Organization Overview

Franciscan Alliance

Headquartered in Mishawaka, Indiana, Franciscan Alliance is a trusted leader in providing faith-based, integrated health care. As a 12-hospital integrated delivery system, it brings together the latest technology, innovative procedures, and highly-skilled and compassionate employees to serve their patients in Indiana, Illinois and Michigan. Beyond their 12 hospitals, Franciscan Alliance also offers a number of nationally recognized Centers of Healthcare Excellence as well as more than a 1,000 providers employed through the employed medical group, Franciscan Physician Network (FPN). Fitch Ratings and Moody’s have affirmed the ‘AA’ and ‘Aa3’ ratings respectively for the System, both of which have been maintained since 1997. For more than 140 years, Franciscan Alliance has stayed true to its founding mission to care for everyone who comes through their doors. They treat their patients with the best possible care by following the guiding ethical values embodied by their founding congregation, the Sisters of St. Francis of Perpetual Adoration. Always mindful of their Christian stewardship to the Roman Catholic Church, they minister with joy, care, and compassion according to the ideals of St. Francis of Assisi and their foundress, Blessed Mother Maria Theresia Bonzel. Franciscan Alliance carries forth Christ’s healing ministry and strengthens the Catholic health care mission by: Providing a broad, coordinated continuum of health care services with an emphasis on improving the

health of persons and communities. Treating the mind, body and spirit with holistic and comprehensive medical options. Developing creative structures for healthcare delivery. Being advocates for those in need. Identifying and developing our sisters and laity for Franciscan leadership.

Candidates should feel comfortable working within a Catholic organization and will model the administrative and philosophical compatibility with the mission and values embodied by the Sisters of St. Francis of Perpetual Adoration.

History The selfless acts of St. Francis of Assisi – a man who left behind all his worldly possessions to follow Christ’s mission and care for those in need – set forth the guiding values under which Franciscan Alliance began. In 1875, a group of Catholic Sisters from Olpe, Germany answered a call to bring St. Francis’ ministries of health and education to the Midwest United States. Here, they opened the doors of their System’s first hospital, what is now Franciscan Health Lafayette East. Founded by Mother Maria Theresia Bonzel, the congregation later became known as the Sisters of St. Francis of Perpetual Adoration. The Sisters expanded the health system with more hospitals, schools, orphanages and homes for the elderly. And nearly a century after the first hospital opened its doors, the System was officially founded as the Sisters of St. Francis Health Services in 1974.

In 2010, their System recognized the evolving changes in the health care landscape and the need to reinforce the shared capabilities across their hospitals and other health care facilities. They adopted a new name – Franciscan Alliance – to strengthen their presence as a large, comprehensive health care system. In 2012, their health system was selected to participate in a Pioneer Accountable Care Organizations (ACOs) initiative by the U.S. Department of Health and Human Services. The ACO model is designed to provide more coordinated care for Medicare beneficiaries, ultimately resulting in cost savings for patients.

Mission and Ministry

Franciscan Alliance’s Mission - “Continuing Christ’s Ministry in our Franciscan Tradition.”

Each day, we commit ourselves to practicing the words of our mission in how we care for our patients:

• As health care providers, we continue the good works and maintain the values and traditions of the thousands of people who have gone before us.

• We model our lives and our work after Jesus Christ, a caring teacher and a gentle healer.

• In whatever the case may be - whether nursing a patient back to health or caring for them through the dying process - our system carries on His healing and teaching ministries.

• Throughout Franciscan Alliance, our physicians and all employees embrace and carry out our mission in every day’s work.

• Each day we embrace our Franciscan Tradition by walking in the footsteps of St. Francis, who left behind his worldly possessions to care for all of those in need.

Franciscan Values - Our physicians and employees follow the values established by our founding congregation, the Sisters of St. Francis of Perpetual Adoration. • Respect for life - The gift of life is so valued that each person is cared for with such joy, respect,

dignity, fairness and compassion that he or she is consciously aware of being loved. • Fidelity to our mission - Loyalty to and pride in the health care facility are exemplified by members

of the health care family through their joy and respect in empathetically ministering to patients, visitors and co-workers.

• Compassionate concern - In openness and concern for the welfare of the patients, especially the

aged, the poor and the disabled, the staff works with select associations and organizations to provide a continuum of care commensurate with the individual's needs.

• Joyful service - The witness of Franciscan presence throughout the institution encompasses, but is

not limited to, joyful availability, compassionate, respectful care and dynamic stewardship in the service of the Church.

future expansion to better serve the population. Franciscan Health Michigan City coordinates care in the right setting through the System’s first freestanding emergency room.

South Suburban Chicago Division: This division is comprised of Franciscan Health Olympia Fields after the consolidation of two hospitals in the south suburbs. The System consolidated all acute care services to its Olympia Fields facility to ensure the service area’s evolving healthcare needs are being met, reduce the overall cost of care provided, and eliminate significant duplication of costs between the Chicago Heights and Olympia Fields facilities which were only five miles apart. As part of this consolidation, the System is expanding surgical and endoscopy services, obstetrical and newborn specialty care services, and emergency capacity at the Olympia Fields facility; expanded the urgent care capacity near the Chicago Heights facility; relocated inpatient rehabilitation services to the Olympia Fields facility; and expanded physician office space at both campuses. This division specializes in heart and vascular services, endovascular services, interventional radiology, cardiovascular surgery, bariatrics, stroke care, orthopedics, joint and spine care, emergency medicine, gastrointestinal services, oncology, women’s and children’s services, diabetes care, rehabilitation services, home health, palliative medicine, and hospice services.

FPN allows for greater size and scope for participation in ACO’s and outcomes-based payer reimbursement programs. In 2017, FPN’s Merit Based Incentive Payment System (MIPS) performance placed it in the top 10 percent for overall quality attainment. Franciscan Home Care and Hospice Care – As part of its continuity of care activities, the System offers home care and hospice care services to its patients and has fostered integral working relationships with providers. The System’s home care service area makes it one of the largest providers of home care services in the State of Indiana. Franciscan Urgent Care – Franciscan Alliance operates 13 urgent care locations across Indiana and Illinois providing approximately 215,000 visits annually. Franciscan PACE – Franciscan Alliance was the first Program of All-Inclusive Care for the Elderly (PACE) service provider in the State of Indiana. In collaboration with the State of Indiana and CMS, PACE provides comprehensive medical and social services to certain elderly individuals most of whom are dually eligible for Medicare and Medicaid benefits. This allows elder patients the freedom to remain in the community versus in a nursing home as well as save both the state and federal government the expensive cost of nursing home services. Franciscan Alliance currently operates two PACE centers in Indiana. Franciscan Durable Medical Equipment (DME) – Since 1990, the System has operated its Franciscan DME and serves approximately 14,000 patients annually.

Franciscan Health ACO – Franciscan Alliance has over 35 years of experience working under full and partial risk reimbursement contracts and managing patients. The Community

About Indianapolis Nicknamed the "Crossroads of America" for the abundance of interstates and ease of access, Indianapolis is the 12th largest city in the United States and the second largest in the Midwest after Chicago. Indianapolis, the state capital of Indiana, is home to 1.6 million people and is one of the fastest growing regions in the United States. Indianapolis/Marion County is by far one of our country’s most livable big cities. Indianapolis has a quality about it that makes even the heart of a bustling downtown feel like home. Visitors are drawn from all over the globe to enjoy top-notch professional sports venues, a thriving convention industry and numerous cultural attractions. Lonely Planet named Indianapolis “Best in the U.S.” for 2015. Famous for racing, basketball and as Peyton Manning’s former home, Indianapolis is a beacon of the national sports scene. Yet for all of the big events of which it has recently played host – from the NFL Super Bowl to the NCAA Final Four – Indy has enjoyed a rise in profile without losing its Midwestern charm. Its city center is a Top 10 Best Downtowns with growing pockets of neighborhood nightlife in its nearby suburbs. Indianapolis has become more than just “The Crossroads of America” – it has become a destination for both for tourists and permanent residents alike. Communities surrounding Indianapolis offer great living options as well. Surrounding communities include: Carmel Located due north of Indianapolis, Carmel is perhaps the best-known Indianapolis suburb. Carmel (pop. 83,889) boasts a median household income of $103,607 and has recently undertaken massive public projects like the Arts and Design District and the Carmel Center of Performing Arts. Residents find plenty of things to do in Carmel. Major companies (CNO Financial Group, Delta Faucet and more) make national headquarters in the city and a $24.5 million Monon Community Center has expanded family fitness activity options. The center connects to the 15.6-mile Monon Trail/Greenway that travels as far south as the west side of Indianapolis’ downtown district. Fishers Another suburb to the north, Fishers has experienced extreme growth in the past two decades. Now rivaling that of nearby Carmel, Fishers offers many of the same benefits of its Hamilton County neighbor. Schools in Fishers are considered some of the best in the state. Fishers housing options range from apartment communities to multimillion-dollar properties that line the shore of the town’s Geist Reservoir. Fishers has a thriving arts and cultural scene. The nearby Klipsch Music Center draws top concert acts to the large open-air venue all summer long, while the Connor Prairie Interactive History Park recreates daily 19th-century Indiana life.

Brownsburg This Hendricks County town (pop. 21,285) shares a border with Indianapolis’ northwest corner about 20 driving minutes from downtown. It features top-notch schools that routinely score well above the state average in graduation. The tight-knit community has recently seen an influx of companies in the motor sports industry and looks primed for continued growth. To the east, Brownsburg borders Indianapolis’ Eagle Creek Park that offers a 1,400-acre lake for fishing and sailing plus 3,900 acres of forest for hiking, birding and even zip lines. Greenwood Greenwood sits due south of Indianapolis. It’s the home to the Greenwood Park Mall that anchors a sprawling shopping and dining corridor along US Highway 31. Recently, the city has grown its job base as a hub for warehousing and fulfillment industries that are taking advantage of its location along Interstate 65. Greenwood is also the closest Indy suburb to Southern Indiana destinations like Brown County State Park, as well as the sprawling Camp Atterbury that serves as the training base of the Indiana National Guard. Zionsville Perhaps the Indy suburb with the most small-town vibe thanks to its historic downtown, Zionsville (pop. 14,160) serves primarily as a bedroom community just eight miles from Indianapolis city center. Every school in the town’s public school corporation has received a Four Star designation, and more than 45 percent of the town’s households have children under 18. A Saturday farmers market in the historic downtown village has been popular for nearly 20 years. Westfield Continuing the theme of Indianapolis’ booming suburban growth, Westfield now serves as home to more than 30,000 residents to the north of Indianapolis. The city has been aggressive with annexation to build its borders and is currently engaging in a revitalization of its quaint downtown. Westfield is also nearing completion of Grand Park, a massive 400-acre youth sports campus geared to attract 1.5 million visitors a year. With urban shopping and dining to its south and rural areas to its north, Westfield offers something for any lifestyle.
