.

• The contracting entity identifies all third parties in existence in a list on its internet website that is updated at least once every 90 days.

• The contracting entity notifies network providers in writing or electronic form that a new third party is leasing or purchasing the network at least 30 days in advance of the relationship taking effect.

• The contracting entity requires a third party to identify the source of the discount on all remittance advices or explanations of

payment under which a discount is taken.

• The contracting entity notifies the third party of the termination of a provider network contract no later than 30 days from the termination date with the contracting entity.

• A third party’s right to a provider’s discounted rate ceases as of the termination date of the provider network contract.

• At the time the provider network contract is entered into or renewed, or when there are material modifications to a contract relevant to granting access of a provider network contract to a third party, a dental carrier shall allow any provider which is part of the carrier’s provider network to choose not to participate in third-party access to the contract or to enter into a contract directly with the health insurer that acquired the provider network. If a provider opts out of a network lease arrangement, this does not permit the contracting entity to cancel or otherwise end a contractual relationship with the provider. When initially contracting with a provider, a contracting entity shall accept a qualified provider even if a provider rejects a network lease provision.

• SB 82 (Act 89) by Sen. Stewart Cathey – This is the LDA’s bill adding the review of dental insurance plans under the Health Insurance Issuer External Review Act. Louisiana passed the Health Insurance Issuer External Review

Act in 2013, allowing individu-als to file claims and appeals for an independent review related to their medical insurance in the event they disagree with the cover-age decision. This act shall apply to external review or adverse determi-nations involving individual claims in excess of $250.

Many states who have this exter-nal review process also include dental insurance claims as eligible for this review process, but Louisiana did not. In fact, Louisiana Medicaid coverage already allows for indepen-dent reviews on dental AND medical claims. However, private insurance did not include dentists. The LDA, through an appro-priation to the LSU School of Dentistry, asked for and received a one-time allocation in HB1 of $100,000 to create Louisiana’s Mass

Disaster Team to have the equip-ment and supplies ready in case of emergencies. Mass disaster teams were used for the Pan Am crash of 1982 in New Orleans and during the aftermath of Hurricane Katrina in 2005. The LDA believes that this is a low investment for Louisiana to have a team prepared and available in case of a mass disaster. It is terri-ble to lose a loved family member or

friend. It is even worse not knowing what happened to them. This team can assist the coroners in giving the family closure.

Dr. Bob Barsley, who passed away in September 2019, was the go-to person in Louisiana on foren-sic dentistry. He was co-chair of the Odontology Section of the Scientific Working Group on Disaster Victim Identification funded by the National

Institute of Justice and the FBI. He was integral in both the Pan Am crash and Hurricane Katrina in forensic identification. With his death, this wealth of knowl-edge ended abruptly. Dentists of Louisiana would like to carry the torch in his honor!

The LDA also supported the very successful efforts of our Medicaid Task Force Chair Dr. Don Donaldson in advocating for increased rates per child and per unit for dental Medicaid hospital anesthesia. As many of you may be aware, dentists’ time in the OR in many of our state’s hospitals have been cut drastically over the last few years. Hospitals have told us that it is because of the much lower rates that they are paid for the dental Medicaid patients, and they simply cannot afford to have as many dental patients. Dr. Donaldson’s work in advocating for the additional payments to hospitals and his continued work to reach out to hospitals to inform them of this change is integral in the improve-ment of access-to-care in Louisiana. This successful endeavor was an incredible step in achieving a goal of putting dentists in a more equal position in reserving OR time. This

will make a huge difference to dental patients and create a totally differ-ent environment for dental cases in hospitals. It may take a few months to work out the kinks and imple-ment the new fees but the fees will be effective July 1, 2021.

Other LDA Priorities:

HB 172 by Rep. Rhonda Butler and Sen. Franklin Foil – Provides for Medicaid coverage of dental care for certain adults with developmental and intellectual disabilities. The bill has gone through the process and is waiting for the Governor’s signature.

HCR 34 by Rep. Rhonda Butler and Sen. Louie Bernard – Requests a study regarding potential means of financing dental care for adult resi-dents of intermediate care facilities.

A long-recommended proposal to untie Louisiana’s income tax collec-tions from federal tax payments won passage, sending the concept to voters for a decision on the October ballot. The proposal would get rid of personal income tax and corporate tax deductions for federal income taxes paid in exchange for lower-ing the state’s income tax rates. Louisiana also would permanently eliminate the corporate franchise tax for small businesses and lower

the tax rate for others. Sen. Bret Allain led the Senate tax debate. The last hurdle will be if Gov. John Bel Edwards vetoes individual pieces of the package that requires all measures to pass before it can take effect.

A Look AheadBy Dr. Robert M. Laughlin Dean, LSU School of Dentistry

Finding new ways to underwrite and strengthen dental education in Louisiana is a necessity today. As

most know, the budget challenges over the past two decades have taken a toll. Today, the immediate charge from the LSU Health leadership is clear. In addition to sustaining our four-fold mission of educa-tion, patient care, outreach and research, we must address the $8 million+ dental school budget deficit and develop a plan to significantly improve access to dental care for underserved adults and children in Louisiana.

The DeficitA multifaceted approach will be neces-

sary to decrease the deficit. Efforts now underway include:

• Decreasing annual expenditures by eliminating duplication and increasing efficiency;

• Increasing tuition revenue via out-of-state and foreign students (class size for Louisiana residents to remain constant);

• Increasing clinic income both on and off-campus;

• Expanding our general practice residency (GPR) and dental hygiene programs in the state.

The end goal—eliminate the debt within five years and increase reserves for future initiatives to benefit and strengthen our educational programs. Education is the first and primary mission of LSUSD. The other three mission-focus areas of patient care, outreach, and research stem from our educational mission.

Access to CareCentral to the expansion of dental

services for the underserved is the establishment of American Dental Association-accredited GPR programs in cities throughout the state within the next five years. LSUSD GPR programs already exist in New Orleans and Baton Rouge. Additional programs will be established

in Alexandria, Lafayette, Lake Charles, Monroe and Shreveport—with the first in Shreveport by year-end. The first phase of the effort also includes Monroe, which will be established within the next year. The second phase, over a four-year period, will include Lake Charles and Alexandria; the third, in year five, will include the Lafayette/Opelousas area.

GPR resident programs will be key to increasing access to dental care as most Louisiana citizens live or work within 60 miles of these more urban areas. To complement resident care, our DDS Rural Scholars Track (RST) students will rotate through the clinics to provide dental care to those in need. Practicing dentists in the communities will also benefit as GPR resi-dents are trained in providing care for more difficult to treat patients, such as those who are medically compromised.

Plans are also underway to establish new dental hygiene educational programs to help alleviate current and/or future professional shortages in the state. The first initiative will be in Alexandria in part-nership with Louisiana State University Alexandria. Approximately 15 students will be admitted each year. Program expansion to other areas may also be possible if the need is justified.

Encouraging some of our DDS grad-uates to live and practice in rural, underserved areas of the state is also a part of our efforts to address access to dental care. The goal is to have at least eight dental students enrolled in the program in any given year. Their base tuition fee will be unwritten by the interest from endowed funds. For every year of tuition waived, graduates are required to serve one year in a rural area. The philosophy will be to trade tuition dollars for service in rural communities. Service obligations would be one for one. The scholarship committee aims to identify students who are from rural areas and/or have plans to live and practice in a rural area long-term.

Maldistribution of dentists is the major issue facing Louisiana. Most are clustered in urban areas.

We also realize the importance of spreading the message early to youth who live in rural areas. The annual “Dash… into Dentistry” event, a partnership between LSUSD and the Central Louisiana Area Health Education Center (CLAHEC), is held in the spring to introduce high school students to opportunities in the dental profession. The program includes infor-mation on the admissions process for all three of our degree programs, salary expec-tations, group discussion and information about dental public health. It targets high school students from CLAHEC’s 17-parish service area that includes Ascension, Avoyelles, Catahoula, Concordia, East Baton Rouge, East Feliciana, Grant, Iberville, LaSalle, Livingston, Pointe Coupee, Rapides, St. Helena, Vernon, West Baton Rouge, West Feliciana, and Winn.

In Other NewsCelebrating our Graduates: The

pandemic has certainly put a strain on traditional graduations over the past two years. Although the joint LSU Health graduation ceremony was canceled this year, the individual schools were granted approval to celebrate separately at the Ernest Memorial Convention Center on May 13. Safety and social distanc-ing practices were in place. This year, the school celebrated with 112 graduates and their families—69 in dental, 39 in dental hygiene, and four in dental laboratory technology.

New Freshman Class: We look forward to welcoming our freshman dental students in July. The class size for the DDS degree program remains at 75 students. For the 2021/2022 academic year, 1,023 appli-cations were received—121 individuals interviewed, 88 accepted, and 75 registered to attend. Next year’s class is comprised of 31 males and 44 females with an average

age of 23.2. Sixty-one students are Caucasian, three are African-American, nine are Asian and two are Hispanic. Of the 75 accepted dental students, 56 are from Louisiana, four are from Arkansas and 15 are from other states. Overall GPA was 3.64 with an average science GPA of 3.52; the average Dental Admissions Test score was 21.6; the average chalk score was 6.54.

The dental hygiene program is equally competitive. Last year, 139 applications were received and 38 students were accepted into the program. The average science GPA was 3.47 and the average ACT score 23. The dental laboratory technology program is not as competitive and serves as a viable alternative for those interested in an oral health profession.

New GPR & OMS Clinics: LSUSD General Practice Residency and Oral & Maxillofacial Surgery (OMS) are enjoying new state-of-the-art clinics this year on Our Lady of the Lake Regional Medical Center’s campus. Prior to the move, the two clinics were oper-ating in separate locations. The new configuration will provide improved services for referred patients for both specialties. The clinic has 16 dental operatories, two surgical suites and two sepa-rate waiting rooms—one for each specialty. Three full-time faculty members are currently on staff for the new clinic—one director for the GPR program and two faculty members for OMS. Eight GPR residents, 24 OMS residents and six OMS interns will provide services at the site.

As we look to the future, please know that we are committed to sustaining and strengthening our educational programs in New Orleans and across the state. The support of the dental commu-nity, our legislators and the LSUSD Team will be instrumental in helping us achieve our future ambitions. Your continued support is valued and very much appreciated.

2021 is off to a great start for the Louisiana Dental Hygienists’ Association (LDHA). We had

our first live in-person CE for 2021 and it was amazing. Thank you to our sponsors, Dentsply Sirona and Orascoptic, LDHA members and Registered Dental Hygienists across the state for such a success-ful event. A special thank you to Myra Hebert and the Acadiana Hygiene Association component for all your hard work to make this live event possible during the COVID-19 pandemic. Due to the great

attendance of our spring event, LDHA is happy to announce our fall session for 2021. This year’s fall session will be Friday, November 12 at the Hilton Garden Inn in Bossier City. Eight clinical hours will be available. We look forward to another great CE event.

During our Annual Session in March, the LDHA elected new officers. I am proud to return as pres-ident for the 2021-2022 year. Tracy Jordan was elected Vice President, Leslie Cvitanovic was re-elected as secretary and Donna Sarabia

will serve another term as trea-surer. Our delegation committee for the state includes Senior dele-gate Joanna Peters, Junior delegate Donna Sarabia, and alternate dele-gates Yancine Eggers and Meghan Nettles. The delegation committee will represent LDHA at the ADHA annual conference in June. This year’s conference will be held June 18-20 in Phoenix, Arizona.

Our legislative team has been hard at work developing ideas to expand the role of the dental hygienist in the state. The LDHA

is currently in support of HB 172: Dental Care for Adults with intellec-tual and developmental disabilities. With the help of our lobbyist Liz Mangham, the LDHA is also looking to expand our role in clinical settings by changing our supervision to general supervision. General supervision would allow the hygienist to perform solo anesthesia as well as explore different career options outside of clini-cal dental hygiene.

Greetings colleagues! Since the last board update, it has been relatively quiet. The big item

requiring attention at the May board meeting was C.E. for 2021 renew-als. There appears to be plenty of live C.E. available for the remainder of the year. However, for the special-ists, there appears to be little relevant live C.E. for them since their main source of meaningful C.E. appears to be at their specialty national meetings. Most of those have been changed to remote meetings. They could get live C.E., but it would not be pertinent to their specialty and thus while meeting the letter of the law, it would not meet the intent. Also, for those with moderate seda-tion permits, live refresher courses for both pediatric and adults have been hard to find. While some of our general dentistry national meet-ings have been converted to remote, many have been changed to a new location due to some parts of the country, especially larger cities, are still in “shutdown” mode.

Considering these factors, and with the fuel supply issues and general uncertainty about the rest of the year, the decision was made to accept any mode of delivery for C.E. for the renewal cycle ending this year, just like we did for the renewal cycle ending 2020. Of course, live C.E. is still accepted. This applies to our hygienists as well so please spread the word to those who work with you and others who are friends and acquaintances. We anticipate that the renewal cycle in 2022 will

be unaffected and will require live C.E. in the normal amounts, which are for dentists at least 20 live and up to 10 remote, total 30, includ-ing 3 hours of live CPR all clinically related. So, it’s a good idea to get live C.E. this year especially if you are renewing in 2022. Also, for clarification, please note that ACLS and PALS must be taken live, just like CPR.

The American Association of Dental Boards recently decided to start a program of C.E. accredita-tion called ACE (similar to PACE and CERP by the ADA and AGD). After evaluating the criteria, the board felt that we could accept courses with the ACE designation for approved C.E. If you are inclined to follow C.E. news or want to provide C.E. in an area of your expertise, you might want to check out their website at www.dentalace.org.

When you are choosing C.E. providers, be aware you might see this designation and know it is an accepted course by our board, as long as it is clinical C.E., not practice management, etc.

Finally, there was a concern regarding sedation safety in the dental offices of general dentists and a consideration to require dentists to have the same level of sedation permit as anyone they use in their offices to provide sedation services (CRNA, Dental Anesthesiologist, moderate sedation permit holder) while they do the dentistry. Some

states have this require-ment, which pushes a lot of patients into the hospital setting. However, we do not have many hospi-

tals that allow private patients in for general

dental procedures since insur-ance rarely covers the extra hospital services and the Medicaid reim-bursement level is so low for the sedation administered for these dental procedures. The question was raised, if a sedation permit holder could do moderate or deep sedation in their offices by themselves with only their support staff, why would it be necessary to have the treating dentist hold a sedation permit to the same level? It was noted that CRNAs have far more training and expe-rience providing sedation services than most dentists holding a seda-tion permit. In the end, these points carried the day and no change will be made. As it stands now, if you wish to have a CRNA or other sedation permit holder provide the anesthe-sia services for you in your office (assuming the patient meets the medical criteria for safely doing so), you only have to be ACLS (adults) or PALS (kids) certified along with the proper monitoring equipment, etc.

Marine Veteran Gets His Smile BackWillie is a 61-year-old veteran who lives in New Orleans and takes care of his mother. He served in the Marine Corps from 1982-1989. After his service, he drove an 18 wheeler truck, and worked

from rheumatoid arthritis, HIV and has had two shoulder surgeries. Because of his health he was forced to stop working.

Willie also had oral health issues; many of his teeth

infections and abscesses causing pain and making

stipend and Social Security Disability, struggles to make ends meet and dental treatment was beyond his means. Thankfully, he found the Donated Dental Services (DDS) program and was linked with Dr. Frank Martello, a LA DDS volunteer and past president of Dental Lifeline Network • Louisiana. With Dr. Martello’s help, along with a volunteer laboratory—Willie received thousands of dollars in donated treatment that restored his dental health and smile.

Access to scientific journals during the COVID-19 pandemicThe role of the LSU Dental Library

During the COVID-19 pandemic lockdown, most health sciences librar-

ies were closed and unable to meet the information needs of the health professionals who seek journal arti-cles for research or patient care. However, the LSUHSC-NO librar-ies did not close and continued to provide access to library resources including databases, books and journals. The libraries have always participated in interlibrary lend-ing programs to provide resources to other libraries and this service became especially important during the lockdown.

The LSUHSC-NO libraries have a collection of books and jour-nals to support all six of the schools that comprise LSUHSC-NO includ-ing allied health, dentistry, graduate studies, medicine, nursing and public health. During the lockdown, we saw a 145% increase in requests from 2020 for journal articles, books and book chapters than the same period in 2019. From March – September 2020, we filled 3,416 requests for journal articles, book chapters, and entire books. The dental library collection alone filled at least 544 of those requests.

Normally, most interlibrary loan requests to LSUSD come from the libraries within the southeastern U.S. but libraries from all over the world found us while searching for an open library who would fill the requests from the dentists and other health

professionals they serve. The Dental Library provided articles to libraries on 5 other continents. Requests came in from as far away as Thailand, Australia, Spain, and South Africa. We sent articles and book chap-ters to international libraries such as Edinburgh University in Scotland, University of Oslo in Norway, Universitaria de Granada in Spain, University of Melbourne in Australia and the National and University Library in Slovenia.

In North America, we provided resources to Harvard University, Yale University, Johns Hopkins University, Stanford University, the Food and Drug Administration, the National Institutes of Health and the American Dental Association. The most requested titles were Dentistry Today, American Journal of Dentistry, Dental Update, Practical Periodontics and Aesthetic Dentistry and the Texas Dental Journal. Out

of the 12 most requested titles from LSUHSC-NO libraries, 5 of them were from the dental library.

The COVID-19 pandemic caused many changes in everyone’s daily life and workday, including that of the library staff at LSUSD. The LSUSD library staff had their hours on campus reduced for safety reasons, never expecting how our small school from a small state would affect the research and patient care process for dentists around the world. After a few emails, the staff immediately noticed an increase of items being requested from our small print collection, and our staff prioritized our new role in fulfilling these requests. The staff identified the significance of what we were able to do with our time on campus and how our collection can help researchers across the world. We were happy to do our part, however small in the grand scheme of things.

Evolution is a scientific fact, and aspects of life are constantly evolving. This change can be

slow, and adaptations are hardly seen. However, upon closer examination, changes for better chances of survival and better quality of life can be observed. The process of retirement planning has evolved as well.1 What was thought of as financial planning for past generations does not check all the boxes for today’s investors. The retirement landscape is changing. A comprehensive retirement planning process has evolved to better iden-tify investor motivation and utilize strategies to increase adaptability in changing financial environments. The comprehensive nature of the evolved financial planning process helps the investor have increased confidence and peace when facing the unpre-dictable and often volatile nature of the free markets, and also connect the numbers in a financial plan to the intangible joys of life.

An Evolved Financial Plan has a Vision Statement

A comprehensive financial plan with a CERTIFIED FINANCIAL PLANNER™ professional,2 should extend beyond numbers to help you determine your vision for your entire financial world, for your lifetime and your legacy. This may be referred to as a holistic plan, which can lead to an actual, physical vision statement for your financial world. The Vision Statement helps the investor gain clarity in his or her personal financial goals. These goals are then empow-ered by igniting passions within the

investor to develop a sense of life-time prosperity and abundance beyond numbers.

An Evolved Financial Plan is Fueled by Purpose

Once a comprehensive financial plan is put into action, then it should be consistently monitored. Periodic and timely updates should be made to reflect any major life changes or market factors.3 It has been said that retirement itself may increase your risk of depression by 40%.4 This may be because typical “work” often gives a sense of purpose to our days. A person’s “sense of self” can be tightly dependent on work. A holistic and comprehensive financial plan should include conversations about goals extending to individual passions and sustaining lifetime purpose.5 Research suggests having a purpose in life helps people to have reduced mortality risks (live longer) and promotes posi-tive aging. Goals are set that extends beyond tangible numbers and accu-mulating wealth, to intangible goals of passion projects and legacy that transform wealth into individualized, empowered prosperity.

An Evolved Financial Plan Considers Individual Motivation and Risk-Tolerance

Historically, investors focused on the fear of running out of money as the basis for retirement planning. Although maintaining an individu-al’s standard of living is paramount during retirement, the primary moti-vation during the planning process

has moved from fear to purpose. This should be based on the individual’s goals where the portfolio has a stated family index number based on risk tolerance. The Family Index Number is the long-term average rate of return that a family needs to try to meet over a market cycle to achieve their long-term goals.6 This is a forward-looking process that focuses on the individual family situation.

An Evolved Financial Plan Continually Assesses Change

The U.S. is experiencing a retire-ment surge due to the population of ages 65+ growing and living longer. Just as you wash your hands regu-larly, so you don’t get sick, it’s critical to adopt good habits

of ‘digital hygiene’ to prevent cyber attacks on your practice. The ‘illness’ threatening your practice is called malware. Malware is an umbrella term for any malicious software criminals use to steal your or your patients’ data.

Ransomware, a particularly sinis-ter malware, burrows into your system and begins encrypting all your data so you can’t access it. Then a cybercriminal holds your data for ransom, demanding you pay a large sum of money in order for them to give you access to your own files.

Just like a human virus can lie undetected, malware can be in your computer system long before you realize it. By the time you see symp-toms, it’s too late. Cybercriminals are continually developing sophisticated methods for infecting computers and servers without you catching on. There are two primary ways malware gets into your system and holds your practice ransom.

Hacking

Hackers secretly tap into your data by exploiting weaknesses in your IT security. Outdated, unmain-tained systems often make smaller, older practices particularly easy targets. Working with a proac-tive team of IT experts, known as Managed IT Services Providers or MSP, is an important layer of defense against attacks. These folks can save you money, time and head-aches over the long run. They detect threats early to eliminate or reduce

damage well before it gets out of hand.

A particular vulnerability is how you are using email. Only use Gmail, Hotmail, Yahoo, etc. for personal or non-patient specific messages. For anything beyond that, set up a fully HIPAA-compliant, cloud-based email system that protects your informa-tion whether it’s sitting in your inbox or sending to another doctor’s inbox. There are big differences between an encryption-only email for general security and a truly HIPAA-compliant email fulfilling every HIPAA secu-rity requirement. These requirements range from verifying recipient identity to making sure no email is altered.

Phishing

Phishing occurs when a crimi-nal tricks an employee into thinking something is a trustworthy source, then convinces them to click a corrupt link or provide sensitive information directly (like a credit card number). The attacker is preying on a lack of awareness on the part of

you or a staff member. You need to educate your whole team to recog-nize suspicious messages, links and questions to avoid falling victim. If the sender is unknown or claims to be your IT person, MSP or someone in your office yet asks you to click an unusual link, verify the email first with the actual person on your team.

No one is inherently immune from cyberattacks. Take action now by work-ing with a qualified dental IT services provider to assess, boost and maintain your IT immune system. Work directly with your staff to understand what to look for and how to prevent these types of criminals from getting in the door. Healing from an attack is much more difficult and costly than preventing it in the first place.

LAST PAGEIdeas That Have Been Set AsideBy David Austin

If you have been a longtime reader of this publication, you know very well that this

reporter is always proud to report the latest ideas and products that our LDA research laboratories have developed. I am proud of their dili-gent thought and engineering of devices that are not only helpful to the dental team and the deliv-ery of quality dentistry, but also to humanity in general.

Unfortunately, these 6 recently added products are being discontinued:

1) Zirconia Fake Fingernails: This is a product that actually worked too well – a fake fingernail that would not break or split. It could be produced and installed in the dental office using existing equipment. However, nail-biters were damag-ing their teeth and quite frankly, the Vita shades for nails never became popular.

2) The Measuring Tape Belt: Actually, a good idea. You wore this belt as part of the LDA diet program and you could constantly scan your waist size to help you in your weight loss regimen. However, several members purchased this for their spouses and, unfortunately, we know of 2 members that were rescued by their neighbors after hearing their screams for help after their angry spouses tied them up with these belts. Litigation is still ongoing.

3) Pet Leash and Umbrella Combo: Another idea that took nearly 3 years to perfect. How many times have you been walking your cat, dog, or goldfish and, of course, it starts raining? Living in Louisiana is indeed a tropi-cal experience many months of the year. While using this product with your pet along with our recent spate of bad weather with resulting high winds, several of our little loved ones have been observed flying off in the distance.

LDA has graciously contacted all the ADA components of all 50 states and several in Europe and the Middle East for the hope-ful return of these pets to their rightful owners. This product has been taken off the shelves permanently.

4) Wenger 16999 Swiss Army Knife: Currently out of stock on Amazon and not sure if it will ever return. No confir-mation from LDA labs if they had any of their engineering incorporated into this product, but several dentists have stated that they have performed crown prepa-rations and also milled the final product using this device. Unfortunately, actual approval for use in dentistry does not appear to be forthcoming.

5) LDA Fire and Carbon Monoxide Detector: This engineering marvel has been recalled and is being updated. It seems that during use and testing its alarm has gone off during UFO sight-ings. Our engineering staff is now trying to perfect this product to also detect any imminent alien abduction and keep you safe. However, a couple of our engineers working on this have failed to return to work in the last couple of months.

6) Note Holder (given out by LDS endorsed company TDSC): This incred-ibly engineered product actually holds 7 patents and can hold a lot of other things as well. In fact, we have been unable to manufacture enough supply, especially for

the Colorado market, so we have decided that producing this versatile appliance will take a sabbatical indefinitely so that we can get manufacturing up to speed. (Actually, used note holders are now selling for 3 times their origi-nal MSRP on the black market if you can find one.)

You may see some of these products in a newer, better form in the future. Let’s hope so! I’ll be reporting on improvements and updates to the entire LDA/LDS prod-uct line in a future article.
